The Stormwater Division of the City of Winston-Salem would like ito thank you for
your time and professional courtesy regarding the June 4, 2014 inspection of
Vulcan Construction Materials.
As per your request, a copy of the completed surface water inspection form has
been enclosed to you. Part 4 of this report (Required Actions to be completed by
Facility) contains Annotations 91424 that Vulcan Construction Materials
must complete. Please provide docuinentation that verifies VulcanConstruction
Materials successful completion of the corrective measures to me by October 16,
2014. 1
The Stormwater Division reserves the right to enter and inspect Vulcan
Construction Materials as often as necessary to determine compliance with permit
requirements of NCG020082.

storage pallet according to your general permit NCG020082.

4. Install locking mechanism on diesel fuel secondary tank drain valve, to ensure no leakage
can occur to produce stormwater pollution.
5. Immediately clean up grease spots on grease pump and drum. Also, place tarp over these
drums when not in use.
6. Replace drain plug in dumpster at heavy equipment maintenance shop, or replace dumpster
as discussed to prevent stormwater pollution.
7. Immediately properly dispose of the liquid mixed with oil behind warehouse storage
building as per your permit requires.
8. Install caps on all exterior bulk liquid container openings to prevent spills of residual fluids.
9. Provide structural/nonstructural BMP's for grease covered cylinder bowls at bone yard to
prevent polluted stormwater runoff from reaching surface waters.
10. Fix leaking oil pump on equipment at boneyard and immediately clean up & properly
dispose of oil saturated materials.
11. Inunediately repair & clean up oil leaks on hydraulic oil system and increase cleaning
frequency to weekly, as per your Good Housekeeping Program requirements. Replace oily
rag with a tight fitting cap.
12. Update SPPP on a yearly basis as required by you permit.

Ron Boone, of the Winston-Salem Regional Office, has reviewed your requests for
Representative Outfall Status (ROS) for Vulcan Materials Company's (VMC) North Quarry and East
Forsyth Quarry, dated March 7, 2011. The results of his review are detailed below.
As we understand it:
East Forsyth Quarry, NCG020115: You are requesting ROS for SDO 003 to be
represented by SDO 005.
North Quarry, NCG020082: You are requesting ROS for SDOs 004, 005, 006, and 007, to
be represented by SDO 003.
Based on the information and maps provided, as well as information gleaned from the
December, 2010, compliance evaluation inspections, we hereby approve both of these requests. In
accordance with 40 CFR §122.21(g)(7), you are authorized to sample SDO 005 as representative of
SDO 003 at the East Forsyth Quarry, and SDO 003 as representative of SDOs 004, 005, 006, and
007, at the North Quarry. This approval is effective with the next sampling event. Please note that all
other SDOs not expressly mentioned in this letter must still be sampled as required by the permit, i.e.
SDOs 001, 002, and 004 for the East Forsyth Quarry.
You are reminded that VMC is still required to conduct Qualitative Monitoring (QM) at all SDOs
at both quarrys, regardless of ROS. Also, please remember that any actions you initiate in response
to benchmark exceedences, as directed in the tiered response provisions of your permit, must
address all drainage areas represented by the representative outfall, where appropriate.

Please append this letter to your Stormwater Pollution Prevention Plans (SP3) or permits to
document that ROS has been approved. If changes in drainage areas, structures, processes,
storage practices, or other activities occur that significantly alter the basis of this approval, ROS may
no longer be valid. You should either resume sampling at all SDOs, or reapply to this office for new
ROS based on the updated information.
Finally, although we have approved ROS for both sites using this single letter, DWO considers
each site completely independent of the other. Therefore, occurrences or activities that occur at one
site, generally do NOT affect the other.

If a facility is required to sample multiple discharge locations with very similar storm water discharges, the
permittee may petition the Director for Representative Outfall Status (ROS). DWQ may grant Representative
Outfall Status if stormwater discharges from a single outfall are representative of discharges from multiple
outfalls. Approved ROS will reduce the number of outfalls where analytical sampling requirements apply.
If Representative Outfall Status is granted, ALL outfalls are still subject to the qualitative monitoring
requirements of the facility's permit —unless otherwise allowed by the permit (such as NCGO20000) and DWQ
approval. The approval letter from DWQ must be kept on site with the facility's Stormwater Pollution
Prevention Plan. The facility must notify DWQ in writing if any changes affect representative status.

