The inspection consists of (1) a review of documentation demonstrating compliance with 15A NCAC 02T .0403, which regulates wastewater collection systems that are not individually permitted, and (2) an on - site inspection of several collection system components. The attached inspection form notes the areas that were evaluated for the inspection, with notable findings outlined as follows. 1. Operation and Maintenance Plan - .0403(a)(3) requires an operation and maintenance plan to be developed and implemented. Such a plan was not available for review during the inspection; operators thought they had such a plan but were unable to locate it before the inspection. 2. Grease Control - .0403(a)(8)(B) requires a grease control program to be in place that includes bi-annual distribution of grease educational materials to users of the collection system. Operators said they believed education material was distributed once per year, but documentation was not available. Material should be distributed twice per year and the distributions documented as required. Documentation - .0403(a)(10) requires documentation to be kept showing compliance with the other sections of the Rule. Documentation should be kept noting any maintenance and checks performed during pump station inspections, annual general system observations and grease material distribution. Site Inspections 1. Pump Stations — Both pump stations were inspected. At the pump station directly across from the wastewater treatment plant, the audible alarm did not work. Operators explained that the electrical panel was recently replaced and the alarm didn't work. At the other pump station, the telemetry system was inoperable, but a representative from the phone company was on site to evaluate the system during the inspection. Please ensure that alarms are operating correctly. However, the check only marks that an inspection was done, not what was done. A similar log for the other pump station wasn't available. Telemetry at PS#2 wasn't working but an AT&T technician was on site during the inspection to fix it. Audible alarm at PS#1 wasn't working during the inspection and should be repaired. Page 5 of 5
