In accordance with your application for discharge permit received on December 2, 1998
by the Division, we are forwarding herewith the subject permit to discharge under the subject
state - NPDES permit. This permit is issued pursuant to the requirements of North Carolina
General Statute 143-215 .1 and the Memorandum of Agreement between North Carolina and
the U.S. Environmental Protection Agency dated December 6, 1983 and as subsequently
amended.
If any parts, measurement frequencies or sampling requirements contained in this
permit are unacceptable to you, you have the right to an adjudicatory hearing upon written
request within thirty (30) days following receipt of this letter. This request must be in the form
of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and
filed with the Office of Administrative Hearings, Post Office Drawer 27447, Raleigh, North
Carolina 27611-7447. Unless such demand is made, this decision shall be final and binding.
The following changes have been made to the above referenced permit and were noted
in the draft permit previously sent to you:
Monitoring requirements for Total Nitrogen and Total Phosphorus have been
increased to monthly. This is based on the need to gather additional nutrient data
prior to the development of a nutrient management strategy in the Yadkin Basin.
Since nutrients are a concern in the upper sub -basins the Yadkin Basin, during the
next permitting cycle, the Division will assess whether NSW designations are
appropriate for all or part of the watershed. As a result, monitoring has been
increased for most of the dischargers in the upper parts of the basin.
Monitoring for NH3-N has been inserted into the permit. Since this facility has had
problems passing toxicity tests this monitoring will help to determine if NF13 is the
cause of toxicity.
Monitoring for Oil and Grease has been eliminated. DMR data from July 1997 to
present has shown no oil and grease in the effluent.

Based on past DMR data, the Division recommends the Town install dechlorination and
post aeration to avoid permit violations and improve effluent quality. Since the effluent from this
facility accounts for approximately 81% of the receiving stream at low flow conditions, the
addition of these components will improve instream water quality.
Any new components may be placed in service once plans and specifications have been
submitted and the Division has issued an Authorization to Construct (ATC). The ATC review
process typically takes 90 days to complete.
Please take notice that this permit is not transferable except after notice to the Division of
Water Quality. Part 11, EA. addresses the requirements to be followed in case of change in
ownership or control of this discharge. The Division of Water Quality may require modification or
revocation and reissuance of the permit.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources,
the Coastal Area Management Act or any other Federal or Local governmental permit that may be

A. (2.) CHRONIC TOXICITY PASS/FAIL PERMIT LIMIT (Quarterly)
The effluent discharge shall at no time exhibit chronic toxicity using test procedures outlined in
the "North Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised November 1995,
or subsequent versions.
The effluent concentration at which there may be no observable inhibition of reproduction or
significant mortality is 81% (defined as treatment two in the procedure document). The permit
holder shall performguarterlg monitoring using this procedure to establish compliance with the
permit condition. The tests will be performed during the months of January, April, July, and
October. Effluent sampling for this testing shall be performed at the NPDES permitted final
effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter
code TGP3B. Additionally, DWQ Form AT-1 (original) is to be sent to the following address:

Test data shall be complete and accurate and include all supporting chemical/physical
measurements performed in association with the toxicity tests, as well as all dose/response data.
Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine
is employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring
is required, the permittee will complete the information located at the top of the aquatic toxicity
(AT) test form indicating the facility name, permit number, pipe number, county, and the
month/year of the report with the notation of "No Flow" in the comment area of the form. The
report shall be submitted to the Environmental Sciences Branch at the address cited above.
Should any single quarterly monitoring indicate a failure to meet specified limits, then monthly
monitoring will begin immediately until such time that a single test is passed. Upon passing, this
monthly test requirement will revert to quarterly in the months specified above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then
monthly monitoring will begin immediately until such time that a single test is passed. Upon
passing, this monthly test requirement will revert to quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina
Division of Water Quality indicate potential impacts to the receiving stream, this permit may be
re -opened and modified to include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum
control organism survival, minimum control organism reproduction, and appropriate
environmental controls, shall constitute an invalid test and will require immediate follow-up
testing to be completed no later than the last day of the month following the month of the initial
monitoring.

There are just three minor changes in this permit since last renewal. They are noted in the table
above. This facility is a 100% domestic (per last WL A) WWTP with no pretreatment program.
The Jan. 1987 WLA completed by C. Sanderson gave secondary limits for at 0.2 MGD grid DO of 5
and 3 for summer and winter that would protect the instream 00 standard. The reason the 3 DO
limit was given was that at that time the plant was experiencing low DOs In there effluent:. Presently
DMR data show that they are do have low DOs !Also there
are occasional instream DO violations upstream and downstream. rave iT >i- s.o 01 Y
The facility would have NH3 limits for toxicity at 2 and 4 but it appears they have elected to have a
WET test instead. Since NH3 is not monitored it is difficult to determine the cause of the toxicity of
the effluent. Therefore it has been added as monitoring requirement for this permit It is;
recommended that this facility install dechlorination and post aeration to avoid permit violations.
Oil and Grease monitoring has been removed since no oil or grease has been detected since July
1997.
Review of DMR data from January 1997 to December 1998 indicates the facility has hadlthree
months of noncompliance for effluent DO and seven months of non-compliance for fecal. flll-eiNer

The Town of Boonville would like to renew its NPDES permit for the 200,000 gallon per day cn
secondary wastewater treatment plant located on the northeast side of town at the end on Lon
Avenue
The existing facility treats the average flow of t110,000 gallons per day with very- good results.
BOD5 removal is at 98%, total suspended solids are less that 5 ppm and influent TKN is 10.4.
The high maximum flow of 0.6 mgd is thought to be almost entirely inflow, as the flow increases
rapidly during major rainfall events and subsides almost as fast afterward. The town of Boonville
has purchased new equipment to televise and clean the wastewater lines. We feel that major
inflow points can be located with this equipment.
Sludge Management Plan - Sludge is continually recirculated form the clarifier to the aeration
basin. Periodically the sludge blanket in the clarifier is drawn off to the sludge holding tank. This
tank is decanted and the thickened sludge is wasted to the sludge drying beds. After thorough
drying this sludge is disposed on in a approved landfill in the Kernersville area.
Municipal Engineering Services will act as our authorized representative during the NPDES permit
