In response to your renewal application for continued coverage under Stormwater General Permit
P,:CG050000, the Division of Water Quality (DWQ) is forwarding herewith the reissued Certificate of Coverage
and stormwater General Permit. This permit is reissued pursuant to the requirements of North Carolina General
Statute 143-215. l and the Memorandum of Agreement between the state of North Carolina and the U.S.
Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.) The following
information is included with your permit package:
• A new Certificate of Coverage
• A copy of the stormwater General Permit
• A copy of a Technical Bulletin for the General Permit
• Copies of the Discharge Monitoring Report (DMR) Form
• Copies of the Annual Discharge Monitoring Report Form (if applicable)
• Copies ofthe Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to
stormwater discharge controls, management, monitoring, and record keeping. Please review the new permit to
familiarize yourself with all changes in the reissued permit. Significant changes to the General Permit are
outlined in the attached Technical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DWQ. This permit
does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve
the permittee from responsibility for compliance with any other applicable federal.. state, or local law, rule,
standard, ordinance, order, judgment, or decree.

This is our written response to the inspection summary that we received May 13, 2010 for the
compliance evaluation inspection dated April 23, 2010.
Inspection Summary:
1. General Permit not available at time of inspection.
i. Permit found, was filed in the wrong location._ -A copy has been attached.
2. Deficiencies Found in SP3 (Stormwater Pollution Prevention Plan)
■ The SP3 is required to be reviewed and updated annually.
i. A coversheet has been added to our SP3 Plan -which will show when it was
reviewed and if any changes were made. A copy has been attached.
■ Documentation of training is required to be within the plan.
i. A formal traininq record has been developed and will be si ned by each
employee trained ins ill response. Attached is a gQpv of this form.
■ SP3 is required to contain a list of significant spill during the last three years..
i. A form has been added to our SP3 that will list all significant sills for each
year. A copy of that form is attached.
■ Secondary containment is suggested for the hydraulic oil and waste oil barrel when
kept inside the building. Secondary containment is required if they are kept outside.
i. The hydraulic oil container inside the building has been replaced with anew
container that sits inside a containment tank.
ii. Containment tanks have been order for any drums that might be stored
outside. The K are scheduled to be delivered by June

On April 23, 2010, I, Dave Parnell of the Raleigh Regional Office of the Division of Water
Quality, performed a compliance evaluation inspection (CEI) at the referenced facility.
Your assistance was helpful and appreciated. This inspection was conducted to determine
your compliance with NPDES stormwater general permit referenced above. The following
observations were noted during the Division of Water Quality inspection:
SITE DESCRIPTION
This facility is a manufacturer of thermoset moulded parts and a supplier for major US
electrical manufacturing companies. The manufacturing operation was observed during the
tour of the facility. There are two stormwater discharge outfalls (SDO) at the facility, which
collects stormwater from the building roof, graveled areas and paved areas and piped to the
outfalls. The SDO discharge to an unnamed tributary to Buffalo Creek, in the Neuse River
Basin. SDO #1 is located on the western side of the property and SDO 92 is located on the
southern side of the property. The discharge from both SDO requires qualitative monitoring.
Vehicle maintenance is not conducted at this facility, therefore, no analytical monitoring is
required.

INSPECTION SUMMARY
1. The current Stormwater General Permit was not available during the inspection. Please
contact Jennifer Jones of the Stormwater Permitting Unit at 919 807-6379 for
assistance in obtaining a copy of your current Stormwater Permit.
The Stormwater Pollution Prevention Plan (SP3) was available and reviewed during the
CEI. The following deficiencies were noted:
• The SP3 is required to be reviewed and updated annually.
• Documentation of employee training is required to be within the plan.
• The SP3 is required to contain a list of significant spills during the last three years.
• Secondary Containment is suggested for the hydraulic oil and waste oil barrel when
kept inside the building, secondary containment is required if they are kept outside.
Please address the SP3 deficiencies.
3. The SDO were visited during the inspection. Qualitative monitoring reports were on hand
and found to be compliant with the requirements of the permit.
Please provide a written response to this letter, within 30 days of the receipt of this letter,
detailing your facilitiy's compliance efforts with respect to the bold items in item 91, 42. If you

Payment of the required annual administering and compliance monitoring fee of $80.00 for this year has not
been received for the subject permit. This fee is required by Title 15 North Carolina Administrative Code 2H.0105, under
the authority of North Carolina Generai Statutes 143-215.3(a)(1), (1a) and (1b). Because this fee was not fully paid within
30 days after being billed, this letter initiates action to revoke the subject permit, pursuant to 15 ncac 2H.0105(b) (2) (k)
(4), and G.S. 143-215.1 (b) (3).
Effective 60 days from receipt of this notice, subject permit is hereby revoked unless the required Annual
Administering and Compliance Monitoring Fee is received within that time. Discharges without a permit are subject to the

Dear Permittee:
In response to your renewal application for continued coverage under the subject permit, the Division
of Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit
is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency
dated December 6, 1983.
The following information is included with your permit package:
■ A copy of the stormwater general permit.
• A Stormwater Pollution Prevention Plan Certification Form. This form certifies that you have
developed and implemented the Stormwater Pollution Prevention Plan (SPPP) required in your
permit. This form must be completed and returned to the Division within 30 days of receipt of
this letter. DO NOT send the SPPP with the signed form.
• Five copies of Analytical Monitoring forms.
■ Five copies of Qualitative Monitoring forms.
■ A copy of a Technical Bulletin on the stormwater program which outlines program components
and addresses frequently asked questions.
■ A corrected Certificate of Coverage if you indicated a name or address change on the Renewal
Form returned to the Division.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required.

If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit

to receiving waters designated as Buffalo Creek in the Neuse River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III
and IV of General Permit No. NCGO50000 as attached.
This certificate of coverage shall become effective March 25, 1994
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
