On May 31, 2018, a site inspection was conducted for the Wade Mine facility located at 4755 Game River
Road in Cumberland County, North Carolina. A copy of the Compliance Inspection Report is enclosed for
your review. Mr. Tony Harris, General Manager and Justin Jackson, Manager, were also present during the
inspection and their time and assistance is greatly appreciated. The site visit and file review revealed that the
subject facility is covered by NPDES Stormwater General Permit NCG020000 under Certificate of Coverage
NCG020562, Permit coverage authorizes the discharge of stormwater from the facility to receiving waters
designated as UT of the Cape Fear River, a Class WS-IV waterbody in the Cape Fear River Basin.
As a result of the inspection, the facility was found to not be in compliance with the conditions of the
NCG020000 permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil
penalty assessment of up to $25,000 per day for each violation. If you or your staff has any questions,

Inspection Summary:
Melissa Joyner and Jason Gill met with Tony Harris, General Manager, at the Wade Mine. The Stormwater Pollution
Prevention Plan (SPPP) was reviewed and included the additional information requested during the previous inspection for
the Site Map. Additionally, the Plant Material Washing area and the Intake locations should be labeled on this map.
Documentation was not included in the SPPP of the annual employee training and the Annual Review with datelsignature.
The Best Management Practices (BMP's), including the silt fence outlet and sediment storage area should be inspected on
a weekly (not monthly) basis, according to the General Permit NCG020000 requirements. There are no permanent
stormwater discharge outfalls at the site. The Annual Summary Discharge Monitoring Reports (DMR)- Wastewater from
2015 through 2017 were reviewed. No discharges were noted during this time period. On these forms the Total number of
outfalls monitored is "Na". This information should be modified to indicate "one outfall" because there is a potential for there
to be an outfall if dewatering occurs.
A tour of the mine was conducted. The silt fence outlet and associated sediment storage area was in need of maintenance.
The riprap of the silt fence outlet was breached, with sediment leaving this location. An on -site natural watercourse was
nearby.

On April 11, 2017, a site inspection was conducted for the Wade Mine facility located at 4755 Game Road in
Cumberland County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your
review. Mr. Tony Harris, General Manager, was also present during the inspection and his time and
assistance is greatly appreciated. The site visit and file review revealed that the subject facility is covered by
NPDES Stormwater General Permit NCG020000 under Certificate of Coverage NCG020366. Permit
coverage authorizes the discharge of stormwater from the facility to receiving waters designated as a UT of
the Cape Fear River, a Class WS-IV waterbody in the Cape Fear River Basin.
As a result of the inspection, the facility was found to be in compliance with the conditions of the NCG020000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.

Inspection Summary:
Melissa Joyner met with Tony Harris, General Manager, at the Wade Mine. At the time of the inspection, the Stormwater
Pollution Prevention Plan (SPPP) was not available for review but was emailed to the Inspector at a later date. In reviewing
the received SPPP, the site map should include all of the components as stipulated in General Permit NCG020000,
including a North arrow, topography, a scale and direction of drainage flow on the site. Documentation was not included in
regards to annual employee training for 2015/2016 and the Monthly Visual Inspection Reports/Log for 2015/2016/2017 for
good housekeeping and preventative maintenance purposes. There should also be included in the SPPP documentation of
the Annual Review of the SPPP with a date and signature for 2015 and 2016 There are no stormwater discharge outfalls at
the site. The Waste Water Annual Discharge Monitoring Reports (DMR) were reviewed. Records were available for review for
2016 but not for the last quarter of 2015 or the first quarter of 2017. Each DMR form should be specifically dated. No off -site
discharges were indicated.
A tour of the mine was conducted. There are no stormwater discharge outfalls at this mine. At Wetland Crossing #1 erosion
was noted on a sideslope in need of stabilization with groundcover. Overail the mine appeared to be well -operated and
maintained.

'The NPDES SW and WW permit NCGO20000 covers closed -loop recycle systems, designed to exclude all stormwater run-off from
the system and operate at or below two -feet of freeboard. These systems maybe permitted by a Non -discharge Recycle System
Permit from the Aquifer Protection Section MPS), OR may instead be permitted under NCGO20000 to eliminate an additional permit.
Recycle systems that do not meet those criteria are considered discharging systems, and are subiect to the NCGO20000 discharge
permit.

