This plant had regraded yard which likely contributed to slightly elevated TSS. An aggregate berm was installed around yard drain to reduce potential solids.

July 27, 2017—A discharge sample was collected during flash flooding at OF1. The plant had not been in operation, discharge was in process when sampler arrived; there had been
significant rainfall earlier in the day. Unknown if a discharge had occurred at that time. Sampler took a sample in spite of the unusual high flow volume at the time as directed.
Sample results exceeded benchmarks, additional control measures have been put in place.
No other samples were obtainable during August or September.

7 3Y14 Our Plant manager took Storm Water Samples on 5115114 during a flash flood. The plant had -2.2" of rain in 3 hours. The plant's runoff controls were not effective due primarily to
the volume. We have exceeded the TSS threshold by 50%. We will continue to monitor and sample the outfall on at least a monthly basis until we achieve 3 consecutive samples within
specifications.

but instead is subject to benchmarks and provisions of Part IV, Section A, including the Tiered Response Action.
7 Flow rate can be measured continuously or calculated. Flow limits for wastewater discharges to HQW waters shall be set to 50% of the Summer 7Q10 Flow as per 15A
NCAC 02B .0224, Permittees who discharge wastewater to HQW waters shall obtain a Summer 7Q10 flow and report this information to DWQ. If the permittee cannot
obtain a Summer 7Q10 flow for the receiving waters at the discharge location, the permittee shall notify DWQ and the DWQ Regional Office may require an annual flow
report on a case -by -case basis.

Due to 2 consecutive exceedances on our TSS levels during the the 1st and 1"d quarter 2013, we have instituted monthly monitoring until we achieve 3 consecutive samples
within normal TSS parameters. This facility is in low production:

No qualifying discharges in January, February or March 2014. We will continue to monitor monthly until we achieve 3 consecutive satisfactory results.
Any testing that can be performed, will be performed and reported as required.
7130114 Our Plant manager took Storm Water Samples on 5 15 14 during a flash flood. The plant had -2.2" of rain in 3 hours. The lant's runoff controls were not
effective due primarily to the volume. We have exceeded the TSS threshold by SM We will continue to monitor and sample the outfall an at least a monthly basis until
we achieve 3 consecutive samples within specifications.
