On July 14, 2011 a Compliance Inspection was performed by George Smith, of this office. Mr. Loren Shumate
was present for the inspection. This facility holds a NTPDES General Permit NCG020000, which covers the
discharge of stormwater for establishments primarily engaged in Mining and Quarrying of Nonmetallic
Minerals.
Permit
This General Permit became effective January 1, 2010. Please read the permit and insure that you are compliant
with all terms and conditions.
Part III, Section A - Stormwater Pollution Prevention Plan (SPPP)
The SPPP is kept in a folder. There is a separate 3-ring binder that covers Employee Training.
All stormwater outfalls must be evaluated for the presence of non-stormwater discharges as shown in the permit
per Part 111, Section A 1 (e). This must be done annually.
Part III, Section B — Stormwater Analytical Monitoring
Analytical tests, such as, Settleable Solids, Total Suspended Solids, and Turbidity are performed quarterly at
Outfall 001 by using approved analytical methods by a certified lab/person. These tests are only required semi-
annually.
Rainfall is measured and documented in a separate log. Sampling must be performed during a representative
rainfall event.

Part III, Section C — On -Site Vehicle Maintenance Monitoring
You must determine if the vehicle maintenance area uses more than 55 gal/month of new motor oil, when
averaged over a calendar year.
Analytical monitoring is performed quarterly at Outfall 001 for the following: pH and TSS.
The following semi-annual analytical monitoring was not performed at Outfall 001: Total Petroleum
Hydrocarbons (TPH). Please perform this test immediately as required per the permit.
These tests must be performed as shown in Table 4. On -site vehicle sampling may be relocated from Outfall
001 after the stream piping project is completed.
These tests may not be required if the new motor oil usage is 55 gal/month or less.
Part III, Section D - Qualitative Monitoring
The following discharge characteristics were performed at Outfall 001 as required per Table 6: Color, Odor,
Clarity, Floating Solids, Suspended Solids, Foam, and Oil Sheen.
The following three discharge characteristics were not performed at Outfall 001: (1) Deposition at or
immediately below the outfall; (2) Erosion at or immediately below the outfall; (3) Other obvious indicators of
stormwater pollution. Starting immediately, please add these three discharge characteristics in your qualitative
monitoring report.
Part III, Section E — Mine Dewatering & Process Wastewaters
Analytical monitoring is performed quarterly at Outfall 001 for the following: pH, Settleable Solids, TSS, and
Turbidity. This meets the permit requirement.
These tests are performed as shown in Table 7. Mine dewatcr sampling may be relocated from Outfall 001 after
the stream piping project is completed.
The process recycle pond had approximately 4-feet of freeboard. This meets the requirement of a minimum of
2-feet. After the stream piping project is completed this may meet the requirements for a closed -loop recycle
wastewater system.
Facility Site Review
The vehicle maintenance area contains various types of petroleum products in drums and storage tanks. They all
have secondary containment measures. This office encourages the facility to make sure the containment areas
are maintained.
There was a complaint received from the landowner at 2119 Claybank Creek. During the inspection there was
some residual sediment on the driveway at 2119 Claybank Road. Mr. Shumate indicated that a 3-inch rainfall
event caused the creek to rise. The pipe under Claybank Road was unable to carry the stormwater capacity.

There were no failures of the recycle or stormwater ponds. Increasing the capacity of the stormwater pond and
completion of the stream piping project should reduce the possibility of excess stormwater being delivered to
Claybank Road.
Effluent/Receiving Waters
The effluent to Claybank Creek, Class C Trout + waters of the State was clear at the time of the inspection.
There were not any adverse stream impacts noticed.
Actions Required
1. Please read your permit and make sure you are in compliance with the sampling requirements, new
motor oil usage, and adherence with the Tier actions that are required.
2. Please inform this office when you will start and complete the stream piping project.
3. Please inform this office when you will enlarge the stormwater pond and move the office and scale
house.
The above actions require a written response to this office within 15 days of receipt of this report.
During late October 2011 a compliance inspection will be scheduled so that we can determine the correct
location of the stormwater and process outfalls. Hopefully all the facility modifications and U.S. Army Corps of
Engineer permit requirements will be completed. In the meantime analytical and qualitative compliance will be
judged from the sampling performed at Outfall 001.
The compliance inspection is satisfactory pending a written response is sent to this office. If you have any