Storm Water Pollution Prevention Plan
Wade Mine
I. INTRODUCTION
A. Materials Management Narrative
This operation mines sand and aggregates, which are delivered to off -site
construction projects. The aggregates are transported by truck to the plant,
stored in stockpiles located outside and fed into the plant via loader and
conveyors.
B. Plant Operations — A description of plant operations follows:
1. Production — Sand and aggregates are ribbon fed, by way of conveyor,
into the plant. Sand and aggregates are separated and stockpiled on
site. Wash water is captured and solids are allowed to settle out.
2. Process Water Management —Process water from the plant area flows
into previously mined area.

Significant Spills and Leaks -
There have been no significant spills or leaks at this location in the past
three years.
3. STORM WATER MANGEMENT PLAN
A. Feasibility of Modifying Operations -
Sand and aggregates used in this operation are stockpiled outside where they
are exposed to rain. It is judged to be impractical to cover these stockpiles.
Since the aggregates are all processed with water and required to contain less
than 2% by weight passing the 4200 sieve, there is minimal contribution of
sediments from this source.
B. Best Management Practices (BMP's)-
Best management practices are physical, structural or management practices
that prevent or reduce pollution of waters of the state. The following BMP's
are in effect at this plant:
1. Process water is channeled from the plant in to previously mine area to
allow sediment to settle out.
2. Drivers and operators are assigned a specific parking location. They
must report any fuel, hydraulic, or other fluid leaks to the plant
manager.
3. Water from the aggregate stockpiles drains into previously .mined area.
4. Spill Prevention and Response Plan
A. Potential Contaminates -
The most likely sources for contaminates are listed below:
1. Parked trucks and equipment.
2. Sand and aggregate stockpiles.

C. Spill Prevention and Countermeasure Plan
The best defense for spill prevention is training and awareness by all plant
personnel. This training should include the importance of operating procedures to
minimize potential spills and emergency responses to take in the event of a spill.
Diesel and hydraulic oil spills are also a concern with the high volume of activity we
have with our off -road trucks. Large spills will be contained with sand and properly
disposed of. The state will be notified of any fuel/oil in excess of 25 gallons.
Spills of other liquids are most likely to occur as a result of handling liquids in 5 gallon
buckets or transferring liquids from 55 gallon drains. These spills should be small enough
to be easily manageable.
5. Preventive Maintenance & Good Housekeeping -
A. Preventive Maintenance & Monthly Visual Inspection Log
Preventive maintenance (PM) includes monthly inspections and random
visual inspections of the maintenance area, plant facilities and equipment. The
purpose of PM is to locate and correct equipment problems before they occur or
cause major problems. PM will reduce truck breakdowns and plant shutdowns in
addition to reducing the risk of polluting storm water. The following lists items to
be checked and frequency:
1. Trucks and equipment will be inspected daily for fuel, oil, and
hydraulic leaks and recorded on the site inspection report.
2. Settling ponds and detention pond/pit, will be maintained in good
operating condition.
3. Secondary containment structures will be inspected as part of the
Monthly Visual Inspection Report. Reports will be maintained in
SWPPP file. Any needed repairs will be made immediately.
4. Storm Drain Outfalls will be inspected monthly as part of the Good
Housekeeping Checklist. (There are no SDO at this site)
B. Goad Housekeeping
Good housekeeping means taking the time and effort to maintain a clean, orderly,
and safe environment in the work place. These steps will help to minimize any potential
for storm water pollution in addition to promoting safety and a sense of well being at
work:
1. Trash and other waste will be picked up regularly. A covered trash receptacle
will be placed outside and emptied on a regular basis.
2. Interior floors will be kept clean and dry. Brooms, mops, and dustpans will be
stored for convenient use.
3. Empty diums and unused pallets will be disposed of.
4. The Monthly AMC Housekeeping Inspection will be performed by the plant
manager and kept on file in the SWPPP.
G. Training
Training of employees is essential to the success of the program. The following
training will be conducted:
A. Annual training for all employees will include:
1. An introduction to the SWPPP, the Storm Water Discharge Permit and
what their responsibilities are.
2. Spill response and cleanup procedures.
3. Preventive maintenance requirements.
4. The role that good housekeeping plays in the plan.
B. Follow-up training at the plant will deal with specific problems and problem
areas to include the implementation of any new BMP's.
C. Training will be continued monthly during the safety meetings.
7. Responsible Party
The Plant Manager shall be responsible for the overall coordination, development,
implementation and revision of this plan. Other responsible parties and their
responsibilities are listed in the Action List at the end of this plan.
8. Plan Amendments
This plan will be amended whenever there is a change in design, construction,
operation, or maintenance, which has a significant effect on the potential for the
discharge of pollutants to surface waters. At a minimum the plan will be reviewed and
updated on an annual basis.
9. Facility Inspections and Monitoring Inspections and monitoring will be conducted as
follows:
1. Each outfall is identified on the site plan.
2. Each outfall must be observed twice a year, once in the spring (April through
July) and once in the fall (September through November).
3. (There are no SDO at this site)
10. Implementation — Implementation of this plan will include the required
documentation of all monitoring, measurements, inspections, maintenance activities, and
training provided to employees.