SECTION 5.0 DISCHARGE SAMPLING AND ANALYSIS
The General Permit requires sampling and analysis of point source discharges that fall in the following
categories.
Mine dewatering and/or discharges of process wastewater, including commingled
stormwater_
Stormwater discharges from other plant areas where the stormwater has contacted areas
directly related land disturbance and process areas..
Stormwater discharges from on -site vehicle maintenance areas.
5.1 Process Water and Stormwater Sampling/Analysis Program
Refer to Table 1 for required sample frequency and analytical parameters. There are four
appropriate sampling location at the North Quarry which will be used to sample various process
water and stormwater discharges. Each outfall to be sampled is listed below along with a brief
description of each outfall's type of discharge.

Quarry site.
Mine dewatering flows and runoff from yard and process areas which drain to the pit
are pumped directly to the creek at outfall 101-001.
Outfall 101-002 is the discharge point for the runoff from an area which includes
most of the yard around the North shop. This is the appropriate point to sample the
runoff from all on -site vehicle maintenance activities.
The entire plant area drains via a perimeter ditch to a pond which has a 36"
standpipe and ouffall to the creek. This discharge point has been designated outfall
101-003 and is the appropriate sampling point for plant yard, and material stockpile
areas. All other yawl outfalls will be represented by outfall 101-003
The following table summarizes the total additional outfalls at this site which will not require
sampling. The abovementioned outfalls that will be sampled have been linked to the
following outfalls based on the overall type of discharge each additional outfall will represent.
All outfall locations are indicated on the partial mine map in Appendix I. Documentation of
representative outfall status should be kept on site within the Mineral Mining Plan.

1. Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ)
conducted a compliance evaluation inspection (CEI) at the subject facility on December 8,•2010.
Your assistance and cooperation during the inspection, as well as that of Philip Phibbs, Plant
Manager, Dean Bullins, Assistant Plant Manager, and Larry Estep, Lab Manager, was greatly
appreciated. An inspection checklist is attached for your records and- inspection findings are
summarized below.
2. The facility is located at 4401 North Patterson Avenue, in Winston-Salem, Forsyth County, North
Carolina. Stormwater and wastewater related to mining activity is discharged to Mill Creek, which
is classified as Class C waters in the Yadkin River Basin.
Documentation & Monitoring Review
3. The Stormwater Pollution Prevention Plan (SP3) is complete but has not yet been fully
implemented. The stormwater discharge outfalls (SDO) have not yet been certified, there is
insufficient stormwater related employee training and documentation thereof and there is no
record of the SP3 annual review. Please make it a high priority to complete the entire plan within
the next couple of months.
4. There are currently six Stormwater Discharge Outfalls (SDO) at the site (002 thru 007) and one
wastewater outfall (001). Qualitative Monitoring (QM) is currently only being done at outfalls 002
and 003. However, you should be conducting QM at all SDOs except SDO 006, which was
eliminated with the relocation of the asphalt plant (stormwater from 006's drainage area has been
redirected to the pit). It is also noted that SDO 007 is scheduled to be eliminated and drainage
from this area will be conveyed to SDO 002, which is also the vehicle maintenance SDO. Once
SDO 007 is plugged and that flow redirected to 002, you may discontinue QM of 007. For the time

being however, in addition to 002 and 003, you currently should also be conducting QM of 004,
005 and 007. It is highly suggested, since they're in such close proximity to each other, that you
also eliminate 004 and convey that SW flow to 005.
5. Analytical monitoring (AM) is currently being conducted at wastewater outfall 001 and SDOs 002
and 003. However, you should be conducting AM at all SDOs except 006 which has been
eliminated. Please begin AM at SDOs 004, 005 and 007 immediately. Also, AM of SDO 002
(vehicle maintenance area) currently includes oil & grease but not Total Petroleum Hydrocarbons
(TPH). Please begin monitoring 002 for TPH as required by the permit; oil and grease monitoring
of SDO 002 is not required.
Site Review
6. Mr. Boone reports that the site was quite clean and well maintained. All above ground storage
tanks are double -walled or have secondary containments. SW drainage from the secondary
containment for the fuel storage tank actually goes to a separator and then to a non -discharging
holding pond. All tanks appeared to be in excellent condition and no signs of spillage were noted.
7. Please ensure that the site's SP3 and monitoring plans continue to change along with any
physical changes at the site, such as the elimination of SDO 006 and the possible future
elimination of 004 and 007.
8. You are reminded that violations of the NCG020000 general permit are subject to civil penalty
assessments not to exceed $25,000 per day, per violation. Thank you for your cooperation in

