Preventative Maintenance and Good House Keeping Statement
Mt. Airy Quarry
Preventative maintenance and efficient housekeeping are imperative to maintain the structure of our
Storm Water Plan. Preventative maintenance will include scheduled and unscheduled inspections,
qualitative and analytical monitoring of designated outfalls, corrective actions, and evaluation of
effectiveness. Many factors such as weather and unexpected incidents may increase or decrease the
need to alter preventative maintenance. Certain boundaries with in our storm water plan must
maintain good housekeeping practices, for example vehicle maintenance areas. Employees must act
with due regard to ensure every spill or release precaution is taken. Barriers and other devices are used
on a regular basis to develop standard practices.
Preventative Maintenance Schedule
Monthly -Vegetation removal during growing season
Quarterly -Inspection and Qualitative Monitoring, Vegetation and Sediment removal
Semiannual -Inspection and Analytical Monitoring, Vegetation and Sediment removal
Unscheduled- Inspection after significant rainfall
Corrective Actions (basic list, not limited to all activities]
Vegetation Removal, Sediment Removal, Repair/Replace outlet pipes -drains -skimmers -etc., seed
eroded areas, containment for petroleum areas, tanks stability, removal of trash and debris.

On March 3, 2016 Glen White of the North Carolina Department of Environmental Quality —
Minerals & Land Resources met with you and Jody Seaver, Compliance Officer, at the subject
facility located at 525 Quarry Road in Mounty Airy to conduct a compliance inspection for the
facilities Industrial Stormwater Permit.
Permit:
This facility holds General Stormwater Permit NCG020158 to allow the discharge of
stormwater, mining dewatering wastewater, and process wastewater to the surface waters
of North Carolina under the National Pollutant Discharge Elimination System (NPDES). A
copy of the current permit is required to be maintained with the Stormwater Pollution Prevention
Plan (SPPP) documents. The current permit and COC was not in the file: A copy of the Annual

Certificate of Coverage (COC) should be placed in the file each time the permit is renewed.
Records/Reports;
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. Inspectors reviewed the SPPP and
each of its components. The SPPP has not been updated annually as required. Training and
monitoring have not been performed as the permit requires. Jody Seaver was just recently hired
by Mr. Crossingham and is working to bring the facility back into compliance with all of their
environmental permits.
Facility Site Review:
This facility is a rock quarry which has continuous disturbed and exposed soils. Mr.
Crossingham assisted by Mr. Seaver took the inspector to review all four areas monitored. Each
of the areas is described below and are also a part of the mining permit.

• Land Disturbance and Process Area — Analytical monitoring must be performed once
(1,) per year on the Land Disturbance and Process Area stormwater area. Sampling
should be completed during a representative rainfall event, greater than 1.0 inch.
Qualitative monitoring must be performed twice annually at this area. Records show that
monitoring has not been performed per permit rules.
• On Site Vehicle Maintenance — Analytical monitoring must be conducted once per year
on the vehicle maintenance stormwater outfalls during a representative event. Qualitative
monitoring must be performed twice annually. Not compliant wiff monitoring
requirements.
• Mine Dewatering — Quarterl Analytical and Qualitative monitoring is on mine
dewatering discharge. Not compliant with monitoring requirements.
• Process Recycle Overflow — Wastewater used in gravel washing operations is recycled
an according to the permit, it is not discharged. Monitoring at this location is only
required when there is a discharge.
Effluent Receiving Waters:
Effluent drains into the adjacent Ararat River, Class "C" waters of the Yadkin Pee -Dee River
Basin,
Self -Monitoring Program:
This facility is required to develop a Stormwater Pollution Prevention Plan (SPPP) and monitor
stormwater and mining permit rules. Monitoring has not been performed as required. The SPPP
has not been updated, a copy of the current permit is not included in the file, Certificate of
Coverage is not in the file, Training Records and Spill Records have not been completed in
several years.
Deficiency Issues to be corrected:
1. Update all components of the Stormwater Pollution Prevention Plan (SPPP) annually.
2. SPPP must include a copy of the current permit. A copy of the permit was sent to Mr.
Seaver via email.
3. SPPP must contain the annual Certificate of Coverage (COC). This can only be printed
at the time of permit renewal each year.
4. Begin Qualitative and Analytical Monitoring per the stormwater permit. Provide the next
analytical results to Glen White at NCDEQ. Begin Monitoring at next measurable
rainfall. Contact an approved testing facility to get supplies and to be set-up and ready
before rainfall comes.
5. Establish Required Training and provide annually. This can be accomplished by adding
. to one of your annual required Safety Training sessions. Have attendees sign and date
log and keep in SPPP.

