Facility Located at 143 Woodlawn Drive, Rockingham County
Dear Mr./Ms. Ramirez or Current Resident:
Our records indicate that this property is covered under the above referenced Certificate of Coverage,
which covers wastewater systems that discharge treated effluent to surface water. A copy of the NPDES General
Permit and supporting documentation are enclosed. Please review these materials carefully.
This permit is not transferrable without written notification to the Division of Water Resources.
Additionally, coverage under this permit requires payment of an annual fee, currently in the amount of $60. Our
records show that an ownership change request has not been received.
NC General Statute 143-215.1(a) requires a valid permit in order to discharge wastewater to waters of
the State. Please note that, pursuant to NC GS 143-215.6A, a civil penalty of not more than twenty-five thousand
dollars ($25,000.00) may be assessed against any person who violates or fails to act in accordance with the
terms, conditions or requirements of any permit issues pursuant to GS 143-215.1. The primary intent of this
letter is to provide an opportunity to bring the permit into good standing by submitting the required
ownership change form and paying any overdue invoices that may be present. If you no longer require this permit
(for example, because you connected to a centralized sewer system), please contact this office. Your immediate
attention in this matter is appreciated.