On June 19, 2012, a site inspection was conducted for the Wade Mine facility located at 4755 Game Road in
Wade, Cumberland County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your
review. Mr. Tony Harris, General Manager, was also present during the inspection and his time and assistance is
greatly appreciated. Permit coverage authorizes the discharge of stormwater and/or wastewater from the facility
to receiving waters designated as a UT of Cape Fear River, a Class WS-IV waterbody in the Cape Fear River
Basin. The site visit and file review revealed that the subject facility is covered by NPDES General Permit-
NCG020000, Certificate of Coverage-NCG020366.
As a result of the inspection, the facility was found to be in compliance with the conditions of the NCG020000
General Permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.
Please be advised that violations of the NPDES General Permit are subject to a civil penalty assessment

Met with Mr. Tony Harris, General Manager. Made observations of site conditions including processing area and
excavation pits. Site has no discernible stormwater discharge outfalls. Water in excavated areas is pumped from one pit to
another. Based on review of available monitoring records for process wastewater and mine dewatering discharges, there
were no off site discharges in 2010, 2011, or 1st & 2nd quarters of 2012. A Stormwater Pollution Prevention Plan has
been developed, which appeared to contain all permit -required components. Mine site appeared to be well managed at

Inspection Summary:
Met with Mr. Tony Harris, General Manager, who provided tour of the mine facility and previous monitoring records from a
single dewatering event in 2004. Mine does not appear to have any stormwater basins or dithces to convey stormwater off
the property. There are a series of ponds that receive wash water and any groundwater that has to be pumped out of
excavation areas, which is recycled back to the processing area for material washing. Other than the records from 2004,
there were no monitoring records available for review. Mr. Harris was informed about monitoring requirements and the
need to document 'No Discharge' during monitoring periods where dewatering does not occur.

the Division of Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit
is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15,
2007 (or as subsequently amended).
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG020000
• A copy of a Technical Bulletin for the General Permit
• Two copies of the Tier 2 / Exceedence Discharge Monitoring Report (DMR) Form for Stormwater
• One copy each of the Annual Summary DMR Forms for Stormwater and Wastewater
• Two copies of the Limit Violation DMR Form for Wastewater
• Two copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater and wastewater, and it specifies your obligations with
respect to discharge controls, management, monitoring, and record keeping. Please review the new permit to
familiarize yourself with all the changes in the reissued permit. If your Certificate of Coverage (COC) restricts
authorization to discharge stormwater only (or other limited terms) based on your original application, or
subsequent modifications to your COG, you must continue to meet these requirements or contact the Stormwater
Permitting Unit to request a modification of this renewal COG.
The more significant changes in the General Permit since your last Certificate of Coverage include the following:
Part II:
• Section B — Language referencing PAMS has been changed. Flocculants may now be used if evaluated
by the Division and administered in accordance with maximum application doses and other requirements.
Part III:
• Section A — The permit now requires a Stormwater Pollution and Prevention Plan (SPPP) tailored to the
mining industry, in lieu of a Stormwater BMP Plan.
• Section A - Inactive mines may be inspected on a reduced schedule: at least once per month and after
0.5" (or more) of rain. To qualify, inactive mines must possess an "Inactive Renewal' from DLR or certify
to DWQ the site has been stabilized. The permittee must notify the DWQ Regional Office of inactive
status. Discharges from inactive mines must still be monitored on the same schedule as active mines.