Changes Specific to Stormwater
• Benchmark exceedences shall be reported to the Regional Office.
Benchmarks are not limits.
• Stormwater discharges shall be monitored from Vehicle Maintenance Areas
for Total Petroleum Hydrocarbons (TPH) rather than Oil & Grease (O&G).
• Stormwater discharges shall be monitored from basins designed to contain
the 10-year storm.
• The SBMP plan has been replaced by a specific mining SPPP.
• Stormwater Analytical Monitoring has been increased to twice per year.
• When an analytical benchmark is exceeded, a Tiered Response applies.
Cut-off concentrations have been removed.
• Qualitative monitoring must be recorded during the same representative
storm event as analytical monitoring.
• 48 hours of little or no precipitation may now precede a "Representative
Storm Event".

Frequently Asked Questions
Do I have to monitor all outfalls?
Yes. However, you may request Repre-
sentative Outfali Status (ROS). If ap-
proved, this status allows analytical &
now qualitative monitoring at fewer
outfalls. To request ROS, submit a ROS
Request Form SWU-ROS (from our web -
site) to the DWQ Regional Office.
What if I can't collect a Storm -
water discharge in 30 minutes?
When distances separate multiple out -
falls and preclude collection within 30
minutes of each discharge event, begin
collection within 30 minutes and con-
tinue until all outfalls with discharges
are sampled. Documentation must be
kept in the SPPP,
Do I still need to monitor my
inactive mine?
Yes. BMP inspections of inactive mines
are reduced (to at least ix/month and
within 24 hours of any storm event 0.5"
or more). However, monitoring of inac-
tive mine discharges remains the same.
Inactive mines must possess an
"Inactive Renewal" from DLR and notify
the DWQ Regional Office, or certify to
DWQ the site has been stabilized.
Where to do I have to sample
for turbidity?
Turbidity may be monitored at the
Stormwater or Wastewater Outfall. The
permittee Tay choose to monitor tur-
bidity in the receiving water, directly
upstream and downstream of the Out -
fall. If you have a question about where
to monitor turbidity, contact your Re-
gional Office for assistance.
What if I don't address qualita-
tive monitoring problems?
If mines do not respond to problems
seen in visual monitoring, DWQ may
require that mines change the monitor-
ing frequency, apply for an- individual
permit, implement in -stream monitor-
ing, install or modify structural Storm -
water controls, or implement other
controls.
What if I forget to monitor?
Mines that do not monitor per permit
conditions, shall monitor every month
until the permit is renewed.
When do I need an AtC?
For both mine dewatering wastewater.
and process wastewater discharges— if
engineered erosion and control struc-
tures adequately treat WW so that limits
are met and water quality is protected,
you do not need an AtC. If your mine
needs other treatment devices in order
to meet permit effluent limits & protect
water quality, you will need an AtC.

When do I need an O&M Plan?
When a mine with dewatering activities
has the potential to drain nearby wet-
lands or surfaces waters, the Division
may require a Pumping O&M Plan be
approved prior to issuance. These 0&M
Plans shall include but ore not limited to:
groundwater monitoring strategies &
detailed plans to maintain the surround-
ing hydrology.
Why did you replace Oil &
Grease with TPH in VM areas?
The TPH test only targets chemicals de-
rived from crude oil. The Oil & Grease
(O&G) test recovers fats from animal &
vegetable sources and chemicals from
crude oil. Because TPH is more specific,
it is a better parameter for vehicle main-
tenance areas. The TPH method we are
specifying for NCG02 is EPA 1664A (SGT-
HEM). It is important to note that DWQ
is not specifying the more expensive Gas
Chromatograph (GC) TPH method.
DWQ found lab costs for EPA 1664A
(SGT-HEM) to be comparable to current
O&G test costs. To test these parame-
ters in Stormwater, labs must perform
this test in accordance with EPA proce-
dures, but do not need to be certified.
Note: A lower benchmark applies for
TPH: 1S mg/I (not 30 mg/i).
What about the Coastal Rules?
If you have a new mine in one of the 20
Coastal Counties with >10,000 BUA, you
may also need a State Stormwater Per-
mit. If you are in the 20 Coastal Coun-
