Dear Mr. Simaan:
The Division of Water Quality's NPDES Unit has reviewed your request for an Authorization to Construct at
the Crown Mobile Home Park WWTP and finds the proposed improvements to be satisfactory and hereby
grants authorization for the installation of a 16,000 gallon equalization tank(with associated aeration as
described in your submittal) and the conversion of the existing grease trap to a preaeration tank.
This Authorization to Construct is issued in accordance with NPDES Permit No. NCO055255, issued
July 23, 2001, and shall be subject to revocation unless the wastewater treatment facilities are
constructed in accordance with the terms and conditions of the permit. This Authorization to
Construct does not represent an increase in flow from the permitted design flow of 0.042 MGD.
The Permittee shall notify the Water Quality Supervisor of the Winston-Salem Regional Office, telephone
number (336) 771-4600. at least forty-eight (48) hours prior to operation of the installed facilities, so that an
in-place inspection can be made. Notification shall be made during the normal office hours of 8:00 a.m. until
5:00 p.m., Monday through Friday.
Engineer's Certification
Upon completion of construction and prior to operation of the modified facility, the Permittee shall provide
the Division with a certification (copy enclosed) from a professional engineer registered in North Carolina
certifying that the permitted facility has been installed in accordance with the NPDES Permit, this
Authorization to Construct, and the approved plans and specifications. The Certification should be submitted
to: NCDENR/ DWQ, NPDES Unit, 1617 Mail Service Center, Raleigh, North Carolina 27699-1617.
Requirements for Certified Operator
The Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC)
of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least
equivalent to or greater than the classification assigned to the wastewater treatment facilities by the
Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate
type and grade to comply with the conditions of Title 15A, Chapter 8G, .0200. The ORC of the facility must
visit each Class l facility at least weekly and each Class 11, 111, and!V facility at least daily, excluding

weekends and holidays;and must properly manage and document daily operation and maintenance of the
facility and comply with all other conditions of Title 15A, Chapter 8G, .0200.
Additional Requirements
The Operational Agreement between the Permittec and the Environmental Management Commission is
incorporated herein by reference and is a condition of this Permit. Noncompliance with the terms of the
Operational Agreement shall subject the Permittee to all sanctions provided by G. S. 143-215.6 for violation
of or failure to act in accordance with the terms and conditions of this Permit.
Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittee
to an enforcement action by the Division of Water Quality in accordance with North Carolina General Statute
143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from complying with any and
all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local,
state, and federal) which have jurisdiction.
The Permittee shall maintain a copy of the approved plans and specifications on file for the life of the
facility. One (1)copy of the approved plans and specifications is enclosed for your records.

NATALIE-The grease trap has not been functioning properly and the owner
will convert it to a small preaeration tank. We have told him that he
can do that since the grease trap was useless and unnecessary. Call if
any questions.-Steve

PART III - EVALUATION AND RECOMMENDATIONS
The facility has had problems with hydraulic surges causing solids wash outs. Effluent limits
have been exceeded and penalties assessed. This A to C is for construction of an aerated 16,000
gallon flow equalization basin.
Recommend the A to C be issued.

The scope of the project includes the installation of one 16,000 gallon equalization tank and

RECOMMENDATIONS

REVIEW SUMMARY
Executive Summary
The subject facility's existing permit was issued on July 23, 2001. The treatment works currently
consist of:
•Bar screen
•Grit and grease tank
-Aeration tanks in series
*Clarifiers in parallel
-Tablet chlorination
•Reaeration tank with diffused air
-Aerated sludge digester
The Winston-Salem Regional Office reports that the facility has experienced problems with
hydraulic surges, which then cause solids washouts. This has led to the several violations of
permit limits. To assist with compliance and to alleviate this problem of surging, the installation of
the 16,000 gallon equalization tank is recommended.

Review Notes:
According to Steve Mauney of the Winston-Salem Regional Office (WSRO), the existing grease trap
has not been functioning for some time and so will be converted into a small preaeration tank. The
WSRO does not feel that a grease trap is necessary for this system. The grease trap will be
bypassed while the work on it is performed. Once the work is complete, wastewater will now to it
by gravity.
The proposed equalization tank provides more than the required 25% storage and seems to be
needed at this site due to the flow surges experienced at the plant. Hopefully, the installation of
equalization will enable the Permittee to achieve a more consistent level of treatment.
Recommendations
I recommend that the Authorization to Construct be issued for the installation of a 16,000 gallon
equalization tank and the conversion of the existing grease trap to a small preaeration tank.

Many of the ongoing improvements at the Crown Mobile Home Park are considered to be
maintenance items and are being handled under the existing permit. This is based on discussions
with Steve Mauney of the Winston-Salem Regional Office. The narrative below is for items to be
done under the requested Authorization to Construct Pennit.
Project Narrative: Construct one 16,000 gallon equalization tank which is to be located
upstream of the existing grease trap. The equalization tank is to be aerated by diffusers and air
from the existing blowers. Piping for return sludge to pumped to the equalization basin shall be
added. The basin shall be furnished with two 0.4-Horsepower submersible pumps which are rated
for 40-GPM at 13-14 Feet. A gravity bypass shall be provided to the grease trap and bypassing
the grease trap. (As ongoing maintenance, the grease trap is being cleaned and aeration is being
added).

On behalf of the Crown Mobile Home Park, we are pleased to submit an application package for an Authorization
to Construct Permit (ATC) with supporting documentation for the Crown Mobile Home WWTP. Please find
enclosed the following documents:
1. Three sets of Detailed Plans and Specifications (8'/2"x I I"format).
2. Two sets of design calculations.
3. Two sets of Flow Schematic 0/2"x I I"format).
4. A Construction Sequence Plan is not provided. Temporary by-pass piping will be provided during
installation of equalization basin.
5. Special Order By Consent Application with cover letter being sent concurrently to Mr. Dave
Goodrich.
This application addresses the permit limitations and conditions for this facility. The ATC is needed so that an
equalization basin and appurtenances can be permitted and constructed to correct the violations at the facility.