• Section B — Revised provisions require that stormwater discharges be sampled twice per year (instead of
annually), and that the sampling results be compared to new benchmark values. (The previous cut-off
concentrations have been removed.)
• Section B — Revised provisions require the permittee to execute Tier One, Tier Two & Tier Three
response actions, based on the first benchmark exceedence (Tier One) and the second consecutive
benchmark exceedence (Tier Two). Tier Two requires that the permittee institute monthly monitoring
(reported to the appropriate Regional Offices) instead of twice -per -year monitoring. Monthly monitoring
shall be done until three consecutive monitoring events show no benchmark exceedences.
• Section B — This part contains a new provision that four exceedences of any benchmark will trigger
increased DWQ involvement in the permittee's stormwater management and control actions. DWQ may
direct the permittee to apply for an individual permit, institute specific monitoring, or may direct the
implementation or installation of specific stormwater control measures.
• Section B — The definition of "Representative Storm Event" (RSE) now allows 48 hours (instead of 72
hours) of little or no precipitation to precede an RSE.
• Section B — Stormwater discharges from a basin designed to contain the 10-year storm must now be
monitored.
• Sections B & C — Failure to monitor according to permit terms triggers monthly monitoring for the
remainder of the permit.
• Section C — Analytical monitoring for Total Petroleum Hydrocarbons (TPH) has replaced the Oil & Grease
parameter in the previous permit.
• Section D — Qualitative monitoring must now be conducted during the same representative storm event
as the analytical monitoring.
• Section D — The permittee is required to keep a written record of his response to problems identified by
his qualitative monitoring observations.
• Section D — If the permittee fails to respond effectively to problems identified by qualitative monitoring, the
permittee may be required by DWQ to perform corrective action.
• Section D — Qualitative ROS: Representative Outfall Status may be granted for qualitative monitoring for
some Stormwater Discharge Outfalls (SDOs).
• Section D — If mines do not respond to problems seen in visual monitoring, DWQ may now require that
mines change the monitoring frequency, apply for an individual permit, implement in -stream monitoring,
install or modify structural stormwater controls, or implement other controls.
• Sections B, C, & D — Inability to sample due to adverse weather must be reported in the SPPP and
recorded in the Annual Summary DMR.
• Section E — A lower TSS Wastewater limit for PNA waters is included in the permit.
• Section E — All mining operations are now required to monitor TSS in wastewater effluent discharges (not
only industrial sand mines). However, TSS limits only apply to industrial sand mining operations.
• Section E — Turbidity limits on effluent discharges have been removed; however, turbidity standards still
apply in the receiving waters.
• Section E — Approval of a Pumping O&M Plan may be required prior to coverage (for sites with
dewatering activities that have the potential to drain wetlands or other surface waters).
• Section E — ATC Requirements have been changed for clarity. These requirements are now outlined for
Mine Dewatering Wastewater, Process Wastewater, Overflow From Non -Closed -Loop Process Recycle
Wastewater Systems, and Overflow From A Closed -Loop Process Recycle (CLPR).
• Section E — Monitoring for Fecal coliforms in discharges to SA waters has been clarified. Fecal coliform
monitoring is now required for all process and mine dewatering wastewater discharges to SA waters.
Part IV:
• Section E — Items 1. and 2. require submittal of a new reporting form: the Annual Summary DMR. This
annual summary of the analytical monitoring results should be submitted to the DWQ Central Office by
March 1 of each year.
• Section E — Exceedences and violations must be sent to the appropriate DWQ Regional Office within 30
days of receipt from the lab.
• Section E — Results of "No Flow" or "No Discharge" must be recorded within 30 days from the sampling
period in the SPPP. "No flow" or "No Discharge" must also be reported on the Annual Summary DMR
form.

