4. Project Description and History
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The land surrounding NC 144 and SR 1321 is low-lying and surrounded by bottomland hardwood swamp. Uplands in the vicinity have historically been used for farming. There is an old
church to the southwest that is listed on the National Register of Historic Places. Highway US 15-501 is about .4 mile northeast of the project site.

Culvert 106 is sub -standard and was damaged in Hurricane Florence.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
Existing Culvert 820106, 3@8'-10" X 7'-2" RCBC, 47'-2" in length on NC-144. Constructed in 1958, the culvert is recommended to be replaced. According to the latest inspection report
dated 6/29/2018, the culvert has several defects. There are multiple cracks, spalls, de -laminations, exposed rebar, and exposed aggregate in the internal barrel walls and exterior wing
walls. In addition, there is vertical scour appx 6ft wide x 1ft deep x 3ft wide with no undermining reported at North inlet of Barrel 1, and Silt 47' long x 7' wide x4' deep in Barrel 2 and 3.
The culvert is nearing the end of its useful life. The proposed culvert is a 2@14'x9' RCBC with bevels, 83.5' in length, buried 1ft with 1ft. sill in South Barrel and 1ft. sill in North Barrel.
Upstream invert 203.011. and downstream invert 203.0ft, slope 0.0%. Roadway drainage runs off on either side of the proposed roadway, into roadside ditches and disperses into
w atlands and/or natural ground. In addition, there is a 42' 30" RCP with headwall that on the -Y- line (SR 1321) that will be replaced with 72' 36" RCP with headwall. Construction of the
new culvert will take place while an off -site detour maintains traffic. Bank Stabilization rip rap will be installed as necessary to prevent streambank degradation.

The emsting culvert on NC 144 is 47'2" long and will be replaced with a 83'5" culvert. The culvert on SR 1321 is 42' long and will be replaced with a 72'
culvert.

NCDOT's best management practices for the installation of pipe in jurisdictional waters will be adherred to thorughout the life of the project.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
STREAM CHANNEL RELOCATION LIMITATIONS:
The following sequence of construction shall be followed in the areas designated on the plans as stream relocations. Failure on the part of the
Contractor to follow this sequence, and complete each step prior to proceeding in this area as specified, will be just cause for the Engineer to direct
the suspension of work in accordance with Article 108-7 of the Standard Specifications.
(A) Clear, but do not grub area within the Environmentally Sensitive Area on the ebsting stream to be relocated.
(B) Construct and stabilize, with vegetation or erosion control materials sufficient to restrain erosion, the proposed stream channel relocation as shown
on the plans.
(C) Divert water into newly constructed channel only after it has been stabilized and approved.
(D) Begin grubbing and/or grading within the Environmentally Sensitive Area of the existing stream.
The Contractor shall perform seeding and mulching and install erosion control matting to all cut/fill slopes adjacent to stream relocations in
accordance with the contract.
The above requirements apply to the stream channels being constructed at the following stations:
