I left you a voicemail message today to discuss with you our request for additional information to evaluate your
Nationwide permit request for the First Health Hospice in Moore County. Our Office would like to receive the
following items in order to determine your application complete:
1. Verification of soils and substrate that would support a bottomless culvert. The Sandhills have not proven to
have reliable soils and substrate for their use. Please provide information demonstrating that the Con-Span will
be set on non-scourable bedrock.
2. Plan view and cross-sectional view of impact area. Please provide drawings showing the dimensions of
impacts with respect to the 404 jurisdictional line to justify your 0.2 acre of wetland and 80 linear feet of stream
impacts. These must be on 8.5 x 11" sheets.
3. Please provide a mitigation proposal. Compensatory mitigation is required for projects exceeding 0.10 acre of
impact. Plese see our website for more information:

Please provide the above information within 2 weeks otherwise we will consider your application incomplete and
