Qualitative monitoring requirements vary, Most permits require qualitative monitoring to be
performed during a "representative storm event" or during a "measureable storm event." However,
some permits do not have this requirement. Please refer to these definitions, if applicable.
A "representative storm event" is a storm event that measures greater than 0.1 inches of rainfall
and that is preceded by at least 72 hours (3 days) in which no storm event measuring greater than
0.1 inches has occurred. A single storm event may contain up to 10 consecutive hours of no
precipitation.
A "measurable storm event" is a storm event that results in an actual discharge from the
permitted site outfall. The previous measurable storm event must have been at least 72 hours
prior. The 72-hour storm interval does not apply if the permittee is able to document that a shorter
interval is representative for local storm events during the sampling period, and the permittee
obtains approval from the local DWQ Regional Office.

Lenoir Mirror Company (LMC) personnel performed a stormwater monitoring event for their facility at 401
Kincaid Street in Lenoir, Caldwell County, North Carolina. The stormwater sampling event included the
collection of a stormwater sample and outfall observations for qualitative parameters during a
representative and measurable storm event. The monitoring was conducted in general conformance with
the requirements of the State of North Carolina Department of Environmental Quality (NCDEQ), National
Pollutant Discharge Elimination System (NPDES), LMC stormwater permit (SWP) NCS000061.
On April 12, 2016, a stormwater sample was collected at the designated discharge point, Stormwater
Discharge Outfall (SDO#1), based on a storm event that meets the requirements for monitoring. The
collected sample was submitted to North Carolina -accredited Blue Ridge Labs in Lenoir, North Carolina,
for analysis of total suspended solids (TSS) by EPA Method SM2540D-11, biochemical oxygen demand
(BOD) by EPA Method 405.1, chemical oxygen demand (COD) by Standard Method 5220D, total silver
and lead by EPA Method 200.7. A total rainfall amount reading was collected from a nearby gauging
location. Results of visual observations, qualitative parameters, and the total rainfall reading are provided
in the attached SDO Qualitative Monitoring Report and SDO Monitoring Report.
Laboratory results and observations of qualitative parameters indicated that the specified monitoring
parameters were within discharge benchmark values as outlined in the NPDES SWP NCS000061. Field
pH was not measured during the monitoring. LMC will assure that pH data are collected during future
monitoring events.
Please note that the SDO Qualitative Monitoring Report and SDO Monitoring Report requires certification
by the signature of permittee (Lenoir Mirror employee) before submittal of the entire report to the address

Lenoir Mirror Company (LMC) personnel performed a stormwater monitoring event for their facility at 401
Kincaid Street in Lenoir, North Carolina. The stormwater sampling event included the collection of a
stormwater sample and outfall observations for qualitative parameters during a representative and
measurable storm event. The monitoring was conducted in general conformance with the requirements of the
State of North Carolina Department of Environmental Quality (NCDEQ), National Pollutant Discharge
Elimination System (NPDES), LMC stormwater permit (SWP) NCS000061.
On September 25, 2015, a stormwater sample was collected at the designated discharge point, Stormwater
Discharge Outfall (SDO#1), based on a storm event that meets the requirements for monitoring. The collected
sample was submitted to North Carolina -accredited Water Tech Labs Inc. in Granite Falls, North Carolina, for
analysis of total suspended solids (TSS) by EPA Method SM2540D-11, biochemical oxygen demand (BOD)
by EPA Method 405.1, chemical oxygen demand (COD) by Standard Method 5220D, total silver and lead by
EPA Method 200.7, and pH, which was field analyzed. A total rainfall amount reading was collected from a
nearby gauging location with reasonably obtainable data. Results of visual observations, qualitative
parameters, and the total rainfall reading are provided in the attached SDO and SDO Monitoring Report.
Laboratory results, field testing of pH, and observations of qualitative parameters indicated that the specified
monitoring parameters were within discharge benchmark values as outlined in the NPDES SWP NCS000061.
The result of the BOD, TSS, COD lead and silver were below the laboratory quantitative reporting limit, which
were below the discharge benchmarks. The field pH was 7.5, which was within the range of the discharge
benchmark values.

