4. Project Description and History
4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The proposed stream restoration project will restore 1,072 linear feet of Stingy Branch and 202 linear feet of UT Stingy Branch in Haywood County to improve natural ecological
conditions and streambank stability. The stream is incised with eroding streambanks causing impairment to water quality and aquatic habitats. The watershed drainage area for Stingy
Branch is 0.2 square miles, consisting of forest and low -density residential land uses. The land area surrounding the proposed restoration project is primarily forest. This project is
funded by a grant from the NC Clean Water Management Trust Fund and will include a permanent conservation easement on the riparian buffer extending at least 50 feet from the
streambanks.

The stream Will be enhanced using natural channel design techniques and native vegetation to improve ecological conditions and streambank stability. The project will provide a
sustainable ecosystem with stable streambanks and natural riparian buffer.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The stream Will be restored by grading the channel and streambanks to a naturally stable morphology and adding native vegetation for bank protection and soil strength. In -stream rock
and log enhanced riffles and vanes will be used to maintain grade control, enhance bedform diversity, and protect streambanks from erosive hydraulic forces. Native riparian vegetation
will be established to support long-term stream health. The project Will have a positive impact on the downstream watershed by reducing sediment loading. Equipment will include
trackhoe excavators, loaders, and track trucks for grading and structure installation.

construction of riffles, log steps, offset boulder steps, boulder j-hook vanes, bank sloping, the placement of biodegradable coir fabric along banks,

Stream restoration is needed to achieve ecological objectives and enhance infrastructure protection along the stream reach and downstream. Impacts
Will be minimized by maintaining as much as possible the existing stream channel and natural riparian buffer.

Construction impacts will be minimized by limiting in -channel work to low -flow conditions and temporarily diverting flow around the work zones with
native gravel/cobble berms. Streambanks and other disturbed areas will be stabilized within 3 days using grass seed, straw mulch, and biodegradable
erosion control mat. No impacts to existing wetlands will occur. Existing healthy stream buffers will be protected to the maximum extent possible.

