Please find attached a copy of the completed Surface Water_ Inspection Report
for Industrial Sites regarding WSTA's NPDES Permit # NCG080023. Part 4 of
this report (Required Actions to be completed by Facility) contains Annotations #1 -
#7 describing corrective measures that must be completed by WSTA.
Please be advised that the Stormwater Division has established a target completion
date of June 30, 2014 for WSTA to complete all corrective measures and
provide documentation to verify WSTA's successful completion of all
corrective measures. To assist WSTA in this matter, our staff would like to meet
with WSTA staff & your stormwater compliance consultant within the next three
weeks to review and discuss WSTA's course of action in this matter.
The Stormwater Division reserves the right to enter and inspect the Winston-Salem
Transit Authority facility as often as necessary to determine compliance with permit
requirements of NPDES Permit # NCG080023. Thank you for your continued

Part I - St_ormwater Controls
1. Best Management Practices
a. List the non/structural controls employed bathe facility:
1. Vehicle service/cleaning inside structure w/ drains plumbed to sanitary sewer through oil/sand
separator.
2. Double -wall waste oil tank inside enclosed structure, waste oil recycled through 3`d party vendor.
3. Spill kits & display of facility's SOPs at interior vehicle servicing & cleaning areas.
4. Oil filters drained/crushed inside structure, exterior enclosed storage bin for pick-up by 3`d party
vendor.
5. Interior battery storage of 10 < with routine delivery & pick-up of new/old batteries by 3`d party
vendor.
b. Are the BMPs reasonable & appropriate for the facility? ❑Yes MNo
BMPs for interior bus maintenance/cleaning activities are fairly appropriate & adequate; but,
BMPs for exterior impervious areas around the maintenance building are practically non-
existent. During the inspections, heavy oil staining was observed on bus parking areas & in
stormdrain conveyances; and, during a light rain event at the 12/5/13 inspection, oil sheen was
observed on parking lot surfaces & in the stormwater runoff. Also, 57% of the monthly
analytical monitoring reports reviewed showed oil & grease concentration limit exceedences.
Additional BMP's and staff efforts are needed to address the exterior vehicle surface areas.
Some examples are: o regular facility PM/GH & inspection programs, u improved employee
training programs, a a routine inspection and cleaning program for all exterior vehicle access &
parking areas and exterior storm drainage conveyance systems, etc.

Comments
BMPs for interior vehicle service, maintenance & storage activities are satisfactory. Additional
BMPs need to be developed & implemented to address deficiencies in cleaning/maintaining of
exterior vehicle parking/storage/access areas as well as the facility's stormwater conveyance
system (SCS). Examples include, but not limited to: street sweeper, oil absorbent booms &
pads, cleaning of SCS w/ biodegradable degreaser, etc.
2. Provide a brief description of other controls that manage/prevent/minimize stormwater runoff
(e.g., erosion and sediment controls, inlet protection/control at storm drains, diversion structures):
None noted.
Part 2 — Miscellaneous

Stormwater runoff is discharged from the property's stormwater collection & drainage system
into the City of Winston-Salem's MS4 at the Western property boundary (SDO41). No non-
stormwater discharges were observed during the inspection, nor noted in past 3 years in the
qualitative inspection reports.

The WSTA facility has a single stormwater discharge outfall SD0#1 on the Western property
boundary as shown on the site map. NOTE: Directly upstream from SDO #1, off -site
drainage is currently co -mingled with WSTA's stormwater runoff.

