In accordance with your application for a discharge permit received on February 5, 2014, we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state
- NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, this certificate of coverage shalFbe final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Energy, Mineral and Land Resources (Division). The Division may require modification or
revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division or permits required by
the Division of Water Resources, Coastal Area Management Act or any other federal, state or local
governmental permit that may be required.

This Certificate of Coverage shall become effective March 28, 2014.

The facility is located on the Fuquay-Varina North USGS Topographic quadrangle. A
topographic map is provided in Appendix B.
Stormwater flows in the same direction throughout the site, towards the back of the
property. Flow paths are marked by arrows on the site -maps in Appendix A. There are
three main outfalls associated with industrial activity on -site. They are identified on the
site maps in Appendix A, and will be referred to at Outfall #1, Outfall #2, and Outfall #3.
Appendix A includes two separate maps, one for each building on site. These buildings

Analysis — Technical/Economic Feasibility

This section addresses the technical and/or economic feasibility of modifying the
methods of operations and storage practices to eliminate or reduce exposure of materials
and processes to stormwater, Current procedures are described, and where feasible,
modifications to the current procedures are proposed and discussed.
Storage Practices
Areas where materials are exposed to stormwater are along the north side of the
manufacturing building and the south and west side of the storage building. Empty wire
spools, wooden pallets, and garbage receptacles are stored outside of the manufacturing
building and come into contact with stormwater runoff. There is also outdoor oil storage
in containment. The storage building has outdoor pallet storage, and garbage receptacles
that come into contact with stormwater runoff.
Due to limited space inside both buildings, it is not feasible for these materials to be
stored inside. Covering the exposed materials with a small shed structure or awning may
be the most feasible option. The storage shed would need to be designed to allow
clearance for forklift traffic and large enough to hold all materials currently stored outside.
Secondary Containment
The manufacturing building currently has outdoor oil storage in drums that are completely
contained within a secondary containment unit.
Best Management Practices (BMPs)
The facility uses, or will use, the following Best Management Practices (BMPs) in an
effort to minimize the contamination of stormwater discharges and to divert, if possible,
stormwater drainage away from sources of contamination.
Prevention
• Monitoring — Berk-Tek LLC will conduct periodic inspections of facility operations
and equipment as outlined in the inspection schedules and preventative
maintenance sections. These scheduled inspections will assist in identifying and
correcting any problems that could lead to stormwater contamination.
• Labeling — Berk-Tek LLC, in an effort to comply with OSHA Hazard
Communication requirements, labels containers of hazardous materials to
properly identify the material and alert employees to possible hazards with the
material. In addition, warning signs are used where appropriate to alert
employees of situations where accidents could lead to inadvertent spills.
• Covering — Outdoor storage will be covered whenever a covered area is
available. To our knowledge, wooden pallets and spools stored outside are not
treated with preservatives or other chemicals, and therefore, will have a
negligible impact on stormwater quality.
Containment
• Oil drums will be stored in the secondary containment unit to prevent any contact
with stormwater.
• Spill kits containing absorbent booms, pads, absorbent material, etc. should be
kept at all loading and unloading areas where the likelihood for a spill increases.

• Berk-Tek LLC will train employees who may affect stormwater quality on proper
stormwater pollution prevention measures to reduce the threat of stormwater

As part of the facility's overall preventive maintenance program, Berk-Tek LLC personnel
will semi-annually (Fall and Spring) inspect the following outdoor areas to limit or prevent

