4h. Explain the purpose of the proposed project,*
The purpose of the project is to provide adequate drainage to prevent future flooding of nearby residential properties.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The project involves removal of the existing 36 inch RCP pipes and installing a new, free span bridge along the ebsting road alignment. The project will require 0.04 acres of permanent
open water impacts due to grading in open water ditches and Thaggard's Lake, 20 linear feet of permanent stream impacts due to excavation in McLean Branch, 0.06 acres of
temporary open water impacts for construction access, and 20 linear feet of temporary stream impacts due to construction access (see attached Impacts Map). Please note that areas
will be excavated only to the elevations shown on the plans and no riprap is proposed that would require additional excavation. The proposed grading should maintain natural flow
conditions. Areas around the perimeter of the project will be temporarily impacted for construction access. Temporarily impacted stream and open water bank slopes will be restored,
covered with biodegradable coir matting, and seeded with a native seed mix. Temporary construction impacts might also include dust, noise, road closure, and soil erosion. A temporary
pump around diversion will be installed so work will be performed in the dry to minimize soil erosion into adjacent waters. No operational impacts are anticipated once the project is
complete. Equipment needed to perform the work might include excavators, backhoes, dump trucks, asphalt pavers, rollers, soil compactors, and skid steer loaders.
4j. Please upload project drawings for the proposed project.

E. Impact Justification and Mitigation
1. Avoidance and Minimization
1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
The bridge structure is proposed above the 100-year FEMA flood elevation to avoid impacts to floodplains. The bridge will be constructed within the
epsting road embankment which will minimize permanent impacts to adjacent aquatic resources.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Silt fence, turbidity curtains, and a pump -around diversion to dewater the work area will be installed to prevent sediment from washing into
jurisdictional waters.
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
r Yes r No
2b. If this project DOES NOT require Compensatory Mitigation, explain why:
There are 0.04 acres of permanent open water impacts and 25 linear feet of permanent stream impacts. These are below the threshold for mitigation required by Nationwide Permit 14.
F. Stormwater Management and Diffuse Flow Plan (required by DWR)
1. Diffuse Flow Plan
1a. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules?
r Yes r No
If no, explain why:
The project is in the Cape Fear basin which has no Riparian Buffer Protection Rules.
2. Stormwater Management Plan
2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250?*
r Yes r No
2b. Does this project meet the requirements for low density projects as defined in 15A NCAC 02H .1003(2)?