Part 4 — Required Actions to be completed by Facility:
1. Permittee shall develop & implement an amended SPPP that shall include, at a minimum, all items as
described in Sections A & B of Part II of their current NPDES General Permit No. NCG08000, and shall be
specific to WSTA's currently performed industrial activities, as well as for any required, new, or
anticipated near -term facility or BMPs improvement(s).
The amended SPPP shall contain an updated general location map (USGS or equivalent) with all required
information, including, but not limited to: a) Indication that the facility's stormwater outfall discharges to
the City of Winston-Salem's MS4; b) The name of the ultimate receiving waters, and c) The accurate
latitude/longitude of the point(s) of stormwater discharge associated with industrial activities of the facility.
The amended SPPP shall contain an updated sitemap(s) that shall provide all required & updated
information; including, but not limited to: a) Drainage area boundaries & total contributing drainage area
for each SDO, b) The delineation of all industrial & non -industrial activity areas of the facility, c) The
industrial activities occurring in each drainage area, d) The location of each SDO & all site drainage
features & structures, e) The direction of flow within each drainage area, f) The location & type of all
stormwater BMP's, and g) The percentage of each drainage area that is impervious surface area.
4, The amended SPPP shall also address the following items:
o Complete, up-to-date & accurate narrative descriptions of all WSTA facility activities, processes and
practices (indoor & outdoor processes, dust/particulate generation, loading & unloading materials,
waste disposal, etc.) and the sources, locations and types of potential pollutants that may be present at
WSTA's stormwater discharge outfall.
o A current reasonable, relevant & effective Stormwater Management Strategy and facility operational
procedures, controls, plans, programs, policies as required by your current NPDES permit to address
noted deficiencies in the following areas:
Update of the facility's BMP Summary (Narrative & Listing) for acceptable practices used by
Vehicle Maintenance Facilities. Incorporate additional structural or non-structural BMPs to
minimize, control and periodically remove deposition of oils, grease & automotive fluids from
exterior impervious surfaces and from the facility's stormwater conveyance system (SCS). (i.e.,
street sweeping; use of oil absorbent booms @ inlets & pads and/or blankets under vehicles;
cleaning of SCS with jet truck & power washer using biodegradable degreasers, etc.).
Effective, timely and consistent interior and exterior facility & site inspection & preventative
maintenance/good housekeeping program(s) aimed specifically detection, control, removal or
elimination of interior & especially exterior sources of pollutants prior to conveyance into the
facility's stormwater collection system. (Update PM & GH Facility Inspection Checklists to
reflect program changes).
y Development of new or updated requirements, protocols and applications relating to employee's
routine work activities & facility operations and intended to reduce or prevent identified sources
of pollution.
r Effective, timely and consistent employee training program(s) to increase employee's
awareness, knowledge and use of the facility's updated SPPP, SRP, PM & GH requirements to
control, contain or eliminate pollutants, and of the potential for facility operations to pollute the
stormwater discharge from the site.
Develop appropriate monitoring schedule(s) and new SOPS for sample collection & analysis for
timely and consistent analytical/qualitative monitoring requirements at current SPO 41.
(Petition DWQ to discontinue monitoring at current Sample Point #2 where the
stormwater discharge is not truly representative of the facility's stormwater discharge,
and remove narrative & map references to Sample Point #2 from the SPPP). All
monitoring & sampling shall be performed in accordance with Established Analytical
Monitoring Test Procedures as provided for in 40 CFR 136.6, including the timely receipt of
samples at the testing lab (pH must be analyzed within 15 minutes of collection).
Narrative description and protocols for providing initial & annual updates for the facility's SPPP
& all required Signatory and/or Certification requirements for Notices, Reports, Certifications as
required by your NPDES permit (i.e. current & annual SPPP updates, non-stormwater Flow,
major spills, discharge monitoring reports, etc.).
> Narrative description and protocols for required retention of the current SPPP and all notices,
reports, records, certifications, etc. as required by your NPDES permit. The facility's current
SPPP and all associated reports, records, certifications, etc. required by your NPDES permit
shall be retained together in a convenient, easily accessible & secure location inside the facility.
5. The facility shall immediately discontinue placing used oil absorbent sweeping compounds (Oil-Dri) in the
facility's trash bin(s) for disposal. All oil absorbent products (granular clay, absorbent pads, booms, etc.) used
by the facility shall be disposed of through the existing 3`d party service vendor (Saftey-Kleen).
6. The existing exterior dumpster near the storage building has an open drain and a defective closable lid. This
dumpster shall be removed and replace with another dumpster having a sealed drain and properly fitting,
closable lid.
7. A Spill Kit shall be provided in the Storage Building in the vicinity of the battery storage area.