4. THE CONTRACTOR IS RESPONSIBLE FOR THE PROJECT AREA UNTIL COMPLETION AND

SHALL TAKE ALL PRECAUTIONS NECESSARY AND SHALL BEAR ALL RISK OF LOSS OR
DAMAGE. THE CONTRACTOR WILL FURNISH ALL NECESSARY EQUIPMENT, TOOLS,
LABOR, TRANSPORTATION, AND SUPERVISION TO CLEAR THE RIGHT-OF-WAY
ACCORDING TO THESE SPECIFICATIONS AND APPLICABLE FEDERAL, STATE AND
LOCAL LAWS AND REGULATIONS. THE CONTRACTOR SHALL CONFINE ALL ACTIVITIES,
INCLUDING EQUIPMENT STORAGE, TO THE LIMITS OF DISTURBANCE, STAGING AREAS,
AND DESIGNATED CONSTRUCTION ACCESS POINTS.
5. THE MANNER IN WHICH THE CONTRACTOR DEALS WITH PEOPLE AND THEIR
PROPERTIES WHILE PERFORMING THIS WORK IS EXTREMELY IMPORTANT TO THE
PROJECT OWNER AND ENGINEER. THEREFORE, THE CONTRACTOR AND THE
CONTRACTOR'S REPRESENTATIVES SHALL MANIFEST A SPIRIT OF FRIENDLINESS AND
COOPERATION WHEN DEALING WITH PROPERTY OWNERS AND THE GENERAL PUBLIC
WHILE PERFORMING WORK UNDER THIS SPECIFICATION.
6. EXTREME CARE AND DILIGENCE SHALL BE EXERCISED BY THE CONTRACTOR TO
ASSURE THE SAFETY OF PERSONS, ANIMALS, AND PROPERTY. IF AT ANY TIME THE
PROJECT OWNER OR ENGINEER DETERMINES THAT THE CONTRACTOR'S METHODS OR
EQUIPMENT ARE INADEQUATE FOR SECURING THE SAFETY OF THE CONTRACTOR'S
EMPLOYEES OR THE PUBLIC, THE DESIGNATED REPRESENTATIVE MAY DIRECT THE
CONTRACTOR TO TAKE SPECIFIC ACTIONS TO ENSURE SAFETY. THE CONTRACTOR
SHALL IMPROVE METHODS AS DEEMED APPROPRIATE BY THE DESIGNATED
REPRESENTATIVE WITHOUT ADDITIONAL COST TO THE PROJECT OWNER, SO AS TO
ASSURE COMPLIANCE WITH THE PROJECT OWNER AND ENGINEER'S SAFETY
CONCERNS. FAILURE OF THE DESIGNATED REPRESENTATIVE TO MAKE THIS DEMAND
SHALL NOT RELIEVE THE CONTRACTOR OF ANY OBLIGATION TO ENSURE THE SAFE
CONDUCT OF ITS WORK.
7. THE CONTRACTOR SHALL MAINTAIN ALL LIGHTS, GUARDS, SIGNS, TEMPORARY
PASSAGES, OR OTHER PRECAUTIONS NECESSARY FOR THE SAFETY OF ALL PERSONS.
THE CONTRACTOR SHALL ABIDE BY ALL SAFETY RULES AND CONSTRUCTION
CONDITIONS REQUIRED BY GOVERNMENTAL AUTHORITIES AND OTHER ENTITIES,
INCLUDING RAILROADS, SO THE PUBLIC IS SAFEGUARDED FROM ACCIDENTS AND
DELAYS. GUARDS AND FLAGS REQUIRED BY GOVERNMENTAL OR RAILROAD
AUTHORITIES SHALL BE PROVIDED AT THE CONTRACTOR'S EXPENSE, UNLESS
DIRECTED OTHERWISE BY THE DESIGNATED REPRESENTATIVE. CONTRACTOR SHALL
AT NO TIME COMPROMISE EITHER SAFETY OR ENVIRONMENTAL REQUIREMENTS.
8. ANY ALTERNATE ACCESS PLANNED BY THE CONTRACTOR SHALL BE APPROVED BY
THE PROJECT OWNER AND ENGINEER PRIOR TO USE.
9. NO FILL IN WETLANDS MAY OCCUR. ALL EXCESS SOILS FROM STREAM STABILIZATION
AND CHANNEL WORK SHALL BE DISPOSED OF IN OLD STREAM CHANNEL TO OTHER
AREAS APPROVED BY THE PROJECT OWNER AND ENGINEER.
10. SITE SHOULD BE "STORM READY" AT THE END OF EACH WORK DAY AND WORK WEEK.
TOPOGRAPHIC SPECIFICATIONS AND NOTES
11. ELECTRONIC SURVEY DATA, BASE DRAWINGS AND SITE DATA WERE CURATED BY
JENNINGS ENVIRONMENTAL PLLC USING LOCAL, STATE AND FEDERAL GIS
RESOURCES, AND FIELD DATA COLLECTION ONSITE.
12. HORIZONTAL DATUM IS NAD83(2011) . VERTICAL DATUM IS NAVD88. ALL
COORDINATES ARE BASED ON NAD83(2011) AND ALL ELEVATIONS ARE BASED ON
NAVD88.
13. EXISTING GROUND SURFACES ARE BASED ON NC FMP LIDAR DATA GENERATED IN

COMPUTATIONS OF CUT AND FILL ARE BASED. SLIGHT DISCREPANCIES BETWEEN THE

16. FIELD CONDITIONS AND PROJECT VARIABILITY MAY REQUIRE ADAPTATION OF THE

DEPENDING ON SITE CONDITIONS OR PROJECT NEEDS. MINOR VARIATION(S) OR

17. PRIOR TO CLEARING AND GRUBBING, THE CONTRACTOR SHALL MARK THE LIMITS OF
CLEARING NEAR TREES FOR VERIFICATION OF INTENT BY THE ENGINEER. SOME MINOR
ADJUSTMENT OF CHANNEL ALIGNMENT MAY BE REQUIRED TO PRESERVE TREES OR

18. THE CONTRACTOR SHALL STAKE OUT THE PROPOSED STREAM CENTERLINE USING
TRADITIONAL SURVEY METHODS OR SURVEY GRADE GPS EQUIPMENT FOR REVIEW BY
THE ENGINEER BEFORE BEGINNING EXCAVATION AND GRADING. DEPENDING ON
ENCOUNTERED CONDITIONS SOME SHIFTING OF THE STREAM ALIGNMENT MAY BE
NECESSARY. STAKING MAY BE OMITTED FOR PORTIONS OF THE STREAM WHEN
SURVEY -GRADE GPS IS USED TO CONSTRUCT THE CHANNEL.
18. WHERE PRACTICABLE, EXISTING TREES AND VEGETATION SHOULD BE LEFT IN PLACE
TO FACILITATE NATURAL REGENERATION AND SOIL STABILIZATION.
19. ANY HARVESTING OF TREES FROM ONSITE MUST BE APPROVED BY THE PROJECT
OWNER AND ENGINEER.
20. CONTRACTOR SHALL MINIMIZE, TO THE MAXIMUM EXTENT POSSIBLE, IMPACTS TO
THE ADJACENT TREES.
21. CONSTRUCTION EQUIPMENT TRACKS AND ACCESS PATHS SHALL BE GRADED AND

