In accordance with your application for a discharge permit received on August 22, 2016, we are
forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except upon approval of the
Division of Energy, Mineral and Land Resources. The Division of Energy, Mineral and Land
Resources may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Energy, Mineral and Land Resources, the Coastal Area Management Act, or any other
federal or local government permit that may be required.
Per the requirements of the Jordan Lake Nutrient Strategy Buffer Rule, all stormwater drainage to
stream buffers, from portions of this site that have been constructed after August 11, 2009, must meet
diffuse flow requirements per 15A NCAC 2B .0267. Diffuse flow requirements are described in
Chapter 8 of the North Carolina Stormwater BMP Manual, available at:

We've received NOls for one new facility and one existing facility in your region:
New Facility -- NCG070213 Amanzi Marble and Granite in Forsyth County. This site discharges stormwater to UT to
Reedy Fork (Class WS-III;NSW). The N01 is attached. I think the owner could use some assistance identifying their
stormwater outfalls.
Existing Facility -- NCG160223 Fibercrete Preservation Technologies in Surry County. This site discharges stormwater at
one outfall to UT to Stoney Creek (Class C). The NOI is attached.
Please let me know within 30 days if the Winston-Salem Regional Office has any concerns about issuing these COCs.

Reference is made to your recent request for rescission of the subject NPDES General Permit.
On the request form you state the subject site has been sold and the permitted entity is now in bankruptcy,
and therefore it no longer requires coverage under the general permit. In accordance with your request,
NPDES General Permit NCG070012 is rescinded, effective immediately.
Please be advised that this permit rescission is based wholly upon your statement that the
activities which would require general permit coverage at the site have ceased or been eliminated.
Division staff has not visited the site to obtain independent verification. Operating a treatment facility,
discharging wastewater or discharging specific types of stormwater to waters of the State without a valid
NPDES permit will subject the responsible party to a civil penalty of up to $25,000 per day. Enforcement
action will be certain for persons that have voluntarily relinquished permit coverage when, in fact,
continuing permit coverage was necessary. If, in retrospect, you feel the site still requires permit
coverage, you should notify this office immediately. Furthermore, if in the future you wish to again
discharge to the State's surface waters, you must first apply for and receive a new NPDES permit.

Your facility is currently covered for Stormwater discharge under General Permit NCG070000. This permit expires
on March 31, 2003. The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would he eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will he expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00.
Larger penalties may he assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid Stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the f I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your Facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to s(ormwater, you can apply for the no exposure exclusion. For additional

Payment of the required annual administering and compliance monitoring fee of $80.00 for this year has not
been received for the subject permit. This fee is required by Title 15 North Carolina Administrative Code 2H.0105,.under
the authority of North Carolina General Statutes 143-215.3(a)(1), (1a) and (1b). Because this fee was not fully paid within
30 days after being billed, this letter initiates action to revoke the subject permit, pursuant to 15 ncac 2H.0105(b) (2) (k)
(4), and G.S. 143-215.1 (b) (3).
Effective 60 days from receipt of this notice, subject permit is hereby revoked unless the required Annual
Administering and Compliance Monitoring Fee is received within that time. Discharges without a permit are subject to the

is herby authorized to discharge stormwater from a facility located at:
UNARCO ROAD
MARSHVILLE, NC
UNION COUNTY
to receiving waters designated as Lick Branch in the Yadkin -Pee Dee River Basin
in accordance with the effluent limitations, monitoring requirements and other conditions set forth in Parts I,
H, III and IV of General Permit No. NCG070000 as attached.
This Certificate of Coverage shall become effective May 1, 1998.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
