IV. ADDITIONAL REGIONAL STAFF REVIEW ITEMS

On November 26, 2019 Division staff conducted site visits to the proposed new land application sites included in the
application for permit modification. The proposed new source facility was recently visited by ARO staff and therefore
was not visited during this review. There were two site maps that need to be corrected and one site summary form that
needs to be corrected. These items require additional information to be requested:
1. The Licensed Soil Scientist (LSS) report for Site JH-01 provided four soil profile descriptions. All four
descriptions reflect the Pacolet Soil Series. However, the application forms indicated the predominant soils
present are Cecil Soil Series. The application forms need to be revised to match the LSS's report. This will
lower the agronomic loading for this field from 209 lbs. of PAN/acre/yr. to 158 lbs. of PAN/acre/yr. for the
intended crops.
2. There is a water supply well present adjacent to the field for Site RD-113 that is not shown on the site map. See
attached site map sketch for approximate location. This well and the associated buffer need to be added to
the site map. If applicable, updated acreages need to be provided on application forms.

3. There is a water supply well present inside the land application boundary for Site RD-02 that is not shown on
the site map. See attached site map sketch for approximate location. This well and the associated buffer need
to be added to the site map. Updated acreages need to be provided on application forms.
In addition to the items described above, the following should be noted from this review:
• There were no topographic maps provided for the proposed new land application sites as required. Review has
been completed for this project; therefore, this does not warrant additional information to be requested.
However, this deficiency in the application was discussed with the Permittee and should not be allowed moving
forward.
Other Notes:
- Reported sufficient proposed acreage to support new annual dry tonnage at the PAN concentrations provided.
- Regulatory limits reported within limits (TCLP, metals, Path & VAR, and SAR).
- O&M Plan, Sampling Plan and Spill Plans appear to be sufficient.
