2j. Provide a brief description of this project (attach a site plan): The proposed project is the installation of a 24" water
main starting from a 24" existing water main near the water treatment plant in Wilson Mills, Johnston County. The 24"
water main will run to Little Creek Church Road and split into two 16" water lines. They will connect to two existing 16"
water lines. One of the connection is located at the intersection of Little Creek Church Road and Steel Bridge Road.
The second connection point is at the intersection of Wellons and Cleveland Road.

Attached is the stream determination initial interest form for the Wilson Mills to Cleveland Area 24" Water Transmission
Line project. Since the project is so long, the project map is too large (18MB) to attach to an email. I will provide you
with all the maps (project map, USGS, soils) for each area when you get here on Wednesday. I am located at 309 E.
