Mr. Tankard,
Our office received the notices of violation on the 27Ih of November and was not distributed to staff
until after the return from the Thanksgiving Holiday. We have taken action to make a quick response.
The Operator has provided the following information:

The data was entered into the spread sheet in error, using the excel spread sheet numbers and not the
calendar dates on the NDMR forms.

A copy of the new permit requirements was submitted to the lab. The lab has not been sampling for
phosphorus. The absence of this parameter was not detected until the receipt of the DWQ notification.
The lab has been contacted and the phosphorus sampling criteria has been corrected. This will be in
effect starting December 2019.
A copy of the amended reports will be e-mailed mailed to you and mailed to the DWQ processing
center. If there is any additional information we can provide, please let me know.