On September 16, 2009, Jenny Graznak of this office performed a Stormwater Permit
Inspection at Winston-Salem Transit Authority located at 1060 North Trade Street in Winston-
Salem, North Carolina. This facility holds General Stormwater Permit No. NCGO80023 under
the National Pollutant Discharge Elimination System (NPDES) to discharge stormwater
associated with activities that have Vehicle Maintenance Areas. You were present for the
inspection. This type of inspection consists of two basic parts: a review of facility files and self -
monitoring data and an inspection of the facility's storage areas and discharge points. This
particular inspection evaluated five (5) areas, and observations from each area are addressed
below:
1. Permit
The permit became effective November 1, 2007 and expires October 31, 2012. A copy of the
permit was available during the inspection.
2. Records/Reports
As required by the Permit, the facility has a complete Stormwater Pollution Prevention Plan
(SPPP) that was available for review during the inspection. A private consultant developed the
SPPP for the facility, The SPPP appeared to be thorough and complete, including a location
map of the outfalls and a site plan listing potential stormwater pollutants and best management
practices used at the site. Employee Training is conducted on an annual .basis. The last
training event occurred on January 8, 2009 and was documented in the SPPP.
3. Facility Site Review
The site has three underground storage tanks for gasoline and diesel. One is a 10,000-gallon
tank and gasoline and two are 12,000gallon tanks for diesel. Antifreeze, engine oil,
transmission fluid, and waste oil are all kept inside the maintenance building. The facility is
equipped with a wash bay fort he buses, which has floor drains piped to a large oil/water
separator and then city sewer. Oil -Dry is maintained on the premises at fueling locations in
case of small petroleum spills.

4. Effluent/Receiving Waters
The site has two Stormwater outfalls. No problems were noted with the outfalls during the
inspection. Stormwater from the property discharges to an unnamed tributary to Peters Creek
in the Yadkin Pee Dee River Basin.
5. Self -Monitoring Program_
The stormwater permit requires semi-annual qualitative and quantitative monitoring of
stormwater discharge outfalls and effluent. An on -site review of the monitoring data during the
inspection showed that all parameters were evaluated per the specifications of the permit. The
last monitoring event occurred on July, 20, 2009 during a rain event totaling 1.28 inches of
precipitation. All of the required parameters were found to be within permit limits:

Division of Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is
reissued pursuant to the requirements of North Carolina General Statute 14'-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated December
6, 1983.
The following information is included with your permit package:

familiarize yourself with all the changes in the reissued permit.
The more significant changes in the General Permit since your last Certificate of Coverage include the following:
• Part I Section A -- A new notation that the No Exposure Exclusion from permitting option may be available to
the permittee.
• Part I Section A —A new provision that facilities draining to 303(d) listed waters, or in watersheds with an
approved TMDI.,, may not be eligible for renewed coverage under the General Permit.
• Pant If Section B Table 1 -- An increased sampling frequency From once per year to twice per year.
• Part 11 Section B Table 3 -- Benchmark concentrations now replace cutoff concentrations; the elimination of
the option to avoid sampling based on cutoff concentrations.

Part l I Section B -- New provisions requiring the permittee to execute Tier One and Tier Two response
actions, based on the first benchmark exceedence (Tier One) and the second consecutive benchmark
exceedence (Tier Two). Tier Two requires that the permittee institute montlily monitoring instead of twice -
per -year monitoring, until three consecutive monitoring events show no benchmark exceedences.
Part 11 Section 13 = A new provision that four exceedences of any particular benchmark will trigger increased
DWQ involvement in the permittee's stormwater management and control actions. DWQ may direct the
permittee to apply for an individual permit, or may direct the implementation or installation of specific
stormwater control measures.
DWQ received a number of comments on a series of general permits similar to this one. A full response to
comments can be found at http://ncwatergLiality.org/su/generalpermitiiifo.litm. Some comments resulted in permit
modifications, others are addressed in the response document or Fact Sheet. A few specific requests for
clarification are addressed below.
. What is the deadline for having a revised SPPP that reflects the provisior7s of the new General Permit?
DWQ will require that the permittee have a revised, complete SPPP no later than six months after the date of
issue of that permittee's new Certificate of Coverage.
2. Must cn7cilyticeil results he by a certified lcrb? What about pH inec7suremenis? All analytical results obtained
to satisfy NPDES permit requirements must be by CPA approved methods. DWQ certification is not required
forstor»nvcrter analytical results. Analytical results for wasteivater analyses are required to be by DWQ
certified labs or analysts. Field pH measurements are subject to the same requirements as to EPA method
(stormwater and wastewater), and as to certification (wastewater only).
Your coverage under the General Permit is transferable only through the specific action of DWQ. The Division may
require modification and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by DENR, nor does
it relieve the permittee from responsibility for compliance with any other applicable federal, state, or local law, rule,
standard, ordinance, order, judgment, or decree.