22. CONTRACTOR SHALL USE AN EXCAVATOR WITH A HYDRAULIC THUMB TO INSTALL

FOR INSTREAM STRUCTURES LIKE RIFFLES, CROSS -VANES AND TOE WOOD

26. STREAM RESTORATION WORK SHALL BE IMPLEMENTED BY FIRST GRADING THE
FLOODPLAIN ADJACENT TO THE CHANNEL TO THE ELEVATIONS AND GRADES
SPECIFIED IN THE PLANSHEETS. THE PROPOSED STREAM CHANNEL SHALL THEN BE
EXCAVATED TO THE CHANNEL CROSS-SECTION GEOMETRY AND LONGITUDINAL
PROFILE IN THE CONSTRUCTION DOCUMENTS. THIS CHANNEL WORK SHALL BE DONE
WITH LOW GROUND PRESSURE TRACK EQUIPMENT. PLANSHEETS PROVIDE
DIMENSIONS, ELEVATIONS AND SLOPES TO AID IN CONSTRUCTION OF THE CHANNEL.
THE THALWEG CAN FIRST BE EXCAVATED TO THE ELEVATION SPECIFIED IN THE
LONGITUDINAL PROFILE AND EXCAVATION AND FINE GRADING OF THE
CORPS -SECTIONS SHALL THEN BE PREFORMED. ANY TEMPORARY STOCKPILING OR
DOUBLE HANDLING OF EXCESS EARTH NECESSARY TO BUILD THE CHANNEL SHALL BE
CONSIDERED INCIDENTAL TO CONSTRUCTION.
27. BANKFULL CHANNEL DIMENSIONS WILL BE HELD TO THE DIMENSIONS SHOWN ON
THE TYPICAL CROSS-SECTION PLANSHEETS. ELEVATIONS SHALL BE CONSTRUCTED
WITHIN 0.1' (VERTICAL). WIDTHS AND DEPTHS MUST FALL WITHIN RANGES SHOWN IN
THE PLANSHEETS. CHANNEL CROSS-SECTION DIMENSIONS SHALL BE WITHIN 0.2'
(HORIZONTAL).
28. IF THE EXISTING GROUND IS LESS THAN 0.2' HIGHER THAN THE PROPOSED BANKFULL
ELEVATION, IT IS NOT NECESSARY TO EXCAVATE TO THE PROPOSED ELEVATIONS AND
GRADES IN THE CONSTRUCTION DOCUMENTS.
29. IN -STREAM STRUCTURES SHALL BE INSTALLED AS THE CHANNEL IS BEING
CONSTRUCTED. INSTREAM STRUCTURES SHALL BE FINISHED TO A SMOOTH SURFACE
IN ACCORDANCE WITH THE LINES, GRADES AND ELEVATIONS SHOWN IN THE
CONSTRUCTION DOCUMENTS. THE FINISHED STRUCTURE SLOPES AND PROFILE
ELEVATIONS SHALL BE WITHIN 0.1' (VERTICAL) OF THE CONSTRUCTION DOCUMENTS
30. ALL FILTER FABRIC INSTALLED AS PART OF THE INSTREAM STRUCTURE SHALL BE A
NONWOVEN GEOTEXTILE UNLESS OTHERWISE SPECIFIED IN STRUCTURE DETAILS OR
SPECIFICATIONS. FILTER FABRIC SHALL BE TRIMMED TIGHT TO THE SURFACE OF THE
STRUCTURE AND SHOULD NOT BE OBSERVED BY VISUAL INSPECTION.
31. BOULDER STRUCTURES SHALL BE CONSTRUCTED FROM BOULDERS THAT ARE CUBICAL
OR RECTANGULAR IN SHAPE AND SIZED ACCORDING TO THE STRUCTURE DETAILS.
32. AFTER THE STRUCTURE IS COMPLETE AND FLOW IS RESTORED TO THE CHANNEL,
SOME ADJUSTMENT TO THE STRUCTURE OR ADDITIONAL STABILIZATION MEASURE
MAY BE NECESSARY TO ACHIEVE THE DESIRED FUNCTION.
26. CHANNEL REALIGNMENT WORK SHALL BE COMPLETED AND STABILIZED PRIOR TO
ALLOWING FLOW TO ENTER INTO THE NEWLY CONSTRUCTED STREAM CHANNEL.
27. THE CONSTRUCTED CHANNEL SHALL BE STABILIZED AS SOON AS POSSIBLE BY
TEMPORARY AND PERMANENT SEEDING, ADDING STRAW MULCH TO BARE SOIL AND
INSTALLING EROSION CONTROL MATTING FROM THE TOE OF THE BANKFULL
CHANNEL TO 4' BEYOND THE BANKFULL STAGE. PRIOR TO INSTALLING THE EROSION
CONTROL MATTING, PREPARE THE SOIL SURFACE BY LOOSENING 2 - 4" OF SOIL OR
APPLYING 2 - 4" OF TOPSOIL TO THE PROPOSED ELEVATIONS AND APPLY SEED AND
THEN STRAW MULCH. SEED SHALL BE BROADCAST EVENLY OF THE AREA USING A
BROADCAST SPREADER PRIOR TO COVERING WITH THE EROSION CONTROL MATTING.
THE MATTING SHALL BE ROLLED OUT IN THE DIRECTION OF ANTICIPATED RUNOFF
FLOW. INSTALL MATTING IN ACCORDANCE WITH THE DETAIL INCLUDED IN THE
CONSTRUCTION DOCUMENTS. REWORKING OF AREAS THAT DO NOT ESTABLISH
VEGETATION OR BECOME UNSTABLE SHALL BE NECESSARY IN THE MATTING
SEPARATES FROM THE SOIL.
28. IF THE TIMING OF CONSTRUCTION IS SUCH THAT PERMANENT RIPARIAN SEED
CANNOT BE PLACED IN THE FALL, THEN THE CONTRACTOR SHALL SEED WITH
TEMPORARY WINTER COVER AND RETURN THE FOLLOWING SPRING TO INSTALL THE
PERMANENT RIPARIAN SEED.
29. THE HARVESTING AND INSTALLATION OF LIVE STAKES AND CUTTINGS SHALL BE
PREFORMED ONLY DURING THE DORMANT SEASON, TYPICALLY BETWEEN DECEMBER
1 AND MARCH 1. THE CONTRACTOR SHALL NOTIFY THE PROJECT OWNER AND
ENGINEER 7 DAYS PRIOR TO HARVESTING TO REVIEW AND APPROVE ALL HARVESTING
SITES AND SPECIES.
30. LIVE STAKES AND BARE ROOTS SHALL BE INSTALLED BELOW THE BANKFULL STAGE TO
THE BASE FLOW WATER SURFACE IN THE CHANNEL. LIVE STAKES SHALL BE INSTALLED
THROUGH THE EROSION CONTROL MATTING TO A DEPTH THAT REACHES THE WATER
TABLE.
31. EXCESS EARTH MAY BE USED TO BACKFILL THE OLD CHANNEL OR SPREAD IN OTHER
LOCATIONS WITHIN THE PROJECT AREA THAT ARE APPROVED BY THE PROJECT
OWNER AND ENGINEER.

