Long Long Hill, LLC PO Box 11625 Winston Salem, NC 27116 Subject: Notice of Discrepancies/General Stormwater Permit Inspection NPDES Permit No. NCG080895 Long Hill LLC, Forsyth County Dear Mr. Long: On June 27, 2014, Sue White of this office performed a Stormwater Permit Inspection at Long Hill LLC located at 1275 Old Belt Way, Rural Hall, NC in Forsyth County. Justin Pardue was present for the inspection. This inspection was at the request of Mr. Pardue and is intended to aide in the compliance of Long Hill LLC. We evaluated five (5) areas as designated on the attached EPA inspection form. Observations from each area are addressed below: 1. Permit This facility holds General Stormwater Permit No. NCG080895 to discharge Stormwater from activities associated with Vehicle Maintenance Areas under the National Pollutant Discharge Elimination System (NPDES). The permit became effective November 1, 2012 and expires October 31, 2017. 2. Records/Reports Part 11, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan (SPPP). The SPPP was not available on -site at the time'of the inspection. A complete and updated SPPP plan and completed monitoring forms and results must be kept on -site for a period of 5 years. The contents of the SPPP plan were discussed with Mr. Pardue. 3. Facility Site Review Overall this site was clean and well managed. There are two identified Stormwater Discharge Outfalls (SDO) associated with this property. 4. Effluent/Receivin_, Waters Most of the stormwater leaves the site via the stormwater pond located on the NW corner of the property. The other is at the front of the property and covers mostly the office and parking areas. Monitoring must occur at both locations until such time as Representative Outfall Status can be established. Runoff from the site flows into an unnamed tributary to Grassy Creek, classified as Class "C waters in the Yadkin Pee -Dee River Basin, 5. Self -Monitoring Program Analytical monitoring is required per Part 11, Section B of the permit since the facility uses > 55 gallons/month of new motor oil. This sampling should be performed semi-annually during a representative storm event. The semi-annual qualitative (visual) monitoring in Part II, Section D should be performed and documented also on a semi-annual basis during a storm event. Since Mr. Pardue requested this inspection for help in becoming compliant, this inspection is considered an Notice of Discrepancy and not a Notice of Violations. Box 11625 Winston-Salem, NC 27116 Subject: Compliance Evaluation Inspection NPDES General Stormwater Permit NCG080000 Certificate of Coverage NCGO80895 Long Hill, LLC Forsyth County Dear Mr. Long: Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Quality.(DWQ.or the Division) conducted a compliance evaluation inspection (CE1) at the subject facility on November 16, 2012. The assistance and cooperation of Mr. Justin Pardue, during the inspection, was greatly appreciated. An inspection checklist is attached for your records and the inspection findings are summarized below. The facility is located at 1275 Old Belt Way, in Rural Hall, Forsyth County, North Carolina, at approximate coordinates 36.2126320,-80.2950020. Stormwater related to industrial activity is discharged from the site to Grassy Creek, which is currently classified as a Class C water located in the Yadkin Pee -Dee River Basin. Documentation & Monitoring Review The Certificate of Coverage (COC) for the site (NCG080895) was issued and became effective on July 13, 2012. As of the date of the inspection, the facility is still under construction and is not yet in operation in its industrial capacity. In accordance with Part III, Section A, Paragraph 1, of the NCG080000 General Stormwater Permit, the Stormwater Pollution Prevention Plan (SP3) should be fully developed and implemented, Qualitative and Analytical Monitoring (QM & AM) should commence, and all secondary containment requirements must be met, immediately upon commencement of industrial activity at the site. Please strive to meet these requirements to remain fully compliant with the permit. Mr. Boone has provided Mr. Pardue a template that he may be able to use to develop the SP3 for the facility. Mr. Boone has also asked Mr. Pardue to notify him immediately when construction is complete and industrial operations begin at the facility. Site Review Mr. Boone found no violations or issues on the -site during the inspection. Mr. Boone also conduction a Construction Stormwater inspection while on site. He found no issues during the construction inspection either. That inspection has been written up and summarized in a separate letter from the Division.
