Duke Energy requests the subject general permit be renewed and reissued. The above referenced permit
expires July 31, 2020. As mandated by North Carolina Administrative Code 15A NCAC 21-1.0105 (e), this
permit application for renewal is being submitted at least 180 days prior to expiration of the current
permit.
Please find enclosed one original and one copy of the Notice of Renewal Intent for each of the
hydroelectric facilities named above.

➢ Site map. If the discharge is not directly to a stream, the pathway to the receiving stream must
be clearly indicated. This includes tracing the pathway of a storm sewer to its discharge point.
➢ Authorization for representatives. If this application will be submitted by a consulting
engineer (or engineering firm), include documentation from the Permittee showing that the
consultant submitting the application has been designated an Authorized Representative of the
applicant, per 15A NCAC 0211 .01IK121(1).

Notes: 'These outfalls are maintained for emergency bypass procedures only. During
normal operation, these sumps discharge into the station sump (outfall 013).
2 Discharge is due to seepage through dam. Flow changes with seasonal
changes in temperature. Maximum flow occurs during winter and is at a
minimum during summer.
