DISCUSSION:
On September 1, 2016, Multi Media Inspector Mr. Ryan Swaim, Environmental Specialist of the WSRO-DEMLR,
contacted Mr. Jay Nivens, Environmental Engineer and Shane Hobson, Plant Manager, of Martin Marietta Materials, Inc.
- Reidsville Quarry to conduct a Multimedia Inspection of the facility. The facility was last inspected by DAQ personnel
on April 20, 201-5 aril it appeared to be operating in compliance at that time. Martin Marietta -Reidsville Quarry crushes
various size stone, which is used for various purposes, such as road construction. The facility is open for business
Monday -Friday, 8 - 10 hours/day, and 51 weeks/year at the present time. The facility does however experience
occasional reduced operating hours due to seasonal and market demand.
APPLICABLE REGULATIONS:
The applicable air quality regulations as listed in condition A.1 of Air Permit No. 04133R14 are: 2D .0202, 2D .0501, 2D
.0510, 2D .0521, .0524 (40 CFR 60 Subpart 000), 2D .0535, 2D .0540, 2Q .0102, and 2Q .0309. This facility is not
required to implement a risk management plan under the federal regulation 40 CFR 68, Section 112(r) because they do not
produce, use or store any of the regulated chemicals in quantities above the threshold limits. However, the General Duty
Clause contained in this rule applies to all facilities and requires the safe and responsible handling of hazardous materials
in any quantity.

