This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in Non -discharge Permit No. WQ0003717. The findings and comments noted during this inspection are provided in the enclosed copy of the inspection report entitled "Compliance Inspection Report". There were no significant issues or findings noted during the inspection and therefore, a response to this inspection report is not required. The purpose of the evaluation was to verify that the facility is operating in compliance with the conditions and limitations specified in Permit No. WQ0003717. Ronnie "Geno" Kennedy with Agriment Services, Inc. and Kevin Krum with Parks Family Meats were present along with Tyler Benson and myself (Bryan Lievre) during the inspection. The facility was issued Permit No. W00003717 for an 1,100 gpd surface irrigation and disposal facility consisting of a grease trap, an aeration basin, a clarifier with sludge return, a chlorine contact tank, a polishing lagoon of approximately 180,000 gallons, and a spray irrigation system with a 100 gpm pump and approximately 8,836 square feet of spray field with two spray heads, each with an influence diameter of 75 feet. The facility has not made use of the on -site spray irrigation system since prior to the date of the last permit renewal in 2015 and during this period, has sent all wastewater into the Town of Warsaw collection system. The facility applied for and received approval of Permit No. WQ0029886 on March 31, 2006 to send wastewater into the Town of Warsaw collection system. Permit No. W00029886 provided for the construction and operation of a 32 gpm duplex pump station with audible and visual alarms with a portable generator receptacle with telemetry/auto dialer and 1,045' of 2" force main for the discharge of up to 10,000 gpd. The current Permit No. WQ0003717 was renewed on May 12, 2015 as a "Zero Discharge" permit, meaning that the system cannot operate until the permit is modified, an ORC is designated, the lagoon and spray areas are restored to an operational condition, and an inspection of the system is performed by the Wilmington Regional Office. During the inspection, wastewater was being routed into the grease trap, then into the aeration basin, then into the clarifier, and then into a pump chamber where it is then sent into the Warsaw collection system. While on -site, activated sludge was being returned to the aeration basin and neither significant odors nor significant foaming were observed. The spray fields seemed to have been in fair condition. A fair amount of vegetation was growing within the lagoon and it was recommended to facility staff to keep the structure maintained in the event it is needed in future operations. By the time of preparation of this summary report, personnel had already cut and removed vegetation from the lagoon, repaired a monitoring well stick-up casing and replaced locks on all three monitoring well stick-up casings. The facility is compliant with the existing non -discharge permit WQ0003717. Please review the comment sections of the inspection report for more details. System is currently required to and is permforming annual groundwater sampling from all three on -site monitoring wells (i.e. MW-1, MW-2 and MW-3). Samples were most recently 11/13/2019, but operator did not have copies of results from Waters Labs. 2018 results were reviewed and matched prior reporting results with no violations noted. Annual soil sampling within spray fields is not currently required nor being conducted, since spray fields are not being utilized. Sampling of the effluent is only beina oerformed by the town or the facilitv if an issue is encountered. The operation of these sDrinklers was not observed as the system has not been utilized for some time. On the date of the inspection, the fields were covered by trimmed crab grass and no ponding was observed. The entire property is enclosed with a chain link fence to limit public access. The facility obtains potable water from the Town of Warsaw. The three monitoring wells located on the property (i.e. MW-1, MW-2 & MW-3) were stick-up wells with diameters of 2", 4" and 4", respectively. The locks on each of the stick-ups were recently cut and removed for samDlina activities and at my reauest were reDlaced later that day. A concrete Dad was also observed around each of the monitorina wells. Page 5 of 5