Although the facility itself appears well managed, it was found to be non -compliant, with its
industrial stormwater permit due to non-compliance with monitoring requirements, training
requirements and upkeep of the SPPP. Re -inspection will occur in approximately 6 months
to verify com fiance with the Stormwater Permit Re uirements and avoid a Notice of
Violation which may result in fines/fees of up to $25000/daywhich may be the next step in

On July 8, 2008, Gary Hudson of this.office met with you and Brad Davis to perform a
General Stormwater Permit Inspection at the Ararat Rock Products facility. This inspection
evaluated the areas as designated on the attached inspection form. The facility was in
compliance with all permit requirements with the exception the semi-annual qualitative
(visual) monitoring. Observations from each area are addressed below:
1. Permit
Ararat Rock Products holds Certificate of Coverage # NCG020158 for the discharge of
stormwater from the facility. The General Permit became effective January 19, 2005 and
expires December 31, 2009.
2. Records/Reports
With the exception of the Qualitative monitoring requirements, all records are being kept per
the requirements of Part III Section B of the permit.'
3. Facility Site Review
Ararat Rock is implementing appropriate Best Management Practices (BMP's) to divert,
reuse or otherwise manage stormwater runoff to reduce pollutants in stormwater discharges
leaving the site. The site appeared very clean and very well managed.
4. Effluent/Receiving Waters
Stormwater runoff from the facility reaches the Ararat River, Class °C" waters in the
Yadkin/Pee-Dee River Basin.

5. Monitoring Program
Part II, Section C of the permit requires that qualitative (visual) monitoring be performed and
documented once every seven calendar days, and within 24 hours after any storm event of
greater than 0.5 inches of rain per 24 hour period. A review of the facility files found that
the qualitative monitoring has not been conducted as required.
Part 11, Section B of the permit requires that analytical monitoring be performed. A review of
the facility files found that the analytical monitoring has been conducted as required.
The analytical monitoring for on -site -vehicle maintenance listed in Section D of the permit is -
required since the Ararat Rock Products plant uses more than 55 gallons'of new motor oil
per month when averaged over the calendar year. A review of the facility files found that the
analytical monitoring has been conducted as required.

The general permit (NCG020000) is not exactly clear concerning whether a Plan is
required. Part III of the permit text says "A BMP Plan shall be developed in
accordance with the requirements of this section." However, later it also says "The
BMP Plan requirement.... may be met by utilizing appropriate requirements in other
documented plans specific to the permitted facility." Beats me.
Anyway, the facility needs to be aware (either by a BMP Plan or other means] that;
1) all stormwater BMP's shall be inspected at least once every seven calendar days
and within 24 hours after any storm event that results in a discharge, 2) BMP's must
be cleaned out when sediment storage capacity is <50%, 3) to take corrective action
if visible sedimentation is leaving the property. Good luck. Mike.

yourself with the changes in the reissued permit. The general permit authorizes discharges of Stormwater and some types of
wastewater. You must meet the provisions of the permit for the types of discharges present at your facility. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.

Dear Mr. Crossingham:
On April 26, 2006, Mike Mickey of this office met with you and Johnny Gwyn to conduct a
General Stormwater Permit Inspection at Ararat Rock Products. The above general permit covers
the discharge of stormwater and mine dewatering wastewater from mining operations into surface
waters of the state.
The inspection found Ararat Rock Products to be noncompliant with the permit due to the
fact that no monitoring has ever been performed. Please note the following record keeping and
sampling requirements in the permit:
Land Disturbance and Process Areas - Analytical sampling is to be performed once per
year on the land disturbance and process area stormwater outfalls per Table 1 of Part III,
Section B of the permit. The sampling should be performed during a representative storm
event (>0.1 inches). In addition, the permit also requires semi-annual qualitative (visual)
monitoring of these areas per Table 5. Visual monitoring must be documented once in the
spring (April — June) and once in the fall (September — November).
•: On -Site Vehicle Maintenance — Analytical sampling must be conducted once per year on
the vehicle maintenance stormwater outfall(s) during a representative storm event per
Table 3. In addition, the permit also requires semi-annual qualitative (visual) monitoring of
this area per Table 5. The visual monitoring must be performed once in the spring (April —
June) and once in the fall (September— November).
Mine Dewaterin - Quarterly sampling shall be performed on the mine dewatering
discharge as specified in Table 6.
Process Recycle Overflow_ — It is our understanding that wastewater generated from the
gravel washing operation is continually recycled with no discharge to surface waters. If this
is the case, the sampling specified in Table 8 is not required. A statement confirming