A renewal request shall consist of a letter requesting .renewal. and completion of

of Operations, 'Stewart Aggregates, Incorporated, the proposed subject
facility will be a closed loop system with no discharge to the State's
Surface Waters. Once the sand and gravel is washed, the wastewater will
flow through a series of two (2) sedimentation ponds. The primary and
secondary settling ponds have volumes of 448,800 and 1,402,500 gallons
respectively. The secondary (clear) pond will house all associated pipes
and motors necessary for make-up recycle wash water. All runoff from the
wash area will be routed back into the primary settling pond, through a
imperviously lined open channel ditch.

It is the recommendation of the Fayetteville Regional Office that
Steward Aggregates,Inc. be issued a non -discharge permit for the proposed
closed loop sedimentation pond system for processing (washing) of aggregate
materials at the Wade Mine, Cumberland county.

d. For industrial wastewater, a complete chemical analysis of the typical wastewater
must be provided. The analysis may include, but shall not be limited to, Total Organic
Carbon, BOD, COD, Chlorides, Phosphorus, Nitrates, Phenol, Total Trihalomethanes,
TCLP analysis, Total Halogenated Compounds, Total Coliforms, and Total Dissolved
Solids.
e. If lagoons are a part of ;he facilities and the recycle water is industrial, provide a
hydrogeologic description of the subsurface to a depth of 20 feet or bedrock, whichever is
less. The number of borings shall be sufficient to define the following for the area
underlying' each major soil type at the site: significant changes in lithology; the vertical
permeability of the unsaturated zone, the hydraulic conductivity of the saturated zone, and
the depth of the mean seasonal high water table.
f. Five copies of all reports, evaluations, agreements, supporting, calculations, etc., must be
submitted as a part of the specifications which are signed and sealed by a North Carolina
Professional Engineer. Although certain portions of this required submittal must be
developed by other professionals, inclusion of these materials under the signature and seal
of a North Carolina Professional Engineer signifies that he has reviewed this material and
has judged it to be consistent with his proposed design.

Dear Mr. Tout:
Following is a brief discussion of the recirculation system for the
sand and gravel processing including the evaluations. Supporting
calculations are in Attachment "A" and drawings and specifications
are in Attachment "B".
GENERAL PROJECT INFORMATION
Process water will be used to wash fine soil particles from sand
and gravel to be excavated at the Wade Mine in Cumberland County.
Operation of the processing plant will be approximately 8 hours per
day, 6 days per week.. The main components of the system will
consist of the following:
1. Gorman Pump Model 66B2-B powered by 200 H.P. Detroit
Diesel.
2. Six inch diameter pumpline.
3. Fourteen 1 1/2 inch diameter spray bars.
4. Settlement pond.
5. Clear water (source) pond.
6. Lined ditch connecting process plant to settlement pond
and settlement pond to clear water pond.
Basically, the water from the clearwater pond will be pumped to the
processing plant for sand and gravel washing. The wash water is
then conveyed by an open lined ditch to the settlement pond where
fine soil particles are settled. The overflow of the settlement
pond is conveyed by an open ditch back to the clearwater pond.
PUMP AND PUMP SYSTEM EVALUATION
Maximum operation of the pump will be approximately 500 gpm (1.11
ft. 3/s) through approximately 850 lineal feet of six inch metal
pipe. At the point of discharge, the water will be discharged
through a spray bar system having fourteen 1 1/2 inch diameter
spray bars. Each spray bar pipe is approximately 20 ft. long.
Based on the evaluation and supplied pump information, the pump and
pipe system are adequately matched. The pump will operate at
approximately 1160 RPM against a system head of approximate 120
ft. to produce a flow rate of 500 gpm. At this rate, and even if
this rate is increased, cavitation of water will not occur (NDSHA
is greater than NSPSHR).
SETTLEMENT POND AND CLEAR WATER POND EVALUATION
Based on numerous soil borings conducted on the proposed mine site,
subsurface materials typically consist of about 4 ft. to 6 ft. of
clayey soils underlain by relatively pervious sand and gravel. At
25 ft. to 35 ft. below ground surface, the soils again become more
clayey. Ground water levels were near Elevation 97 ft.
(approximately 6 ft. below ground surface)
Based on the subsurface conditions, the settlement pond base should
be established as shallow as possible so the more impermeable
clayey soils are not penetrated. By establishment of the pond as
shallow as possible, loss of water due to infiltration into the
ground is reduced. Although infiltration into the ground would not
effect groundwater quality, loss of water would result in less
water recirculated back to the clear water pond. With the