QUANTITIES AND MATERIALS SPECIFICATIONS
1. THE CONTRACTOR SHALL FURNISH ALL MATERIALS NECESSARY TO COMPLETE THE
PROPOSED WORK UNLESS OTHER PROVISIONS HAVE BEEN AGREED UPON PRIOR TO
CONSTRUCTION. THE CONTRACTOR SHALL DELIVER ALL MATERIALS TO THE
DESIGNATED ACCESS POINTS AND STAGING AREAS OR TO A LOCATION SPECIFIED BY
THE PROJECT OWNER OR ENGINEER. MATERIAL QUANTITIES, DIMENSIONS AND SIZES
SHALL CONFORM TO THE NOTES AND SPECIFICATIONS PROVIDED IN THE
CONSTRUCTION DOCUMENTS OR ON THE QUANTITIES AND MATERIALS LIST. THE
ENGINEER MAY INSPECT AND APPROVE ALL MATERIALS PRIOR TO CONSTRUCTION. IF
MATERIALS DO NOT MEET THE MINIMUM REQUIREMENTS SPECIFIED IN THE
CONSTRUCTION DOCUMENTS, THE ENGINEER SHALL REJECT THE MATERIALS.
2. COSTS INCURRED DUE TO PROJECT DELAYS RESULTING FROM FAILURE OF THE
CONTRACTOR TO MEET THE REQUIREMENTS OF THE GENERAL SPECIFICATIONS,
CONSTRUCTION DOCUMENTS, OR CONSTRUCTION SPECIFICATIONS, SHALL BE THE
EXPENSE OF THE CONTRACTOR. QUANTITIES LISTED ARE ESTIMATES ONLY AND
SHALL BE CONFIRMED BY THE CONTRACTOR.
3. THE EROSION CONTROL MEASURES DEPICTED ON THE PLANS ARE TO BE INSTALLED
AS NEEDED TO KEEP ALL SEDIMENT ON SITE AND OUT OF STREAMS AND WETLANDS.
ADDITIONAL EROSION CONTROL MEASURES (ABOVE THOSE SHOWN ON THE PLANS
AND ON THE QUANTITIES AND MATERIALS LIST) MAY BE REQUIRED IN ORDER TO KEEP
ALL SEDIMENT ON SITE AND OUT OF STREAMS AND WETLANDS. THE CONTRACTOR
SHALL OBTAIN APPROVAL FROM THE ENGINEER AND NC DEQ (WHEN APPLICABLE)
PRIOR TO INSTALLATION OF ADDITIONAL EROSION CONTROL MEASURES.
4. ANY ADDITIONAL GRADING OTHER THAN WHAT IS SHOWN ON THE PLANS SHALL
REQUIRE PRIOR APPROVAL FROM THE ENGINEER. ANY ADDITIONAL AREAS OF
DISTURBANCE (CHANGES IN ACCESS, GRADING, CLEARING, ETC.) WILL REQUIRE
APPROVAL FROM THE ENGINEER AND NCDEQ (WHEN APPLICABLE).
5. THE USE OF ANY BRAND NAMES/MANUFACTURERS OR MODELS IS INTENDED SOLELY
TO DENOTE THE QUALITY STANDARD OF THE DESIRED PRODUCT. ANY USE OF BRAND
NAMES IS NOT INTENDED TO RESTRICT BIDDERS TO A SPECIFIC BRAND, MAKE,
MANUFACTURER, OR NAME. THE BRAND NAMES. MANUFACTURERS OF MODELS ARE
INTENDED TO CONVEY THE GENERAL STYLE, TYPE, CHARACTER, AND QUALITY OF
PRODUCT. EQUIVALENT PRODUCTS WILL BE ACCEPTABLE IF THE ENGINEER HAS GIVEN
APPROVAL OF THE SPECIFIC PRODUCT IN WRITING.
6. THE CONTRACTOR SHALL BE RESPONSIBLE FOR LOCATING AND PROVIDING STORAGE
AREAS FOR CONSTRUCTION MATERIALS AND EQUIPMENT. THE MATERIAL AND
EQUIPMENT STORAGE SHALL COMPLY WITH THE CONSTRUCTION DOCUMENTS AND
ALL TOWN, LOCAL, STATE AND FEDERAL REGULATIONS THROUGHOUT THE
CONSTRUCTION PERIOD. THE CONTRACTOR SHALL RESTORE THE STORAGE AREA TO
ITS ORIGINAL (OR BETTER) CONDITION UPON COMPLETION OF THE PROJECT OR
UPON SUCH TIME AS DIRECTED BY THE PROJECT OWNER. SUCH RESTORATION SHALL
BE AT NO ADDITIONAL COST TO THE PROJECT OWNER. THE CONTRACTOR SHALL BE
RESPONSIBLE FOR THE SAFEGUARDING OF MATERIALS AND EQUIPMENT AGAINST
FIRE, THEFT AND VANDALISM AND SHALL NOT HOLD THE PROJECT RESPONSIBLE IN
ANY WAY FOR THE OCCURRENCES OF SAME. FOR MATERIALS AND EQUIPMENT
STORAGE AREAS THE CONTRACTOR SHALL FURNISH AND ERECT, AT NO ADDITIONAL
COST, WHATEVER WORKS MAY BE NECESSARY FOR THE PROTECTION OF THE PUBLIC,
INCLUDING BUT NOT LIMITED TO BARRICADES, FENCES, ETC. PRIOR TO FINAL
PAYMENT BEING MADE, THE CONTRACTOR SHALL OBTAIN A RELEASE FROM THE
PROPERTY OWNER OF THE STORAGE AREA UTILIZED FOR THE PROJECT.
7. THE CONTRACTOR SHALL ESTABLISH, PROVIDE AND MAINTAIN AN EFFECTIVE
QUALITY CONTROL PROGRAM THAT DETAILS THE METHODS AND PROCEDURES TO BE
TAKEN TO ASSURE THAT ALL MATERIALS AND COMPLETED WORK, REQUIRED FOR THE
PROJECT, CONFORM TO THE CONSTRUCTION DOCUMENTS. ALTHOUGH GUIDELINES
ARE ESTABLISHED AND CERTAIN MINIMUM REQUIREMENTS ARE SPECIFIED HEREIN
AND ELSEWHERE IN THE CONSTRUCTION DOCUMENTS, THE CONTRACTOR IS FULLY
RESPONSIBLE FOR DEVELOPING AND EXECUTING THEIR OWN QUALITY CONTROL
PROGRAM. THE CONTRACTOR SHALL PERFORM INSPECTION, TESTING, AND
MEASUREMENT OF ALL ITEMS OF WORK REQUIRED BY THE CONSTRUCTION
DOCUMENTS AND TECHNICAL SPECIFICATIONS, INCLUDING THOSE PERFORMED BY
SUBCONTRACTORS. REVIEW OF THE MATERIALS AND COMPLETED WORK BY THE
PROJECT OWNER AND ENGINEER DOES NOT RELIEVE THE CONTRACTOR OF

