Box 429 Kings Mountain, N.C. 28086 SUBJECT: Permit No. NCO020737 Authorization to Construct City of Kings Mountain Pilot Creek Wastewater Treatment Plant Aeration Basin Improvements Cleveland County Dear Mayor Moss: A letter of request for an Authorization to Construct was received July 22, 1987, by the Division and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of a 4.0 MGD wastewater treatment facility consisting .of improvements to three aeration basins which will consist of: Basin 1:Six 15 HP Aqua-Jet Aerators, two 20 HP D.D.M. mixers, concrete liner Basin 2:Six 15 HP Aqua-Jet Aerators, two 20 HP D.D.M. mixers Basin 3:One 30 HP Aqua-Jet Aerator, four 40 HP Aqua-Jet aerators, four 20-HP D.D.M. mixers. This Authorization to Construct is issued in accordance with Part III paragraph C of. the NPDES Permit No. NCO020737 issued November 20, 1987, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit. No. NCO020737. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph D of the referenced permit. The sludge generated from these treatment facilities must be disposed of in accordance with G. S. 143-215. 1 and in a manner approvable by the North Carolina Division of Environmental Management. The Asheville Regional Office, telephone number 704/251-6208, shall be notified twenty-four (24) hours in advance of operation of the installed system so that an in-place inspection can be made. Within 90 days of issuance of this authorization to construct two (2) monitor wells shall, be installed downgradient of the existing lagoons. The location and construction details for these wells must be approved by the Asheville Regional Office, from which a well construction permit must be obtained. The two (2) new monitor wells and the existing upgradient well (No. Q73V1) must be sampled every March, July and November for the following parameters: NO3 Total Ammonia TDS Water Level (measured prior to sample collection) TOX Total Suspended Solids (Initial sampling of new wells only) PH TOC Chloride Total Coliform The analytical methods used for TOX must be capable of detecting the total of all halogenated organic compounds present at a concentration of 5.0 parts per billion (ppb) or greater. Any individual halogenated organic compound present at a concentration at or above the method detection limit (MDL) must be identified, quantified and reported with the sampling results. The results of the sampling and analysis shall be sent to the N.C. Division of Environmental Management on Form GW-59 • [Compliance Monitoring Report Form] every April, August and December. Any additional groundwater quality monitoring, as deemed necessary by the Department, shall be provided. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications.