Loading and Unloading
Chemical drums and containers will be inspected by Berk-Tek's shipping and receiving
personnel upon arrival to determine if they are leaking or in poor repair. Prior to transport
to the oil storage location, all bungs, vents, and rim seals are to be inspected for leaks
and secured. Drums and containers will be transported immediately to the storage area
via pallet and forklift. Chemicals will never be left at the receiving dock longer than
necessary.
Preventative Maintenance and Good Housekeeping Program
Preventative Maintenance Program
A preventative maintenance program is in effect at the facility, and inspections occur on a
monthly basis. The program documents schedules of inspections and maintenance
activities of stormwater control systems, plant equipment and systems. Inspection of
material handling areas and regular cleaning schedules of these areas is incorporated
into the program for the following areas!
• Shipping and receiving docks: Raw materials and finished goods will be stored
inside the facility. Miscellaneous 55-gallon drums (full or empty) will be stored in
designated holding areas, and not on or near any dock areas, or outdoors.
• Material handling equipment: Forklifts, pallet jacks, and other material handling
equipment where failure could lead to spills or leaks that could cause
contamination of surface water.
Employees at the facility responsible for conducting inspections and completing repairs
on machinery, equipment, and drainage systems that could affect stormwater pollution
will be trained in the correct procedures for such tasks. Documentation of this training will
be kept on file with other stormwater documents.
Good Housekeeping Program
Berk-Tek LLC is committed to maintaining a clean and therefore safe work environment for its employees.
Housekeeping issues shall be addressed in company safety meetings. In addition, department managers
and employees are responsible for maintaining a clean and orderly work place in order to improve safety,
reduce chance of spills, and reduce possible contamination of stormwater.

Measurements must be done twice annually during a measureable storm event. Grab samples
need to be collected within the first 30 minutes of discharge. Samples should be collected at each
stormwater discharge outfall (SDO). Total toxic organics sampling is only required for those
facilities which preform metal finishing operations, manufacture semiconductors, electronic
crystals, or cathode ray tubes. For each sampled measureable storm event, the total precipitation
must be recorded. An on -site rain gauge or local rain gauge reading must be recorded.
Training Schedules
Berk-Tek LLC will conduct training at various levels of the organization that directly or indirectly
assist in minimizing the probability of contamination of stormwater discharges with industrial
pollutants. The stormwater pollution prevention team is responsible for implementing the training
requirements outlined in this section. The following types of training are currently conducted or
will be implemented at the facility to minimize stormwater pollution.
Spill Prevention and Response
Berk-Tek LLC will provide training in the following areas:

Good Housekeeping Program
See the Good Housekeeping Program section of this plan. The training outlined in that section
will be conducted annually.
Amendment and Annual Update
• This plan shall be reviewed and updated on an annual basis (as required).
• All sampling (as required in the monitoring section of the stormwater permit), measurements,
inspections, maintenance activities, BMP implementation, and training shall be documented.
Such documentation shall be kept on -site for period of five years.
• If the director (NCDENR) determines that the plan does not meet one of more of the minimum
requirements of the permit, Berk-Tek LLC will submit, within thirty days of notification, a time
schedule for modifying the plan to meet minimum requirements.

need to be collected within the first 30 minutes of discharge. Samples should be collected at each
stormwater discharge outfall (SDO). Total toxic organics sampling is only required for those
facilities which preform metal finishing operations, manufacture semiconductors, electronic
crystals, or cathode ray tubes. For each sampled measureable storm event, the total precipitation
must be recorded. An on -site rain gauge or local rain gauge reading must be recorded.
Training Schedules
Berk-Tek LLC will conduct training at various levels of the organization that directly or indirectly
assist in minimizing the probability of contamination of stormwater discharges with industria!
pollutants. The stormwater pollution prevention team is responsible for implementing the training
requirements outlined in this section. The following types of training are currently conducted or
will be implemented at the facility to minimize stormwater pollution.
Spill Prevention and Response
Berk-Tek LLC will provide training in the following areas:
• Hazard communication
• Spill response/stormwater awareness
Goon Housekeeping Program
See the Good Housekeeping Program section of this plan. The training outlined in that section
will be conducted annually.
Amendment and Annual Update
• This plan shall be reviewed and updated on an annual basis (as required).
• All sampling (as required in the monitoring section of the stormwater permit), measurements,
inspections, maintenance activities, BMP implementation, and training shall be documented.
Such documentation shall be kept on -site for period of five years.
• If the director (NCDENR) determines that the plan does not meet one of more of the minimum
requirements of the permit, Berk-Tek LLC will submit, within thirty days of notification, a time
schedule for modifying the plan to meet minimum requirements.
• Documentation associated with the facility and equipment inspections will be kept on file.
