Aana Taylor -Smith of the Winston-Salem Regional Office of the NC Division of Energy, Mineral, and Land
Resources (DEMLR) conducted a compliance evaluation inspection at the subject facility on 24 July 2014.
Your assistance and cooperation, as well as that of Mr..Alan Cervantes, were greatly appreciated. An
inspection checklist is attached for your records and the inspection findings are summarized below.
This facility is located at 1491 Brookford Road in Kernersville, Forsyth County, North Carolina.
Stormwater related to industrial activity is discharged from the site to an unnamed tributary to the West
Fork of the Deep River, which is currently classified as Class WS-IV waters, in the Cape Fear River Basin.
Documentation & Monitoring Review
The facility's Stormwater Pollution Prevention Plan (SPPP) is very thorough, up to date, and fully
implemented. Employee training is completed once each month and recorded in the SPPP. All
documents were well organized and available for inspection.
Qualitative and analytical monitoring have been completed as required. No discrepancies were noted
and sample results were well below benchmark values. No vehicle maintenance is performed on site.
The facility has one Stormwater Discharge Outfall (SDO) that discharges stormwater related to industrial
activity.
Site Review
The site is orderly and well maintained. Secondary containment is implemented as required. The outfall
was inspected and found to be in excellent condition. Best Management Practices (BMPs) at the outfall
include rip -rap, check dams, and vegetation to reduce potential contaminant loss from the site. No
evidence of erosion or instability at the SDO was observed. The receiving stream was free of pollution
indicators.

discharge stormwater associated with industrial activities performed at your facility. Please be
advised that the Winston-Salem Regional Office will be performing NPDES Stormwater
Inspections sometime in the near future.
If this office has not previously inspected your facility, you should be prepared to demonstrate
compliance with all terms and conditions included in the Permit. Specifically, we will be
evaluating the following: Stormwater Pollution Prevention Plan, Stormwater Outfall Locations,
Qualitative and Analytical Monitoring Data and any other activities required by your permit.

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCG140000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements,
and addresses frequently asked questions
* A Certificate of Coverage
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

Dear Permittee:
Your facility is currently covered for stormwater discharge under General Permit NCG140000. This permit will
expire on July 31, 1999. North Carolina General Statute 143.215.1(c) requires that an application for permit
coverage renewal be filed in a timely manner. A prior notice of renewal was sent for this site on January 12, 1999.
To date, no response has been received by the Division of Water Quality (DWQ). In order to assure your
continued coverage under the general permit, you must apply to DWQ for renewal of your permit coverage.
Enclosed you will find a general permit renewal application form. This form serves two purposes. First, it is an
application for renewal of your permit coverage. Second, it serves as an invoice for the $80.00 permit fee. The
application must be completed and returned with the required fee by April 5, 1999 in order to assure continued
coverage under the general permit_
Recent legislation modified the fee structure for DWQ permits. Your general permit fee has been changed to an
annual fee of $80.00, A copy of the new fee schedule is enclosed in this package.
Failure to request renewal by April 5, 1999 will result in a civil assessment of at least $250.00. Larger penalties
may be assessed depending on the delinquency of the request. Discharge of stormwater from your facility without
coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could
result in assessments of civil penalties of up to $10,000 per day.