The analytical and qualitative monitoring reporting forms for documentation are attached
to this letter along with a Technical Bulletin that gives a general overview of the stormwater permit
requirements. It is suggested that you contact a certified laboratory or consultant to assist you
with setting up your monitoring program. A list of possible contacts is attached.
Please be advised that the failure to conduct the required monitoring is a violation of the
General Permit. Per Part IV Section A.2, "The permittee must comply with all conditions of this
general permit. Any permit noncompliance constitutes a violation of the Clean Water Act and is
grounds for enforcement action". Under NC state law, a daily civil penalty of not more than twenty-
five thousand dollars ($25,000) per day, per violation, may be assessed against any person who
violates or fails to act in accordance with the terms, conditions, or requirements of a permit [Ref:
NC General Statute 143-215.6A].
You are requested to respond in writing to this office on or before Ma 31, 2006 advising if
Ararat Rock will conduct the required monitoring with onsite personnel or if you plan to enlist the
services of a qualified company. The analytical results must be submitted to the Raleigh address
listed on the monitoring forms within 30 days of receipt of the results from the certified laboratory.

Key Permit Requirements:

Stormwater:
Develop and implement the stormwater BMPs for the site (Part !!l, Section A,
NCG020000).
• Perform inspections of BMPs for land disturbance activities every seven calendar days
and within 24-hours of a storm event (Part lll, Section A, NCG020000),
• Perform inspections and qualitative monitoring of all other stormwater controls semi-
annually,
• Perform annual analytical monitoring for stormwater discharges (Part 111, Section B,
NCG020000).
• Provide secondary containment around all bulk storage tanks. For existing but newly
permitted facilities, within 12 months of issue date (Part il, Section A 6, NCG020000)
• Provide for appropriate operation and maintenance of BMPs as necessary to comply
with permit requirements Including Part III, Section A provisions of the general permit.
Wastewater:
• Obtain Authorization to Construct(A to C) permit for new or expanding treatment
facilities as indicated in table below in accordance with NCAC 2H .0140. Submit a
certificate of completion prior to operation.
• Provide plans and specifications prepared by a professional engineer or qualified staff
for each A to C request.
• Perform operation and maintenance requirements for existing process treatment
facilities in accordance with Part II, Section B, General Permit.
• Perform quarterly analytical monitoring for wastewater discharges as indicated in table
below (see also Part 111, Section C, NCG020000).
• Assure that dewatering activities do not affect surrounding wetlands and hydrology
(see Part It Section C, NCG020000).

• Language has been added to note that where Total Maximum Daily Loads (TMDLs)
are implemented, sites covered by this permit must demonstrate that their discharges
will not violate the conditions of the TMDL for the parameters of concern. (Para !
Section A)
• Language has been added to clarify that water quality and wetland standards apply to
discharges covered under the permit.
• Qualitative monitoring requirements have been amended to include checks for
deposition and erosion at or below outfalls.
• Wastewater monitoring requirements have been modified to require fecal coliform
monitoring for mine dewatering discharge to SA waters (Table 6) and to set appropriate
TSS effluent limits for discharges to HQW waters (Table 9)

