REQUEST FOR MORE INFORMATION
Dear Sir or Madam:
On August 9, 2006 the Division of Water Quality (DWQ) received your undated and unsigned AFTER-
THE-FACT application, to impact 160 or 163 linear feet of perennial stream to conduct stream bank
stabilization at he site. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below. The DWQ will require five copies of the additional
information requested below in order to process your application to impact protected wetlands and/or
streams on the subject property. Please provide the following information so that we may set up and
review your project.
Additional Information Requested:
1. Please show all stream impacts, including all fill slopes, dissipaters, and bank stabilization on the
site plan. No site plan was submitted for the 163-foot stream impact area.
2. Please provide cross section details showing the provisions for aquatic life passage.
3. In the table contained within section VI, No. 4, of your PCN application, you have listed the
stream impact as 1601inear feet and 163 linear feet. Please clarify which impact is correct and
make the appropriate changes within your PCN application.
4. Please sign and date your PCN application before you resubmit it.
In regards to your mitigation plan, please submit a planview that contains longitudinal profiles
and cross sections of structures to be installed, including cross sections of pools and riffles,
details of grade control, and/or thalweg management structures and tie-ins to the existing
channels at up and downstream locations. Please use a "reference reach", not a conceptual reach
for morphological characteristics and planviews. Additionally, please include details for the
types of trees and shrubs to be used after the banks are stabilized with non-invasive grasses and

matting as referenced on page 10 of the mitigation plan. The planting plan needs to be a
minimum of 360 stems/acre.
Please contact the DWQ within three weeks of the date of this letter to verify that you have received this
letter and that you remain interested in continuing to pursue permitting of your project and will be
providing the DWQ the requested information at a later date. Please contact me in writing. If we do not
hear from you within three weeks, we will assume that you no longer want to pursue this project and we
will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
