On January 20, 2016 Glen White of the North Carolina Department of Environmental Quality.
Department of Energy, Minerals and Land Resources met with you at the subject facility located
at 2831 Uwharrie Road, Archdale, NC to conduct a compliance inspection for the facilities
Industrial Stormwater Permit. Inspection consist of review of the Stormwater Pollution
Prevention Plan, training and monitoring records and observation of the general condition of the
site and outfall. Your assistance with the inspection was greatly appreciated.
Permit:
This facility holds General Stormwater Permit NCGO80188 to discharge stormwater from
industrial activity associated with Vehicle Maintenance Areas under the National Pollutant
Discharge Elimination System (NPDES). The permit became effective November 1, 2012 and
expires on October 31, 2017. A copy of the current permit is required to be maintained with the
Stormwater Pollution Prevention Plan (SPPP) documents. A copy of the current permit was
available for review at the time of inspection.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. The SPPP was was well prepared, up
to date and available for review at the time of inspection. SPPP includes documentation for
employee training per the permit. No deficiencies noted.
Facility Site Review:
This facility provides Tractor -Trailer maintenance and washing. Currently operations are
conducted at outdoor parts of the facility. Under current addition/renovation, the majority of
activities will be under roof. A new indoor maintenance facility and indoor.truck wash facility
has been designed and the will soon being in use. There is a new concrete secondary
containment area that will house the gasoline/diesel fuel and truckwash wastewater. The facility
was very clean and obviously well managed. The SPPP siteplan will require updating.

Effluent Receiving Waters:
The facility has one outfall. It is located at the rear of the property in a grassed area behind the
security chain link fence. Stormwater from industrial activity discharges to an unnamed tributary
of the Uwharrie River, Class "WS-III", waters of the Yadkin Pee Dee River Basin.
.Self-MonitoringPro ram:
The facility is required to perform qualitative and analytical monitoring twice annually. Records
show that monitoring has been performed per permit requirements for the past several years. No
discrepancy noted.
The facility was found to be compliant with its stormwater permit requirements. Please include a
copy of this report in your SPPP records. Again, thank you for your time and assistance with

Comment: Mr. Stanley walked to the outfall and through the facility with the inspector. There is one outfall.
Its located at the rear of the property behind the fenced facility. It's at pit located in a small ditch
and labeled as the stormwater testing site. Silt fence barriers are installed to catch or reduce
any solids that may be present and the grassed area also acts as a barrier. Water in the small
pit was clear at inspection.

On July 22, 2010 Mike Mickey and Mike Thomas of this office met with you and David Durham in
order to perform a General Stormwater Permit Inspection at the Superior Carriers facility in Archdale, NC.
This inspection evaluated the five (5) areas designated on the attached inspection form. Observations from
each area are addressed below:
1. Permit
Superior Carriers, Inc, holds Certificate of Coverage 9NCGO80188 for the discharge of stormwater from the
site. The permit expires next on October 31, 2012.
2. Records/Reports
Part I1 Section A of the permit requires the development of a Stormwater Pollution Prevention Plan (SPPP).
A copy of the SPPP, prepared by FMS Engineering was reviewed during the inspection. This plan was
found to be thorough and addressed all items as outlined by the permit. Please note that the permit also
requires that the SPPP be reviewed and updated on an annual basis.
3. Facility Site Review
Superior Carriers has two truck maintenance buildings, a truck wash building and two secondary
containment structures (one for gas/diesel and one for truck wash wastewater). Both containment structures
were noted to be clean and free of any sheen or spilled material. Please note that Part II.A(2)(b) of the permit
requires that records be kept of any release from the secondary containment areas. These records must
document the individual making the release, a description of the accumulated stormwater and the date and
time of the release.
The release valve for the gasolineldiesel secondary containment structure was not locked. Part ILA(2)(b) of
the permit specifies that the release valve be secured with a locking mechanism. The release valve for the
wastewater secondary containment structure was secured by a lock.

4. Effluent/Receiving Waters
The facility has a single outfall located along the back fence behind the truck wash building. Maintenance
had been performed recently on the gravel ditch leading to the outfall. No concerns or problems were
observed. Stormwater leaving the property eventually reaches an unnamed tributary to the Uwharrie River.

5. Monitoring Program
Part II, Section D of the permit requires that qualitative (visual) monitoring be performed and documented
semi-annually at the facility. The inspection noted that this monitoring was being performed as required and
was last documented on 10/5/09 and 4/24/10. Qualitative records do not need to be submitted to the
Division, however, the records must be kept on file at the facility for a period of five years.
The analytical monitoring for on -site vehicle maintenance listed in Part 11, Section B of the permit is required
since Superior Carriers uses more than 55 gallons of new motor oil per month when averaged over the
calendar year. This monitoring was also performed on 10/5/09 and 4124/10. It was noted that the TSS
sample in April exceeded the 100 mg/L benchmark value triggering a Tier One response (visually re -inspect
within two weeks, evaluate possible causes, implement controls within two months and make a written
record of the 'Pier One response in the SPPP). Lynne Hays with EnviroLinc is currently working on this
response.
Please be reminded that note a Tier T%vo response is required if two consecutive sample results are above a
benchmark value for any specific parameter. This triggers monthly. monitoring until such time as three
consecutive sample results are within the benchmark range.
Additionally, any analytical monitoring conducted at the Superior Carriers outfall must be submitted to the
Division on form SW6-250-102107 no later than 30 days from the date the facility receives the sampling
data from the laboratory.
The analytical monitoring for oil water separators and/or petroleum bulk stations and terminals listed in Part
11 Section C of the permit is not applicable to the Superior Carriers facility since the oil water separator is
connected to municipal sewer and the gasoline/diesel bulk station is not "designated" as a required sampling
location.