'Phis report documents stormwater sampling at the Lenoir Mirror Company (LMC) facility in Lenoir
North Carolina. The sampling was performed in accordance with requirements of the LMC
stormwater permit NCS000061 (SWP) issued by the North Carolina Department of Environment and
Natural Resources (NCDENR). This report is for the sample obtained during Year 5 — Period 2
(Sample Number 10) as listed in Table 2 — Monitoring Schedule in Section B of the SWP.
The scope of work included sampling the Stormwater Discharge Outfall (SDO) during a rainfall
event totaling 0.34 inches of precipitation on April 29, 2014. The sample was obtained during the
first '/2 hour after runoff began; the total precipitation at the time of sampling was about 0.1 inches.
The SDO was sampled by inserting polyethylene tubing into the flow at the SDO manhole and
extracting a sample through the tubing with a peristaltic pump. The samples were collected into 250-
milliliter HDPE jars with laboratory -added preservative as appropriate, placed on ice and taken
directly to Blue Ridge Laboratories Inc. in Lenoir, North Carolina for analysis of total silver and lead
by EPA Method 200.7, biochemical oxygen demand (BOD) by EPA Method 405.1, chemical oxygen
demand (COD) by Standard Method 5220D, total suspended solids by Standard Method 2540D, and
pH by Standard Method 4500. The laboratory reports and chain -of -custody record are attached.

At the time of sampling, stormwater was flowing through the SDO at a depth of about 6 inches. The
flow was observed for qualitative indicators of stormwater pollution as listed I Section C of the
SWP, including color, odor, clarity, foam, oil sheen etc. Based on observations during the sampling,
no obvious qualitative indications of stormwater pollution were noted.

Re: Information about MQL of BOD sample.
Most of the BOD samples we get in from BRL have a higher concentration of BOD than the
sample in question. We normally set up the samples at 1% and 3% dilutions. This gives us a
fairly broad range to test the samples.
However, the sample in question had less concentration of BOD than we expected. With no other
information about the sample, we followed our standard procedure for setting up samples. The
sample was set at 1 % and 3% dilution. The MQL for those dilutions is 66.7mg/L. When the
sample came back with too little D.O. usage for a solid result, we could only report out
<66.7mg/L as a halt.

Dear Mr. DeBeve:
This report documents stormwater sampling at the Lenoir Mirror Company (LMC) facility in Lenoir
North Carolina. The sampling was performed in general accordance with requirements of the LMC
stormwater permit NCS000061 (SWP) issued by the North Carolina Department of Environment and
Natural Resources (NCDENR). The permit includes a sampling schedule listed in Table 2 --
Monitoring Schedule (Section B of the SWP). This permit is currently in renewal and the last
sample listed under the SWP schedule was obtained in the spring of 2014. This current sample was
obtained as a continuation of the scheduling sample pending LMC's receipt of the renewed permit.
The scope of' work included sampling the Stormwater Discharge Outfail (SDO) during a rainfall
event on November 23, 2014. The sample was obtained during the first '/z hour after runoff began.
The rainfall gauge at the site was damaged and therefore rainfall amounts could not be determined;
however online weather records indicate that the storm produced over 1 inch of total rainfall. The
SDO was sampled by dipping water from the flow in the manhole using a plastic container. The
samples were collected into 250-milliliter HDPE jars with laboratory -added preservative as
appropriate, placed on ice and taken directly to Blue Ridge Laboratories Inc. in Lenoir, North
Carolina for analysis of total silver and lead by EPA Method 200.7, biochemical oxygen demand
(130D) by EPA Method 405.1, chemical oxygen demand (COD) by Standard Method 5220D, total
suspended solids by Standard Method 2540D, and pH by Standard Method 4500. The laboratory
reports and chain -of -custody record are attached.

Dear Mr. DeBeve:
Unifour Engineering and Testing Laboratories, (Unifour) is pleased to submit this report of stormwater
system sampling at the Lenoir Mirror Company (LMC) facility in Lenoir North Carolina. The
sampling was performed to provide data requested by the North Carolina Department of Environment
and Natural Resources (NCDENR) as part of their Stormwater permit renewal evaluation for LMC.
Our scope of work included sampling the Stormwater Discharge Outfall (SDO) at base flow conditions
on June 2, 2009, and re -sampling the SDO and the receiving stream upstream and downstream from the
SDO on July 27, 2009. The SDO was sampled by inserting polyethylene tubing into the flow at the
SDO manhole and extracting a sample through the tubing with a peristaltic pump. The upstream and
downstream surface water samples were obtained by submersing a cleaned 3-liter beaker into the
stream, recovering the beaker, and pouring the collected water directly into the sample containers. The
samples were collected into 250-milliliter HDPE jars with laboratory -added nitric acid preservative,
placed on ice and taken directly to Blue Ridge Laboratories Inc. in Lenoir for analysis of total silver
and lead by EPA Method 200.7. The laboratory reports and chain -of -custody records are attached.
The initial base flow sample obtained on June 2, 2009 was reported to have total lead at a
concentration of 0.081 milligrams per liter (mg/1) and total silver at a concentration of 0.001 mg/. The
three samples obtained on July 27, 2009 did not contain lead or silver above the laboratory minimum
quantification limit (0.01 mg/l for lead and 0.001 mg/l for silver).
