Received on 5/10/19 Limit Exceedance Violations NOV 2019-LV-0330 • 001 BOD 1 /31 /2019 Limit 10 Reported 11 Monthly avg exceedance 001 TN 1 /31 /2019 Limit 10 Reported 11.35 Monthly avg exceedance The second train was being brought online in some extreme cold weather and without the immediate capability to feed MicroC to denitrify. Spotty and unstable biological activity, led to these two issues. Reporting violations • 001 Kieldahl Nitrogen 1 /31 /2019 Limit monitor Reported not reported • 001 PO4 1 /31 /2019 Limit monitor Reported not reported For some reason, the collection and analysis of theses monitor only parameters was overlooked, and not part of the normal test package issued by the contract lab. We became aware of the situation and rectified it with the lab, somewhere around mid -April. Also a checklist has been developed to prevent the reoccurrence of this issue.
