Dear Mr. Allen:
On October 30, 2009,the Division of Water Quality(DWQ)received your application dated September
23, 2009 for the above referenced project. The DWQ has determined that your application was
incomplete and/or provided inaccurate information as discussed below. Please provide the following so
that we may continue to review your project.
Additional Information Requested:
1. The cover letter reflects the project is for Buffers only; however, on page 5 of 11 there are wetland
and stream impacts noted. Whether the impacts are permanent or temporary, important or unimportant,
intermittent or perennial they need to be accounted for. Please remit$570 made payable to Division of
Water Quality(DWQ).
2. Incorrect and/or missing wetland, intermittent-perennial stream and/or buffer impacts.
In reviewing wetland total impacts on page 5 of 11, section 2f and 2g shows .39 acres of impact; however,
when tallying the impact it comes to 0.38 acres Please supply one(1) corrected original and four(4)
corrected copies of page 5.
