Division of Water Resources' Central and Regional staff has reviewed the application package
received August 22, 2019. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on December
20, 2019.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items,
or failure to provide the additional information on or before the above requested date may result in your

A. Application:
1. The application forms have the applicant as Cervini Farms, Inc. but the corporation registered is Cervini
Farms North Carolina, Inc. Clarify.
2. The property of the proposed facility is owned by Cervini Farm (1993) Inc. Provide documentation
verifying that the applicant owns or has rights to the property.
3. For the generation application, Section VII needs to correlate with what is shown on the plans and
calculations. The table lists anoxic basins and post flow equalization tank that are not identified on the
plans. A grinder pump is referenced in the calculations but not shown elsewhere. Provide clarification so
that all units are in the application form, engineering plans, and calculations.
4. On Sheet N-4.3 show the floodplain.
5. Per 15A NCAC 02U .0402(n), a minimum of 30 days of residual storage is required. Address.
6. Address overflow prevention in the proposed greenhouse irrigation setup.
7. Address if a sediment and erosion control permit is required. If so, provide the status.
8. Per 15A NCAC 02U .0801(a), an operational and maintenance plan is required. A draft plan can be
submitted with final submitted prior to operation.
9. The engineering plans shall represent a completed design and not be labeled with preliminary phrases that
indicate they are not anything other than final specifications. However, the plans may be labeled with the
phrase "Final design — not released for construction".
10. For the generation application, Section VIII. Design Information for Earthen Storage Impoundments
needs to be completed for the 5-day upset pond.
11. For the generation application, the septic tanks and equalization tank need to be listed in Section VII.
Design Information (table a).
12. Address if the calcite filter pod meets the duality requirement per 15A NCAC 02U .0402(c).
13. The effluent sampling and turbidity readings needs to occur prior to mixing with recycle wastewater.
B. Calculations (15A NCAC 02U .0401 & .0402):
1. The pollutant loading calculations must include all parameters required for Type 2 reclaimed water per
15A NCAC 02U .030I(a).
2. Clarify the water balance to demonstrate that all wastewater effluent generated can be utilized and that the
irrigation tank is sufficiently sized to hold enough wastewater (per the allowable irrigation rate).
3. Provide calculations verifying the backup generator is sufficient to power the proposed treatment and
disposal system.

C. 5-Day Upset Pond
1. Address if the bottom of the structure will intersect the seasonal high water table. If so, the
applicant must provide a plan to prevent the synthetic liner from floating.
2. Installation of the liner and pond construction should be addressed in the specifications.
3. Address how the pond will handle flood events. Will flooding affect the stability of the
embankment?