SAFETY:
A safety vest, shoes, and eyewear are required at the plant. Martin Marietta requires each visitor to watch a brief safety
video before entering the quarry premises. The office parking lot for visitors has a slight incline and each parking space is
fitted with a wheel chock.
PROCESS and INSPECTION:
The process begins with holes being drilled into the quarry walls, usually in a vertical axis. After holes are drilled, a
mixture of ammonium nitrate and other material is ignited with blasting caps causing an explosion, causing rock to break
into pieces of various sizes, which fall mainly into the quarry floor and/or roads leading into and out of the quarry. The
loose rock is then picked up by approximately 50-tan front end loaders and placed into piles known in the industry as
"Muck Piles." The piles are occasionally watered by the facility's water truck to prevent fugitive dust, before being
transported by 50-70-ton dump trucks and offloaded into the dump hopper (ID # 1) and associated feeder (ID #2). The
rock is then fed into the primaryjaw crusher (ID #3) and then routed to the remaining circuit of various conveyors,
screens, and secondary crushers. The crushed pieces of rock eventually are stock piled into different diameter stone.
Some RIP -RAP (the largest stone — 7 inches in diameter) is also made at this location. The following are the examples of
stone crushed at this location:
78M (crushed stone) - Used in the asphalt and concrete industry.
57, 67 (crushed stone) - Also used in the concrete and asphalt industry.
5 (crushed stone) - Used or placed around septic tank drainage (nitrification) lines
Aggregate Base Course (ABC) — Crushed stone used for road bases, building pads, residential
driveways, and some parking lots.
Most of the plant (only the secondary circuit) was crushing stone during the MMI inspection. No visible emissions were
detected from any of the equipment indicating compliance with 2D .0510 and 2D .0521, but also subject NSPS Subpart
000 sources. The facility was also moving stone inside the quarry and trucks were being loaded with various sized stone
for customers.
The facility's permit exempt tanks consisting of: two aboveground, 10,000-gallon Diesel fuel storage tanks, I-OST-1 and
I-OST-2, which store fuel for the haul trucks, water truck and bucket loaders. The facility also has one aboveground
3,000-gallon gasoline storage tank, I-OST-3, two aboveground 2,000-gallon motor oil storage tanks, I-OST-4 and I-OST-
5, one aboveground, 250-gallon crusher oil storage tank, I-OST-6 and one aboveground, 550-gallon recycled oil storage
tank, I-OST-7... .
PERMIT CONDITIONS:
Condition A.2 (213 .0202) deals with "Registration of Air Pollution Sources." The permit for this facility expires on
January 31, 2018. At least 90 days prior to the expiration of this permit, the Permittee shall submit a permit renewal and
an emissions inventory for the 2016 calendar year. Mr. Hopson was reminded of this requirement during the inspection.
Condition A.3 contains the 15A NCAC 2D .0501(c) equipment reporting requirement. This condition requires this
facility to maintain an on -site equipment list and plant flow diagram of all equipment covered under this permit. This list
must include the rated crushing capacity of all crushers, width of belt conveyers, dimensions and configurations of
screens, the rated capacity of all other equipment not exempt from permitting, dates of equipment manufacture, dates of
required NSPS testing, and must bear the date of last revision. This condition also requires the permittee to provide a 15-
day written notification to the Regional Supervisor of the Division of Air Quality, including a revised plant flow diagram
and equipment list, any time the permittee installs new non -primary crushing equipment or relocates existing non -primary
crushing equipment. A current equipment list and flow diagram that adhere to the requirements of this condition were
included in the most recent permit revision (mainly for self-propelled equipment). Upon arrival at the facility Mr. Hopson
provided an equipment list and plant flow diagram to Mr. Swaim. This list appeared to be the same as the list submitted
during the last permit renewal, indicating no equipment changes. The facility appeared to be in compliance with 2Q
.0501(c).
Condition A.4 contains the 15A NCAC 2D .0510, "Particulate Control Requirement." This condition requires the
permittee to not produce materials in such a manner that PM, PMio, and total suspended particle ambient air quality
standards are exceeded beyond the property boundary. Additionally, the facility is required to control fugitive dust
emissions as required by 15A NCAC 2D .0540 (and permit condition A.11), and control process generated emissions
from crushers with wet suppression so that all applicable opacity standards are met. Mr. Swaim observed no visible
emissions from haul roads. The facility appeared to be in compliance with this permit condition.
Condition A.5 limits the primary crusher limited to 670 tons per hour production capacity at a maximum discharge
opening of 7 inches. The crusher currently has never exceeded 670 tons per hour production rate according to plant
personnel and the 7-inch requirement. Mr. Hobson stated that the primary crusher jaw was set at 4.25 inches with a
production rate of — 450 tons/hour. The facility appeared to be in compliance with this permit condition.
Conditions A.7 and A.8 contain the 2D .0521 visible emissions control requirement. This condition limits sources to 20
percent or 40 percent opacity depending on manufacture date. It should be noted that 40 CFR Part 60, Subpart 000
limits many of these sources to more stringent visible emissions standards than the limits listed in 2D .0521 and is
discussed below. The facility appeared to be in compliance with this permit condition.
Condition A.9 (2D .0524, 40 CFR 60, Subpart 000) concerns "New Source Performance Standards." The facility is
required to notify the Division in writing no later than thirty days after the commencement of construction of a crusher,
screen or conveyor except in the case of mass-produced sources, and to report again within fifteen days after the initial
startup of the affected source. Additionally, this condition limits the visible emissions from these affected sources to 15%
opacity for crushers, 10% opacity for the conveyors and screens and 0% opacity for conveyors and screens that process
saturated materials. A rule change for this subpart requires affected sources constructed after April 22, 2008, to comply
with the more stringent visible emissions limits of 12% opacity for crushers and 7% opacity for conveyors and screens.
For affected sources that use wet suppression and were installed after April 22, 2008, this condition requires monthly
inspections of water systems and spray nozzles to be recorded in a logbook. The facility is required to conduct
performance testing on any affected source within sixty days of achieving maximum production.but no later than 180 days
after the initial startup. A testing protocol must be submitted to the Division at least 45 days prior to testing and at least
15 days advance written notice must be afforded the Division to provide an observer. A provision of this condition
exempts the replacement equipment from testing if it is of equal or lesser production capacity than the existing source. As
previously discussed, the facility commenced operating the HP 300 cone crusher, # 16, on February 14, 201.1. The -facility
submitted a test protocol for visible emissions testing on April 5, 2011, conducted testing on April 26, 2011, and
submitted the test report on May 31, 2011. Results indicated the highest 6-minute average opacity for crusher 916 was
1.67 percent from the inlet and 1.46 percent from the outlet, demonstrating compliance. Test results were accepted by the
WSRO on May 31, 2011 and a letter indicating the fact was mailed on June 2, 2011 to the facility. This facility is also
required under condition A.9 to perform monthly periodic inspections to check that water is flowing to discharge nozzles
on wet suppression systems for sources that were constructed on or after April 22, 2008. The plant must record each
inspection including the date and any corrective action taken in a logbook that can be made available to DAQ personnel
upon request. The facility performs inspections (a total of 8 pieces of equipment) subject 000 equipment regardless if is
before the April 22, 2008. The facility performs monthly inspections on the equipment and were verified back to April
2015. The facility appeared to be in compliance with this permit condition.
Condition A.10 contains the 15A NCAC 2D .0535 rule which requires.the permittee to notify the DAQ director of any
excess emissions lasting longer than four hours resulting from a malfunction, a breakdown of process or control
equipment or any other abnormal conditions. Mr. Hobson stated that no such incidents had occurred. The facility
appeared to be in compliance with 2D .0535.
Condition A.I l per 2D .0540 requires fugitive dust controls from the unloading and loading area, stockpiles, parking lots,
and haul roads. The water truck was not operating during the inspection due to recent rain. The facility appeared to be
in compliance with .0540.
Condition A.12 contains the 15A NCAC 2D .1806, `'Control and Prohibition of Odorous Emissions" requirements. The
condition was discussed with Mr. Hobson. No objectionable odors were detected during the approach to the facility or
within the property boundaries. The facility does not have any previous odor complaints in the 1BEAM database and the
process is inherently low in odor. Compliance with 2D .1806 is expected.
Condition A.13 (40 CFR 63, Subpart CCCCCC) pertains to NESHAP Subpart CCCCCC and is applicable to the permit
exempt, aboveground, 3,000-gallon gasoline storage tank, I-OST-3. Facilities having a gasoline throughput less than
10,000 gallons per month must exercise good work practices including minimizing spills, expediting spill clean-up,
minimizing gasoline delivery to open waste collection systems and the use of seals on portable containers. The facility is
required to have on -site records of their gasoline throughput available within 24 hours of being requested by the Division.
Mr. Hopson provided Mr. Swaim with the facilities 2015 consumption report showing compliance with this condition.

