As a part of this permit, Hexion is required to submit semiannual Discharge Monitoring Reports (DMRs). Please find enclosed the completed DMR Form (Attachment 1) and the analytical testing report (Attachment 2) for Period 2 (April to September 2018). Also included is an additional copy of the form, as required by Permit NCS000332 for the monitoring period April - September 2018.. The results from the Period 2 sampling event indicate that Formaldehyde levels were above the benchmark value. As such, facility personnel followed the Tier one steps outlined in Section B of Permit No. NCS000332. Documentation of the stormwater management inspection and subsequent actions has been recorded and is included in the facility's Stormwater Pollution Prevention Plan (SWPI'P), which is maintained on - site. The facility stormwater outfall was sampled on February 10, 2014 and laboratory results were received February 24, 2014. The facility failed to sample during the previous sampling window due to an error by the person responsible for stormwater compliance at the site. The individual who was responsible for the stormwater monitoring at the site is no longer with the Company, and corrective actions have been put in place to ensure all sampling and recordkeeping requirements are met going forward. In addition there was a release of material from the building due to an overflow of the water recovery tank on December 121h. Most of the overflow material was contained in the building but 75 gallons escaped the building. From observations that night and the next morning some material did get by our spill response materials and get to soil. We estimate based on an analysis of the water in the water recovery tank that approximately 0.25 Ibs of Formaldehyde escaped the building. Much of this was captured in absorbant materials but some did make it to soil. Corrective and Preventative actions have been initiated and all action items are expected to be completed by May. The COD analytical result from February exceeded the benchmark value established in the permit. Tier One activities were completed. A stormwater management inspection of the site was conducted on February 26`h. No obvious signs of contamination were present. Employees of a nearby company have been observed to park near the ditch that leads to the stormwater Mr. Muller —Page 2 March 25, 2011 outfall and smoke. We.have informed the adjacent company and the problem has stopped. The release from December may have affected the sample but the Formaldehyde concentration was below the limit of detection which leads us to think that that was not the cause. Momentive will be sampling an upstream location to begin determining the source of the COD exceedence. Containment releases from chemical storage areas following rain events are being reviewed. All the following action items will be completed in the following two months and will be documented in the Stormwater Pollution Prevention Plan.