8. THE CONTRACTOR SHALL WARRANTY ALL MATERIALS AND WORKMANSHIP FOR A
PERIOD OF ONE (1) YEAR FROM THE DATE OF ACCEPTANCE OF BY THE PROJECT
OWNER AND SHALL REPLACE ANY PORTIONS THAT FAIL DUE TO FAULTY MATERIALS
OR WORKMANSHIP, AT NO ADDITIONAL COST TO THE PROJECT OWNER. A SIX (6)
MONTH AND ELEVEN (11) MONTH INSPECTION WILL BE PERFORMED DURING THE
WARRANTY PERIOD. THE CONTRACTOR SHALL IMMEDIATELY REPAIR ALL ITEMS
DETERMINED BY THE PROJECT OWNER OR AUTHORIZED REPRESENTATIVE TO BE
DEFECTIVE UPON NOTIFICATION. THE CONTRACTOR SHALL IMMEDIATELY REPAIR OR
REPLACE FAILED ITEMS UPON NOTIFICATION BY THE PROJECT OWNER. SEASONALLY
INSTALLED ITEMS SHALL BE REPAIRED OR REPLACED DURING THE NEXT AVAILABLE
INSTALLATION PERIOD. ITEMS REPAIRED OR REPLACED UNDER THIS PROVISION SHALL
HAVE AN ADDITIONAL ONE (1) YEAR WARRANTY PERIOD FROM THE NEW DATE OF
ACCEPTANCE. AREAS AND/OR OTHER WORK DISTURBED WHILE ACCESSING AND/OR
REPAIRING/REPLACING WARRANTY COVERED ITEMS SHALL BE STABILIZED.

