However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on November 6, 2019. Please be aware that you are responsible for meeting all requirements set fo rth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant’s responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through C, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. Triplett October 7, 2019 Page 2 of 3 A. Engineering Calculations: 1. As per the DCAR 06-16 form submitted, in the residuals quality information, the amount of residuals entered is 700 dry tons per year as compared to 3,400 in the previous permit. Please provide a quantitative justification and calculation for the total residual tonnage to be certified. B. Site Map: 1. As per 15A NCAC 02T .1104, for new and expanding residuals treatment and storage facilities: 2. Site plans. If required by G.S. 89C, a professional land surveyor shall provide location information on boundaries and physical features not under the purview of other licensed professions. Site plans or maps shall be provided to the Division by the applicant depicting the location, orientation, and relationship of facility components, including:  A scaled map of the site, with topographic contour intervals not exceeding 10 feet or 25 percent of total site relief and showing all facility-related structures and fences within the treatment and storage areas;  The location of each of the following that are located within 500 feet of a waste treatment, or storage site, including a delineation of their review and compliance boundaries: (i) wells, including usage and construction details if available; (ii) ephemeral, intermittent, and perennial streams; (iii) springs; (iv) lakes; (v) ponds; and (vi) other surface drainage features;  Setbacks as required by Rule .1108 of this Section; and  Site property boundaries within 500 feet of all treatment and storage facilities. C. Operation & Maintenance Plan: Please submit an O & M plan in accordance with 15A NCAC 02T .1110 that includes at a minimum: 1. Operational functions; describe the operation of the program to show what operations are necessary for the program to function and by whom the functions are to be conducted. 2. Maintenance schedules; may include equipment calibration, maintenance of signs, etc. 3. Safety measures; may include safety training program, manuals, signs, etc. 4. Spill response plan; including control, containment, remediation, emergency contact information, etc. 5. Inspection plan including the following information;  Names and titles of personnel responsible for conducting the inspections. Ms. Elisa R. Triplett October 7, 2019 Page 3 of 3  Frequency and location of inspections, including those to be conducted by the ORC, and procedures to assure that the selected location(s) and inspection frequency are representative of the residuals management program.  Detailed description of inspection procedures including record keeping and actions to be taken by the inspector in the event that noncompliance is observed. 6. Sampling and monitoring plan including the following information;  Names and titles of personnel responsible for conducting the sampling and monitoring.  Detailed description of monitoring procedures including parameters to be monitored.  Sampling frequency and procedures to assure that representative samples are being collected. Fluctuation in temperature, flow, and other operating conditions can affect the quality of the residuals gathered during a particular sampling event. The sampling plan shall account for any foreseen fluctuations in residuals quality and indicate the most limiting times for residuals to meet pathogen and vector attraction reduction requirements (e.g. facilities that land apply multiple times per year but have an annual sampling frequency, may need to sample during winter months when pathogen reduction is most likely to be negatively affected by cold temperatures.