Specific General Permit Questions and Answers:
Q How Do I Renew My General Permit Coverage When The Permit Is Set To Expire ?
A In order to assure continued coverage under the permit you must submit the required
forms and fees as are required by the agency no later than 180 days prior to the expiration
date of permit coverage. If you do not request renewal in a timely manner you may be
subject to enforcement procedures.
Q When Do I Require A Professional Engineer Sign- off On My New or Expanded
Treatment Facility?
A The plans and specifications requirement holds for all construction of new or expanding
treatment facilities except those that are designed, constructed and put into operation by
employees internal to the company and who are qualified to perform such work for their
respective companies in accordance with General Statues, 89C-25, paragraph 7.
Q How Do I Obtain Representative Outfall Status For My Stormwater Discharges?
A Representative outfall status may be granted for facilities with multiple stormwater outfalls
that are substantially identical. You will need to evaluate drainage area characteristics for
each outfall to assure that the outfalls selected have similar activities and materials exposed
to stormwater as those outfalls they are representative of.
You may petition the Director of the Division of Water Quality to grant representative outfall
status for your outfall(s).r The following additional information must also be provided:
• A drainage map identifying the drainage areas for each outfall including the size, percent
impervious, flow direction, buildings, BMPs, process areas and storage areas.
• A written description of all industrial activity which has the potential to be exposed to
stormwater runoff in each drainage area and a prediction of the pollutants that may be
contained in the runoff.
If the Division approves representative outfall status then you will only have to monitor those
stormwater outfalls that "represent" your stormwater discharge and not all of your outfalls.
Please note that Qualitative Monitoring must be conducted for all stormwater outfalls.
Representative outfall status cannot be granted for wastewater discharges.
Q What Happens When I Pull Samples For My Annual Analytical Monitoring And I'm
Unable To Get A Sample For One Of The Parameters?
A If this happens, it is fine to go ahead and just sample for the missing parameter during
another representative storm event. Just be sure to note the total rainfall event, and total
flow apart from the initial monitoring data.
Q Does A Certified Lab Need To Be Used To Analyze Stormwater Samples?
A Yes, a North Carolina certified lab must be used to perform the analyses. The only
exception to this rule is when measuring the value of pH, pH should be measured in the field
because it may change considerably between when the sample is pulled and when the
laboratory analyzes it. A list of certified labs can be obtained from the Division's Laboratory
Section at [phone] or from their website http://h2o.enr.state.nc.us/labtcert.htm.
Q What If I Sell My Business Or The Name of My Business Changes?
A The Division views changes of name or ownership as a modification that requires the
Director's approval and reissuance of the permit. Name and ownership changes require you
to complete a Name/Ownership Change Form from the Division and mail it back to us at the
address below along with all required information.

Q How Do Cut-off Concentrations
for Stormwater Work?
A The cut-off concentrations are used
to compare against the average
analytical monitoring data for
stormwater discharges. When the
average monitoring data are below the
cut-off concentrations, monitoring can
be suspended at that outfall for that
parameter until the last year of the
permit term. The inclusion of 'cut-offs'
allow facilities with low pollutant levels
to reduce monitoring efforts.

that is impounded in, or that collects
in, the mine and is pumped, drained
or otherwise removed from the mine
through the efforts of the mine
operator. However, if a mine is also
used for treatment of process waste

2. Mine reaps must be accurate and appropriately scaled drawings, aerial photographs or enlarged
topographic maps of the entire mine site. All aspects of the mine site must be clearly IabeIed on the
maps along with their corresponding (approximate) acreage. As a reminder, mining permits can
only be issued for up to 10 years; thus, all mine and reclamation maps must only denote those
activities that are intended to be conducted during the life of the mining permit. All maps must be
of a scale sufficient (see minimum requirements listed below) to clearly illustrate the following, at a
minimum:
a. Property lines of the tract or tracts of land on which the proposed mining activity is to be located
including easements and rights -of -way.
b. Existing or proposed permit boundaries.
c. Initial and ultimate limits of clearing and grading.
d. Outline and width of all buffer zones (both undisturbed and unexcavated).
e. Outline and acreage of all pits/excavations.
f. Outline and acreage of all stockpile areas.
g. Outline and acreage of all temporary and/or permanent overburden disposal areas.
h. Location and acreage of all processing plants (processing plants may be described as to location and
distance from mine if sufficiently far removed).

from the Division of Water Quality, Water Quality Section. In addition, the applicant is required to
register water use with the Division of Water Resources if the operation withdraws more than
100,000 gallons per day and needs a capacity use permit from the Division of Water Resources if
the operation lies in a capacity use area and withdraws more than 100,000 gallons per day.

a. A buffer will be required between any mining activity and any mining permit boundary or right-of-
way. It may 'be an unexcavated buffer (no excavation, but roadways, berms and erosion &
sedimentation control measures may be installed within it), an undisturbed buffer (no disturbance
within the buffer whatsoever), or a combination of the two, depending upon the site conditions. Note
that all buffers must be located within the mining permit boundaries.
How wide a buffer will be maintained between any mining activity and any mining permit boundary
or right-of-way at this site? A minimum buffer of 25 feet is recommended, although a wider buffer
may be needed depending on site conditions. Show all buffer locations and widths on the mine