Comment: The Stormwater Pollution Prevention Plan (SPPP) was prepared in 2006
by FMS Engineering. A cursory review of the SPPP found that it followed the outline of
Part II, Section A of the permit. The plan is updated as necessary (personell changes,
etc.). Employee training last occurred on 7-7-10.

Comment: Part II, B of the permit requires analytical monitoring if new motor oil usage
> 55 gallons per month. Superior exceeds this threshold. This monitoring was
performed on 10-5-09 and 4-24-10. The 4-24-10 TSS sample exceeded the 100 mgll
benchmark value requiring a Tier One response (visual reinspection w /in two weeks,
evaluate possible causes, implement controls within two months and make a written
record of the Tier One response in the SP PP). It is our understanding that Lynne Hays
w/EnviroLinc is currently working on the Tier One response.
The analytical sampling referenced in Part Il, C of the permit is not applicable to the
release of stormwater from the secondary containment area holding the gasoline and
diesel tanks.

Comment: The facility has a single outfall located behind the truck wash building,
Observations of the outfall did not reveal any issues.
There are two secondary containment areas. One structure holds a 10,000 gasoline
and a 15,000 gallon diesel tank with an unlocked release valve. Please note that Part
II. A(2)(b) of the permit requires that the secondary containment release devices be
secured closed with a locking mechanism. In addition, the release of accumulated
uncontaminated stormwater must be documented (individual making the release,
observations and date and time). These records m ust be kept for five years.
The other containment structure holds wastewater tanks (wastewater is hauled off -site
for treatment). This release value was locked.
The facility also has an oil water separator that serves the vehicle manintenance
buildings. This separator is connected to municipal sewer.
Wastewater generated at the covered truck wash area is pumped to the two wastewater
storage tanks for disposal

Following up on Bethany's request for comment on your question: I concur with Bethany's report on how we implement
NCG08, and as far as I know, we have consistently implemented it that way for some time.
Read what follows only if you need to know more.
Consider the following train of authority going back to the federal rules.
40CFR122.26 b 14 viii —this part of the NPDES Stormwater program qualifies captured facilities based on SIC
code — 5171(Petroleum Bulk Stations and Terminals), AND on having vehicle maintenance; and further is
qualified to include only the portion of the facility where such vehicle maintenance is conducted. Note:
'Transportation facilities classified as Standard Industrial Classification ... 5171 which have vehicle maintenance
shops, equipment cleaning operations, or airport deicing operations. Only those portions of the facility that are
either involved in vehicle maintenance... or which are otherwise identified under oaragraohs—of this section are
associated with industrial activity' , meaning that at a SIC 5171, the permit requirements pertain only to the
runoff from vehicle maintenance areas, plus any other industrial activity areas on the site (never seen an NCG08
where this last phrase has been invoked.)
• Consider EPA's March 1992 NPDES Storm Water Program Question and Answer Document which is EPA's initial
guidance resource for the current program. Question 33, p. 11: 'Are tank farms at petroleum bulk storage
stations covered by the rule? No, unless the storm water discharge from the tank farm area commineles with
storm water from anv vehicle maintenance shoes or eauipment cleaning operations on site. However. tank
farms located onsite with other industrial facilities —are included in the regulation.' So, a stand alone discharge
from a tank farm at a SIC 5171 would not be categorically covered in and of itself.
And, note that the construction of NCG08 is consistent with these citations, but expanded too, as Bethany
pointed out. Consider that the first half of the second paragraph notes that the permit applies to, '...all owners
or operators of stormwater point source discharges associated with activities that have Vehicle Maintenance
Areas... associated with activities classified _as ... Petroleum Bulk Stations and Terminals [51711 with total
petroleum storage capacity, of less than 1 million gallons.' And the second half of that paragraph, 'Other
activities... such _as point source stormwater discharges from... secondary containments structures at petroleum
storage facilities with total oetroleum storage capacity of less than 1 million gallons ... mav be designated on a
case -by, -case basis as being required to be permitted.' So, there are two additions here not found in the federal
rules cited above — the 1 MG breakpoint, and the case -by -case designation. I'm uncertain if we have a federal
rules basis for these two provisions, but they have a long history in our program.

Mike,
I need some help with this one because I think it's a tricky answer (so I'm copying Bradley and Ken).
This permit's requirements for O/W separators and secondary containment structures at these bulk terminals are kind
of weird because the front of the permit refers to those specific discharges as "other activities, not categorically
required to be permitted, ... , which may be designated on a case -by -case basis as being required to be permitted." I
think that means that not every petroleum bulk storage station under the NCG08 has to monitor these discharges,
unless the region has found or suspects a problem and "designates" that discharge as subject to our permit. You'll
notice that those types of discharges actually have limits in that permit, which is not usual for typical stormwater
discharges... so I think it does need to be designated as a special case.
Bradley or Ken, could you help corroborate that or set me straight?
My understanding is that normally those terminals are only responsible for monitoring SW discharges associated with
vehicle maintenance, unless formally directed otherwise by DWQ.

gasoline tank and a 15,000 gallon diesel tank. Does the permit required them to monitor stormwater releases semi-
annually from this containment structure? I was of the opinion that they should. They say they are not.

Mike, your interpretation of NCGOB requirements is correct. They must monitor 20 containment discharges twice per
year according to the schedule in Table 2. And note that those are enforceable limits, not benchmarks, that they must
meet in Table 4.

herewith the subject certificate of coverage to discharge under the subject state - NPDES general permit. This
permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
