So, the package to us would be this transmittal letter, a few pages from the Joint Venture agreement previously reviewed, our Name/Ownership form, and the referenced list of facilities with each one's COC number noted. I've attached our Name/Ownership Change form and I have stepped through it trying to see how little effort you might be able to invest in it. See attached for my idea on minimum required input. This form will go to an admin person to key into the database, and although the form is repetitive to a pretty large degree compared to the communications that you and I have had, I still think the form serves to reduce our time invested in revising our database to record the transfer.
