(DEMLR) met with you and Lee Whitaker and conducted a compliance evaluation inspection at the subject
facility on September 09, 2015. It was a pleasure to meet both of you and your assistance and cooperation
were greatly appreciated. An inspection checklist is attached for your records and the inspection findings
are summarized below.
1. Permit
This facility holds General Stormwater Permit No. NCG070122 to discharge Stormwater from activities
associated with Stone, Clay, Glass and Concrete Products and like activities under the National Pollutant
Discharge Elimination System (NPDES). The permit became effective June 1, 2013 and expires May 31, 2018.
Your Perm if'renewaI package must be received in the Central Office in Raleigh no later than 180 days prior to
the expiration date of the permit.
2. RecordsZReports
Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan (SPPP).
The plan must include all 9 sections of Part II, Section A requirements of the permit. This site is not required
to complete analytical monitoring but qualitative monitoring is required to be completed and documented
twice yearly as defined in Part Il, Section C of the permit. The plan must be updated annually and qualitative
monitoring must be conducted semi-annually to maintain compliance with the permit. The SPPP plan and
current qualitative records were available for review at the time of inspection.

3. Facility Site Review
The facility was in operation during the time of inspection. The site was clean, neat and anything stored
outside was under cover as the permit requires. Safety vest, hard hats and safety goggles were required to
be worn during the inspection.
4. Effluent/Receiving Waters
Stormwater from the site is discharged into Hamby Creek, Class "C" waters of the Yadkin Pee Dee River Basin
via large RCP's that empty into Hamby Creek at the property outfall. The RCP's were clear at the time of
inspection with no evidence of staining, sediment or odor:
S. Self -Monitoring Program
Analytical Monitoring: This site is not required to provide analytical monitoring under current conditions of
the permit. Less than 55 Gallons are used per month.
6. Qualitative Monitoring: All qualitative monitoring records were current and available for review at the
time of inspection. 'The latest monitoring event was completed by Lee Whitaker in June 2015. Records go
back several years with monitoring completed twice annually. Records do not need to be submitted to
DEMLR, however, the records must be kept on file at the facility for a period of five years.

On August 26, 2009, Gary Hudson of this office met with Russell Price to perform an
Individual Stormwater Permit Inspection at the Hydro Conduit Corp facility located on Centre
Park Blvd in Winston-Salem. This inspection evaluated the areas as designated on the
attached inspection form. Observations from each area are addressed below:
1. Permit
Hydro Conduit Corp holds an NPDES permit # NCG070122 for the discharge of stormwater
from the facility. The General Permit became effective June 1, 2008 and will expire May 31,
2013.
2. Records/Reports
Part 11, Section A of the permit requires the development of a Stormwater Pollution Prevention
Plan (SPPP). A copy of the SPPP was available for review. The plan contained all of the items
required by the stormwater permit. Please note that the permit requires the plan to be reviewed
and updated on an annual basis.
3. Facility Site Review
Observations of the outfalls at the facility did not reveal any concerns. The site appeared very
clean and well managed
4. Effluent/Receiving Waters
Stormwater runoff from the facility reaches Hamby Creek, Class "C" waters in the Yadkin/Pee
Dee River Basin.

required since the Hydro Conduit Corp plant uses less than 55 gallons of new motor oil per

Dear Permittee:
In response to your renewal application for continued coverage under stormwater General Permit NCG070000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG070000
• A copy of a Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes in the General Permit since the your last Certificate of Coverage include the following:
• Permit cover page — Coverage is expanded to include industrial activities similar to the categorically identified industries.
• Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the
permittee.
• Part II Section A — Several minor expansions and clarifications of the required content of the Stormwater Pollution
Prevention Plan have been added. Please be sure that your plan is updated to comply with the new requirements.
• Part II Section B — Qualitative monitoring must now be conducted during a representative storm event.
• Part II Section B — The permittee is required to keep a written record of his response to problems identified by his
qualitative monitoring observations.
• Part II Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require that discharges from the vehicle maintenance areas be sampled twice per year, and that the
sampling results be compared to new numerical benchmark values. (The previous cutoff concentrations have been
eliminated.)
• Part II Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require the permittee to execute Tier One and Tier Two response actions, based on the first benchmark
exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two requires that the
permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive monitoring events
show no benchmark exceedences.
• Part II Section C — This part contains a new provision that four exceedences of any particular benchmark wilt trigger

requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG070000
• A copy of a Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (OMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes in the General Permit since the your last Certificate of Coverage include the following:
• Permit cover page — Coverage is expanded to include industrial activities similar to the categorically identified industries.
• Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the
permittee.
• Part 11 Section A — Several minor expansions and clarifications of the required content of the Stormwater Pollution
Prevention Plan have been added. Please be sure that your plan is updated to comply with the new requirements.
• Part Il Section B — Qualitative monitoring must now be conducted during a representative storm event.
• Part Il Section B — The permittee is required to keep a written record of his response to problems identified by his
qualitative monitoring observations.
• Part 11 Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require that discharges from the vehicle maintenance areas be sampled twice per year, and that the
sampling results be compared to new numerical benchmark values. (The previous cutoff concentrations have been
eliminated.)
• Part 11 Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require the permittee to execute Tier One and Tier Two response actions, based on the first benchmark
exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two requires that the
permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive monitoring events
show no benchmark exceedences.
• Part II Section C — This part contains a new provision that four exceedences of any particular benchmark will trigger
increased DWQ involvement in the permittee'9 stormwater management and control actions. DWQ may direct the
permittee to apply for an individual permit, or may direct the implementation or installation of specific stormwater control
measures.

Reference is made to your recent request for rescission of the subject NPDES General Permit. On the
returned annual fee invoice you state the subject facility has been sold and therefore you no longer require
coverage under the general permit. In accordance with your request, NPDES General Pernut
NCG170122 is rescinded, effective immediately.
Please be advised that this permit rescission is based wholly upon your statement that the activities which
would require general permit coverage at the site have ceased or been eliminated. Division staff has not
visited the site to obtain independent verification. Operating a treatment facility, discharging wastewater
or discharging specific types of stormwater to waters of the State without a valid NPDES permit will
subject the responsible party to a civil penalty of up to $25,000 per day. Enforcement action will be
certain for persons that have voluntarily relinquished permit coverage when, in fact, continuing permit
coverage was necessary. If, in retrospect, you feel the site still requires permit coverage, you should
notify this office immediately. Furthermore, if in the future you wish to again discharge to the State's
surface waters, you must first apply for and receive a new NPDES permit.

Your facility is currently covered for stormwater discharge under General Permit NCG070000. This permit expires
on March 31, 2003, The Division staff is currently in the process of rewriting this permit and is scheduled to have
the permit reissued by early spring of 2003. Once the permit is reissued, your facility would be eligible for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you in
advance that your permit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and returned by October 2, 2002 in order to assure
continued coverage under the general permit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not be sent.
Failure to request renewal within the time period specified, may result in a civil assessment of at least $250.00,
Larger penalties may be assessed depending'on the delinquency of the request. Discharge of Stormwater from your
facility without coverage under a valid Stormwater NPDES permit would constitute a violation of NCGS 143-215.1
and could result in assessments of civil penalties of up to $10,000 per day.
Please note that recent federal legislation has extended the "no exposure exclusion" to all operators of industrial
facilities in any of the I I categories of "storm water discharges associated with industrial activity," (except
construction activities). If you feel your facility can certify a condition of "no exposure", i.e. the facility industrial
materials and operations are not exposed to stormwater, you can apply for the no exposure exclusion. For additional

If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
