Dear Mr. Weber:
A letter of request for an Authorization to Construct was received February 24,
1987, by the Division and final plans and specifications for the subject project have
been reviewed and found to be satisfactory. Authorization is hereby granted for the
construction of a 0.027 MGD wastewater treatment facility consisting of an influent
bar screen; a 27,000 gallon capacity aeration tank with dual 150 CFM blowers; a 3,700
gallon capacity aerator sludge holding tank; a 9,600 gallon capacity clarifier with
mechanical sludge collector., travelling air lift pump and skimmer; a 1,600 gallon
capacity baffled chlorine contact tank with tablet chlorinator; a 1,000 gallon
post-aeration tank; a recording effluent flow meter; and all associated piping,
valves, fittings, and appurtenances to serve Frye Bridge Estates.
This Authorization to Construct is issued in accordance with Part III paragraph C
of the NPDES Permit No. NCO065587 issued June 1, 1987, and shall. be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit: No. NC0065587.
The Permittee must employ a certified wastewater operator in accordance with Part
I.11 paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G. S. 143-215. 1 and in a manner approvable by the North Carolina
Division of Environmental Management.

The Winston-Salem Regional Office, telephone number 919/761-235.1, shall be
•notifi.ed at least twenty-four (24) hours in advance of start up of the installed
facilities so that an in-place inspection can be made of said facilities prior to
operation. Such notification to the Regional Supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding
State Holidays.
In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the Permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional wastewater
treatment and disposal facilities.