GROUND STABILIZATION AND MATERIALS HANDLING PRACTICES FOR COMPLIANCE WITH
THE NCG01 CONSTRUCTION GENERAL PERMIT
Implementing the details and specifications on this plan sheet will result in the construction
activity being considered compliant with the Ground Stabilization and Materials Handling
sections of the NCG01 Construction General Permit (Sections E and F, respectively). The
permittee shall comply with the Erosion and Sediment Control plan approved by the
delegated authority having jurisdiction. All details and specifications shown on this sheet
may not apply depending on site conditions and the delegated authority having jurisdiction.

Note: After the permanent cessation of construction activities, any areas with temporary
ground stabilization shall be converted to permanent ground stabilization as soon as
practicable but in no case longer than 90 calendar days after the last land disturbing
activity. Temporary ground stabilization shall be maintained in a manner to render the
surface stable against accelerated erosion until permanent ground stabilization is achieved.
GROUND STABILIZATION SPECIFICATION
Stabilize the ground sufficiently so that rain will not dislodge the soil. Use one of the
techniques in the table below:

POLYACRYLAMIDES (PAMS) AND FLOCCULANTS
1. Select flocculants that are appropriate for the soils being exposed during
construction, selecting from the NC DWR List of Approved PAMS/Flocculants.
2. Apply flocculants at or before the inlets to Erosion and Sediment Control Measures.
3. Apply flocculants at the concentrations specified in the NC DWR List of Approved
PAMS/Flocculants and in accordance with the manufacturer's instructions.
4. Provide ponding area for containment of treated Stormwater before discharging
offsite.
5. Store flocculants in leak -proof containers that are kept under storm -resistant cover
or surrounded by secondary containment structures.
EQUIPMENT AND VEHICLE MAINTENANCE
1. Maintain vehicles and equipment to prevent discharge of fluids.
2. Provide drip pans under any stored equipment.
3. Identify leaks and repair as soon as feasible, or remove leaking equipment from the
project.
4. Collect all spent fluids, store in separate containers and properly dispose as
hazardous waste (recycle when possible).
5. Remove leaking vehicles and construction equipment from service until the problem
has been corrected.
6. Bring used fuels, lubricants, coolants, hydraulic fluids and other petroleum products
to a recycling or disposal center that handles these materials.
LITTER, BUILDING MATERIAL AND LAND CLEARING WASTE
1. Never bury or burn waste. Place litter and debris in approved waste containers.
2. Provide a sufficient number and size of waste containers (e.g dumpster, trash
receptacle) on site to contain construction and domestic wastes.
3. Locate waste containers at least 50 feet away from storm drain inlets and surface
waters unless no other alternatives are reasonably available.
4. Locate waste containers on areas that do not receive substantial amounts of runoff
from upland areas and does not drain directly to a storm drain, stream or wetland.
5. Cover waste containers at the end of each workday and before storm events or
provide secondary containment. Repair or replace damaged waste containers.
6. Anchor all lightweight items in waste containers during times of high winds.
7. Empty waste containers as needed to prevent overflow. Clean up immediately if
containers overflow.
8. Dispose waste off -site at an approved disposal facility.
9. On business days, clean up and dispose of waste in designated waste containers.
PAINT AND OTHER LIQUID WASTE
1. Do not dump paint and other liquid waste into storm drains, streams or wetlands.
2. Locate paint washouts at least 50 feet away from storm drain inlets and surface
waters unless no other alternatives are reasonably available.
3. Contain liquid wastes in a controlled area.
4. Containment must be labeled, sized and placed appropriately for the needs of site.
5. Prevent the discharge of soaps, solvents, detergents and other liquid wastes from
construction sites.
PORTABLE TOILETS
1. Install portable toilets on level ground, at least 50 feet away from storm drains,
streams or wetlands unless there is no alternative reasonably available. If 50 foot
offset is not attainable, provide relocation of portable toilet behind silt fence or place
on a gravel pad and surround with sand bags.
2. Provide staking or anchoring of portable toilets during periods of high winds or in high
foot traffic areas.
3. Monitor portable toilets for leaking and properly dispose of any leaked material.
Utilize a licensed sanitary waste hauler to remove leaking portable toilets and replace
with properly operating unit.
EARTHEN STOCKPILE MANAGEMENT
1. Show stockpile locations on plans. Locate earthen -material stockpile areas at least
50 feet away from storm drain inlets, sediment basins, perimeter sediment controls
and surface waters unless it can be shown no other alternatives are reasonably
available.
2. Protect stockpile with silt fence installed along toe of slope with a minimum offset of
five feet from the toe of stockpile.
3. Provide stable stone access point when feasible.
4. Stabilize stockpile within the timeframes provided on this sheet and in accordance
with the approved plan and any additional requirements. Soil stabilization is defined
as vegetative, physical or chemical coverage techniques that will restrain accelerated
erosion on disturbed soils for temporary or permanent control needs.

1. Do not discharge concrete or cement slurry from the site.
2. Dispose of, or recycle settled, hardened concrete residue in accordance with local
and state solid waste regulations and at an approved facility.
3. Manage washout from mortar mixers in accordance with the above item and in
addition place the mixer and associated materials on impervious barrier and within
lot perimeter silt fence.
4. Install temporary concrete washouts per local requirements, where applicable. If an
alternate method or product is to be used, contact your approval authority for
review and approval. If local standard details are not available, use one of the two
types of temporary concrete washouts provided on this detail.
5. Do not use concrete washouts for dewatering or storing defective curb or sidewalk
sections. Stormwater accumulated within the washout may not be pumped into or
discharged to the storm drain system or receiving surface waters. Liquid waste must
be pumped out and removed from project.
6. Locate washouts at least 50 feet from storm drain inlets and surface waters unless it
can be shown that no other alternatives are reasonably available. At a minimum,
install protection of storm drain inlet(s) closest to the washout which could receive
spills or overflow.
7. Locate washouts in an easily accessible area, on level ground and install a stone
entrance pad in front of the washout. Additional controls may be required by the
approving authority.
8. Install at least one sign directing concrete trucks to the washout within the project
limits. Post signage on the washout itself to identify this location.
9. Remove leavings from the washout when at approximately 75% capacity to limit
overflow events. Replace the tarp, sand bags or other temporary structural
components when no longer functional. When utilizing alternative or proprietary
products, follow manufacturer's instructions.
10. At the completion of the concrete work, remove remaining leavings and dispose of
in an approved disposal facility. Fill pit, if applicable, and stabilize any disturbance
caused by removal of washout.
HERBICIDES, PESTICIDES AND RODENTICIDES
1. Store and apply herbicides, pesticides and rodenticides in accordance with label
restrictions.
2. Store herbicides, pesticides and rodenticides in their original containers with the
label, which lists directions for use, ingredients and first aid steps in case of
accidental poisoning.
3. Do not store herbicides, pesticides and rodenticides in areas where flooding is
possible or where they may spill or leak into wells, stormwater drains, ground water
or surface water. If a spill occurs, clean area immediately.
4. Do not stockpile these materials onsite.

