REGAN Sa rerary L!1INDA CULPEPPER D r i.rar Robert Johnson III 502 Scott King Rd Durham NC 27713-9762 Dear Mr. Johnson: Fnvlronmenfa! Quali ry December 18, 2019 Subject: Compliance Evaluation Inspection Single Family Wastewater Treatment System Permit No. NCG551296 502 Scott King Rd SFR Durham County On September 12, 2019, Joan Schneier from the Raleigh Regional Office visited your single- family residence (SFR) wastewater treatment system to evaluate compliance with the above permit to discharge wastewater. Your assistance during the inspection was greatly appreciated. The checked boxes below show what conditions were noted at your facility: N Failure to analyze the effluent: The effluent that is discharged from your system must be analyzed once each year. See Part I(A) of your permit about his requirement. Make arrangements for sampling to be carried out within the next 3 months and submit results to this office within 3 weeks after the sampling has been conducted. You can do your own sampling but must have the samples analyzed by a state certified lab, which will require that you use its bottles. A list of certified laboratories who may do these type analyses is attached. Please call the lab for details. Alternately, please check the Yellow Pages for Environmental Consultants who may do the sampling, but they also have to use a certified lab for analysis. ® Other: You have an Advantex system, which is highly effective but requires periodic maintenance by trained operators. Please resume a maintenance contract, as soon as you are able. If you have questions or comments about this inspection or the requirements to take corrective action, please contact Joan Schneier or me at 919-791-4200 or joan.schneier c ncdenr.gov. y [D N u Ift LJ -:I ou 04 Fr n tir r i r 0i Fnvuun f -r ni tl to t. by Div •,i,:n of W'alrr kc5uur r es f It yirt.1I0'.;, '-?QIOB,unti f)rivv PA-1tili A.ortlr('.ur•lir:a Sincerely, Scott Vinson, Regional Supervisor Water Quality Regional Operations Section Raleigh Regional Office Division of Water Resources, NCDEQ Attachments: Inspection Report Certified labs Sampling parameters cc: (minus attachments) RRO.'SWP Files NPDES Permitting Unit Files by Laserfiche 1 n, r a States Environmental Pr, tect on Agency Form Approved EPA w0:hingtcn D C 24460 OMB No. 2040-0057 Water Compliance Inspection Report Approval expires 8-31-98 Section A: National Data System Coding (Le, PCS) Transaction Code NPDES yrimo, day Inspection Type Inspector Fac Type 1 u 2 I, 1 3 I NCG551296 111 12 19,09.'12 17 18 L.1 I r I 19 t tc t 20I I =1 L_J 21 s Inspection Work Days Fac lity Self-Monilorrig Eva:uation Rating 61 OA Reserved 67 70 LJI 71 I J72 u( 73 JI74 751 I I I I I J80 Section Bi Facility Data Name and Lc; ation cf Fa=lity Inspected (Far Industrial Uses discharging to POTW, also include Entry TimelDate Permit Effective Date POTW name and NPDES permit Number} 02:50Ph1 19/09/12 14/02/04 502 Scott King Road Exit TimelDate Permit Expiration Date 502 Scott King Rd Durham NC 27713 03 35Ph1 19/09/12 18/07/31 Name(s) of Onsite Representative(s)ffities(s)iPhone and Fax Number(s) Other Facility Data !l+ Name, Address of Responsib'e Offcial/Titte,Phone and Fax Number Robert H Johnson,502 Scott King Rd Durham NC 27713l1+ Contacted No Section C: Areas Evaluated During Inspection (Check only those areas evaluated) Permit Operations & Maintenance N Effluent/Receiving Waters Section D: Summary of Finding 'Comments (Attach additional sheets of narrative and checklists as necessary) (See attachment summary) Name(s) and Signature(s) of Inspector(s) Agency;OfficelPhone and Fax Numbers Dale Joan Schneier DWRIRRO GWl919-7914200: n ! o I Signature of Management O A Reviewer Agency.'Otfice/Phone and Fax Numbers Date llix 1z/,Gq/S EPA Form 3560-3 (Rev 9-94) Previous editions are obsolete. rayttrr NPDES yrlmolday Inspection Type (Cont.) 31 NCG551296 Ill 12 19,09,12 W 18ICI Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) The system consists of a septic tank, Advantex AX-20 RT pod with self-contained cloth filters, recirc pump, and UV. There is gravity flow to the riprapped outfall with vermin grate to the road ditch on 5alix Dr. The system was installed in August, 2014 to replace a failing sand filter. Bob ,Johnson assisted with the inspection. The septic covers were opened and the efflent filter partly cleaned by repeated dunking in the first chamber. The pod had two good sprayers but the filter seemed a bid sludgy. The last maintenance was in Dec 2018 by McFarland, but the system is presently not covered by a contract. The owner indicated that he would resume a maintenance contract in the future. The panel was repaired in December, 2018 after a lightning strike. The high water alarms came on after time delay, although the audio was weak. The UV bulb worked. No samples have been taken lately but the November 2016 showed less than 4 for all three parameters, which is excellent. No problems were seen in the ditch area. Non -compliant only due to failure to sample. Access and Location Notes: 3116 inch Alien Wrench needed for lank covers, 112 inch socket for pod. The outfali is under the holly bushes, just north of the gate on 5alix Dr. Page# Permit: NCG551296 Owner -Facility: 502 Scott King Road Inspection date: 0911212019 Inspection Type: Compliance Evaluation Permit Yes No NA NE (If the present permit expires in 6 months or less).
