Dear Mr. Alphin:
A letter of request for Authorization to Construct was received October 22, 1985,
by the Division and final plans and specifications for the subject project have been
reviewed and found to be satisfactory. Authorization is hereby granted for the con-
struction of a 0.020 MGD wastewater treatment facility consisting of a 6,600 gallon
capacity equalization chamber with dual 30 GPM pumps with high water alarms, a blower
capable of delivering 33 cfm, inlet bar screens, two (2) 10,000 gallon capacity aera-
tion tanks, two (2) 1,630 gallon capacity clarifiers, a 2,600 gallon capacity sludge
holding tank, two (2) blowers capable of delivering 92 cfm each, a 1,000 gallon capacity
chlorine contact tank with a tablet chlorinator, an effluent pump station with dual 30
GPM pumps with high water alarms and all associated valves, piping and appurtenances to
serve the Scarlett Acres Mobile Home Estates located in Forsyth County.
This Authorization to Construct is issued in accordance with Part III paragraph
C of NPDES Permit No. NC0061204 issued June 10, 1985, and shall be subject to revocation
unless the wastewater treatment facilities are constructed in accordance with the con-
ditions and limitations specified in Permit No. NC0061204.
The Permittee must employ a certified wastewater operator in accordance with Part
III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina Division
of Environmental Management.
• The Winston Salem Regional Office, telephone number 919/761-2351 shall be notified
in advance of operation of the installed system so that an in-place inspection can be

Such notification to the Regional Supervisor shall be made during the normal office
hours from 8:00 A.M. until 5:00 P.M. on Monday through Friday, excluding State
Holidays.
In event the facilities fail to perform satisfactorily in meeting it's NPDES
permit effluent limits, the Permittee shall take such immediate corrective action as may
be required by this Division, including the construction of additional wastewater treat-
ment and disposal facilities.