SECTION A: SELF -INSPECTION
Self -inspections are required during normal business hours in accordance with the table
below. When adverse weather or site conditions would cause the safety of the inspection
personnel to be in jeopardy, the inspection may be delayed until the next business day on
which it is safe to perform the inspection. In addition, when a storm event of equal to or
greater than 1.0 inch occurs outside of normal business hours, the self -inspection shall be
performed upon the commencement of the next business day. Anytime when inspections
were delayed shall be noted in the Inspection Record.

the required observations on the Inspection Record Form provided by the Division or
a similar inspection form that includes all the required elements. Use of
electronically -available records in lieu of the required paper copies will be allowed if
shown to provide equal access and utility as the hard -copy records.
(c) All data used to complete the Notice of Intent and older inspection records shall be
maintained for a period of three years after project completion and made available
upon request. (40 CFR 122.41]

SECTION C: REPORTING
1. Occurrences that must be reported
Permittees shall report the following occurrences:
(a) Visible sediment deposition in a stream or wetland.
(b) Oil spills if:
• They are 25 gallons or more,
• They are less than 25 gallons but cannot be cleaned up within 24 hours,
• They cause sheen on surface waters (regardless of volume), or
• They are within 100 feet of surface waters (regardless of volume).
(a) Releases of hazardous substances in excess of reportable quantities under Section 311
of the Clean Water Act (Ref: 40 CFR 110.3 and 40 CFR 117.3) or Section 102 of CERCLA
(Ref: 40 CFR 302.4) or G.S. 143-215.85.

(c) Noncompliance with the conditions of this permit that may endanger health or the
environment.
2. Reporting Timeframes and Other Requirements
After a permittee becomes aware of an occurrence that must be reported, he shall contact
the appropriate Division regional office within the timeframes and in accordance with the
other requirements listed below. Occurrences outside normal business hours may also be
reported to the Division's Emergency Response personnel at (800) 662-7956, (800)
858-0368 or (919) 733-3300.

SEED SHALL BE SOWN WITH A SPREADER OR A SEEDING MACHINE. SEED IS NOT TO BE
BROADCAST OR DROPPED WHEN WIND VELOCITY EXCEEDS 5 MPH. SEED SHALL BE
EVENLY DISTRIBUTED BY SOWING IN TWO DIRECTIONS AT RIGHT ANGLES TO EACH
OTHER. WET SEED OR SEED THAT IS MOLDY OR OTHERWISE DAMAGED IN TRANSIT OR

BARE ROOTS
11. ALLOW BARE ROOTS TO SOAK IN WATER AN HOUR OR TWO BEFORE PLANTING. DO
NOT SOAK THE ROOTS FOR MORE THAN 24 HOURS.
12. DIG A PLANTING PIT THAT IS THREE (3) TIMES WIDER THAN THE TREE'S CURRENT ROOT
SYSTEM. SPREAD OUT ROOTS TO ENCOURAGE OUTWARD GROWTH.
13. KEEP THE TREE VERTICAL IN THE PLANTING PIT (PERPENDICULAR TO THE GROUND) SO
THAT IT GROWS STRAIGHT.
14. REFILL THE PIT WITH NATIVE SOIL (WHAT WAS REMOVED AT DIGGING TIME), AND ANY

LIVE STAKES
16. LIVE STAKES MUST BE DORMANT WHEN CUT. KEEP LIVE STAKES MOIST UNTIL
PLANTING. THE STAKE SHOULD BE PREPARED WITH THE BUDS POINTED UP, AND THE
BOTTOM SHOULD BE CUT AT AN ANGLE FOR EASY INSERTION INTO THE GROUND.
17. LIVE STAKES SHOULD BE PLACED WITH 3 TO 3 OF THE LENGTH OF THE STAKE BELOW
GROUND AND ANGLED DOWNSTREAM. ENSURE THE BASE OF THE LIVE STAKE WILL
REACH THE WATER TABLE.
18. AN IRON BAR CAN BE USED TO MAKE A PILOT HOLE TO PREVENT BARK FROM BEING