Subpart 000 is applicable to the subject equipment at this facility. Subpart CCCCCC is applicable to the permit exempt
storage tank (I-OST-3). The facility does not have an emergency generator at the current time.

On March 23, 2016 Glen White of the North Carolina Department of Environmental Quality;
Department of Minerals and Land Resources met with you at the Asphalt Plant and Quarry
located on NC 87 in Reidsville, NC to conduct a compliance inspection per the Industrial
Stormwater Permit requirements. Inspection consist of review of the Stormwater Pollution
Prevention Plan, training and monitoring records and observation of the general condition of the
site and outfall. Your assistance with the inspection was greatly appreciated.
Permit:
The Asphalt plant holds General Stormwater Permit NCG160120 to discharge stormwater from
industrial activity associated with Asphalt Paving Mixtures and Blocks under the National
Pollutant Discharge Elimination System (NPDES). The permit became effective October 01,
2014 and expires on September 30, 2019.
The Quarry holds General Stormwater Permit NCG020061 to discharge stormwater from
industrial activity associated with Mine Dewatering Wastewater and Process Wastewater.
The Quarry is permitted for its mining operations under Mining Permit 79-06,
A copy of the current permit is required to be maintained with the Stormwater Pollution
Prevention Plan (SPPP) documents. The current permit was included in both files.
Records/Reports.
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part 11, Section A of the permit. A new SPPP was prepared and
issued on February 22, 2016. The document is well prepared. The documents have been
maintained and past documents were available for review at the time of inspection.

Facility Site Review:
This asphalt plant produces asphalt paving mixtures for roadways, parking facilities and
driveway construction. The quarry produces rock of varies sizes used throughout industry in
construction.
Self -Monitoring Pro rg am:
The facility is required to conduct qualitative and analytical monitoring twice annually. Records
show that monitoring has been conducted as required and results are within the required permit
parameters. There is only one actual discharge from the facility. It empties into the adjacent
creek as effluent from the quarry's settling basin. The quarry includes the settling basins and the
pit itself. The pit was virtually clear with a blue-green surface from minerals at the time of
inspection. The quarry reuses it's process water to spray it's roadways and paths to keep dust
down and improve air quality at the quarry, asphalt plant and at the adjoining concrete plant.

annual and quarterly schedules specified in the permit effective immediately upon receipt of this letter.
Be advised, that if this facility should trigger a tiered response again due to permit violations, a return to
monthly sampling will be required.

Mike Thomas of the Winston-Salem Regional Office of the NC Division of Water Quality
(DWQ) conducted a compliance evaluation inspection (CEI) at the subject facility on 12 April 2011.
Your assistance and cooperation during the inspection was greatly appreciated. An inspection checklist
is attached for your records and the inspection findings are summarized below.
The facility is located at 7639 NC Highway 87, in Reidsville, Rockingham County, North
Carolina. Stormwater related to industrial activity is discharged to an unnamed tributary Wolf Island
Creek, which is currently classified as Class C waters in the Roanoke River Basin.
Documentation and Monitoring Review
The Stormwater Pollution Prevention Plan (SP3) is complete and well implemented. Training
sessions are conducted annually to keep staff informed and up to date. Records of Qualitative Analysis
were available and appeared to be complete. Routine maintenance and daily site inspections were also
recorded and made available for inspection. All stormwaters and wastewaters are contained within the
facilities treatment/closed-loop recycle system, until it is necessary to discharge due to capacity. No
issues were observed with any of the required BMPs and inspection and maintenance of BMPs is being
performed and recorded as required.
Site Review
The Reidsville Quarry has one stormwater discharge. Analytical samples are collected by train
members of your staff and processed by Pace Analytical Laboratories. Members of your staff also
conduct qualitative monitoring as well. Monitoring records were complete and current. You indicated

attached for your records and the inspection findings are summarized below.
2. The facility is located at 7639 NC Highway 87, in Reidsville, Rockingham County, North Carolina.
The -facility discharges stormwater and wastewater to an unnamed tributary (UT) to Wolf Island
Creek, which is currently classified as a Class C water in the Roanoke River Basin.
Documentation & Monitoring_ Review
3. The facility has a written best management practices (BMP) plan as required by the permit.
Structural and non-structural BMPs employed at the facility are considered effective and sufficient
for the management and control of the runon/runoff, erosion and sedimentation and wastewater
produced at the facility. - All stormwater and wastewaters are contained within the facility's
treatment/closed-loop recycle system, until it is necessary to discharge due to capacity.
Secondary containment is also provided as required and meets all requirements. No issues were
noted with any of the BMPs and inspection and maintenance of BMPs is being accomplished and
recorded as required.
4. Both analytical and qualitative monitoring has been conducted in accordance with the permit.
Only one turbidity exceedence was noted. Monitoring frequency has been increased in response
to the exceedence and BMPs are being reviewed to determine the cause and possible corrective
actions to prevent future turbidity exceedences. .It was noted that this exceedence might also
have been the result of a contaminated or otherwise misrepresentative sample.
