On February 03, 2016 Glen White and Sue White of the North Carolina Department of
Environmental Quality - Minerals and Land Resources met with you at the subject facility
located at 317 E. Bondenhamer Street, Kernersville, NC to conduct a compliance inspection for
the facilities Industrial Stormwater Permit. A prior inspection conducted on January 30, 2014 by
Sue White found the site non -compliant with the.permit requirements. Inspection consist of
review of the Stormwater Pollution Prevention Plan, training and monitoring records and
observation of the general condition of the site and outfall. Significant progress has been made
since that date. Your cooperation and assistance with inspection was greatly appreciated.
Permit:
This facility holds General Stormwater Permit NCG070199 to discharge stormwater from
industrial activity associated with Stone, Clay, Glass & Concrete Products under the National
Pollutant Discharge Elimination System (NPDES). The permit became effective June 1, 2013
and expires on May 31, 2018. Initial site specific Certificate of Coverage was issued on June 22,
2012. A copy of the current permit is required to be maintained with the Stormwater Pollution
Prevention Plan (SPPP) documents. A copy of the current permit was available for review at the
time of inspection.
Records/Reports-
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. At the prior inspection the SPPP
does not appear to have been prepared as it could not be located. At the current inspection an
SPPP had been prepared but monitoring and training records were not complete. Significant
progress has been made. Monitoring and training records were incomplete. Please ensure that
the SPPP is updated annually. Even if there is no change to operating procedures note and
provide revision date in on the SPPP.

Facility Site Review:
This facility fabricates granite, marble and manmade material stone products. The facility was
well organized, an improvement since the last visit. Inspectors and the owners walked the site.
They were allowed to observe facility's water filtering and recycling system located at the rear of
the building. This site has one outfall at the left rear corner of the site. The outfall was dry at
time of inspection with no visible industrial pollutants.
Effluent Receiving Waters:
Effluent from this facility drain to an unnamed tributary of Reedy Fork Creek, Class WS-1II;
NSW ofthe Cape bear River Basin.
Self -Monitoring Program:
The facility is only required to perform qualitative monitoring twice annually. Please begin
monitoring at the outfall twice a year as permit requires. A current copy of the qualitative
monitoring form is included with this letter. Please make copies and complete monitoring on
this form. Keep all records in the SPPP for a minimum of 5 years.
The site was found to be compliant with its Industrial Stormwater Permit requirements with two
deficiencies to be addressed.
1. Perform Qualitative Monitoring twice annually and record in SPPP.
2. Provide training annually and a sign off sheet signed by each employee.
3. Update plan annually. Even if there is no change to operating procedures or facilities. Add
a revision note, sign and date once annually.

Dear Mr. Hage:
On January 21, 2014, Sue White of this office performed a Stormwater
Inspection at Marble Granite World, located on Bodenhamer Road in Forsyth County.
You were present for this inspection which evaluated the areas designated on the
attached EPA inspection form. Observations from each area are addressed below:
1. Permit
This facility holds General Stormwater Permit No. NCG070199 to discharge Stormwater
from activities associated with Stone, Clay, Glass & Concrete Products under the
National Pollutant Discharge Elimination System (NPDES). The permit became
effective June 1, 2013 and expires May 31, 2018.
2. Records/Reports
Part II, Section A of the permit requires the development of a Stormwater Pollution
Prevention Plan (SP3). The SP3 plan for this site was apparently done but was stored
on the hard drive. The SP3 plan should be printed and stored in a binder with
monitoring data, employee training records, etc. and updated yearly. A portion of the
inspection was used going over the SP3 plan and what was to go in it. A follow-up
inspection was scheduled on January 28th to review the plan. You were not on site at
the time of the follow-up inspection.
3. Facility Site Review
Any items that have potential to carry leachate or solids off site during storm events
must be stored under cover. There were concerns mentioned about the water holding
tank located outside occasionally overflowing and discharging into the outfall. This
would constitute an illegal discharge and be subject to continued Enforcement and the
possibility of fines that go along with it.

4. Effluent/Receiving Waters
The facility's outfall discharges into an unnamed tributary to Reedy Fork, a class WS-III;
NSW waterbody in the Cape Fear River Basin.
5. Self -Monitoring Program
Analytical monitoring is not required at this site per Part II, Section B of the permit since
the facility uses < 55 gallons/month of new motor oil.
The semi-annual qualitative (visual) monitoring in Part II, Section C is required but is not
being performed and documented on a semi-annual basis. The proper documentation
practices were also discussed during the inspection.

Division) conducted a compliance. evaluation inspection (CEI) at the subject facility on November 14, 2012.
Your assistance and cooperation during the inspection was greatly appreciated. An inspection checklist is
attached for your records and the inspection findings are summarized below.
The facility is located at 317 East Bodenhamer Street, in Kernersville, Forsyth County, North Carolina,
at approximate coordinates 36.119724°,-80.067739°. Stormwater related to industrial activity is discharged
from the site to an unnamed tributary (UT) to Reedy Fork, which is currently classified as a Class WS-III (water
supply), nutrient sensitive water (NSW) located in the Cape Fear River Basin.
Documentation & Monitoring Review
The Certificate of Coverage (COC) for your site (NCG070199) was issued and became effective on
June 22, 2012. In accordance with Part III, Section A, Paragraph 1, of the NCG070000 General Stormwater
Permit, you have until June 21, 2012, to become fully compliant with the permit.
This inspection revealed that you do not currently have a Stormwater Pollution Prevention Plan (SP3)
for the site and that no qualitative monitoring has yet been accomplished. Please strive to develop and
implement a complete SP3 in accordance with NCG070000 as soon as possible, but in no case later than June
21, 2012. Also, although you are technical not required to begin qualitative monitoring until June 21, 2012, as
required by Part ll, Section B, of the NCG070000 general permit, you are highly encouraged to do so. Doing
so should give you a head start on identifying potential site problems that you could have corrected before
June 21, 2012.
Site Review
Mr. Boone did not inspect the entire site in detail during the inspection, but it appears that the site could
use a significant amount of housekeeping. Mr. Boone reports that there was a very large amount of
equipment, supplies, materials, etc., throughout the entire site. The NCG070000 general permit does require a

housekeeping program to be established for the site, which includes regular self inspections. The basic
.purpose of this program is to help enforce a relatively clean site, thereby minimizing exposure of materials to
precipitation, which could be mobilized and carried into a receiving stream as a potential pollutant. Previous
visits to the site (earlier this year) conducted by Mr. Boone in response to citizen complaints, also revealed the
same site conditions.
Mr. Boone also reports that he has offered his professional assistance in developing your SP3 and
implementing your stormwater program. Please feel free to contact him or me at any time should you require
assistance with any aspect of developing and implementing your SP3 and stormwater program.
You are reminded that violations of the NCG070000 general permit are subject- to civil penalty

In accordance with your application for a discharge permit received on March 7, 2012, we are
forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state —
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the Division
of Land Resources, Coastal Area Management Act or any other federal or local governmental permit that
may be required.