the basin will have the capability to trap for the most part all
fine soil, particles contained in the wash water (at least a
particle size of 0.007 mm) at a flow rate of-500-gpm IZRefenti.on

The base of the 01ear-Water-Pond-should be established as deep as
possible to tak advantage of groundwater recharge through the
relatively pervious sand and gravel materials, if needed. Assuming
that no water is recirculated from the settlement pond, the maximum
pumping rate (500 gpm) could be conducted for more than 8 hours
before the f.5ti`fl wide, 3070-ft llong-and-12-5 f,,i d"ee and is
depleted ofwater. During normal daily operation, recirculation
of water from the Settlement Pond can be expected to aid in
recharging the Clear Water Pond. Only during limited situations,
such as during initial startup, will recharge by groundwater be
required'until the settlement basin is filled and water begins to
discharge back to the Clear Water Pond.
LINED DITCH EVALUATION
The lined ditch nearly I. ft. square and composed of pvc or metal

materials, at a grade slope as flat as 0.005 ft./ft., can convey
the water at a depth of 4.3 inches with _a flow rate of 500 gpm.
The water velocity will be about 3.0 ft/sec. which should be
sufficient to maintain a clean channel (minimal sediment
deposition).

This permit shall be effective from the date of issuance until May 31, 1997, and shall be subject to
the conditions and limitations as specified therein. Please pay particular attention to the monitoring
requirements in this permit. Failure to establish an adequate system for collecting and maintaining the
required operational information willresult in future compliance problems.
If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have
the right to request an adjudicatory hearing upon written request within 30 days following receipt of this

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina
as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
Stewart Aggregates, Incorporated
Cumberland County
construction and operation of a 240,000 GPD wastewater recycle system consisting of primary and
secondary settling ponds having volumes of 448,000 gallons and 1,402,500 gallons respectively and all
associated valves pumps and appurtenances to serve Stewart Aggregates, Incorporated's Wade Mine with
no discharge of wastes to the surface waters, pursuant to the application received March 9, 1992 and in
conformity with the project plan, specifications, and other supporting data subsequently filed and
approved by the Department of Environment, Health and Natural Resources and considered a part of this
permit.
This permit shall be effective from the date of issuance until May 31, 1997, and shall be subject to
the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in accordance with
the conditions of this permit, the approved plans, specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes describer) in
the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable. In the event there is a desire for the facilities to change
ownership, or there is a name change of the Permittee, a formal permit request must be
submitted to the Division of Environmental Management accompanied by an application
fee, documentation from the parties involved, and other supporting materials as may be
appropriate. The approval of the request will be considered on its merits and may or may
not be approved.
5. Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted
facility has been installed in accordance with this permit, the approved plans and
specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box
29535, Raleigh, NC 27626-0535.
6. In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as
may be required by this Division, such as the construction of additional or replacement
wastewater treatment or disposal facilities.
7. The Fayetteville Regional Office, telephone number (919) 486-1541, shall be notified at
least forty-eight (48) hours in advance of operation of the installed facilities so that an in -
place inspection can be made. Such notification to the regional supervisor shall be made
during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday,
excluding State Holidays.
8. The sludge generated from these treatment facilities must be disposed of in accordance with
General Statute 143-215.1 and in a manner approved by the North Carolina Division of
Environmental Management_
9. The issuance of this permit shall not relieve the Permittee of the responsibility for damages
to surface or groundwaters resulting from the operation of this facility.
10. The facilities shall be effectively maintained and operated as a non -discharge system to
prevent the discharge of any wastewater resulting from the operation of this facility.
11. Diversion or bypassing of untreated wastewater from the treatment. facilities is prohibited.
12. Freeboard in the primary and secondary ponds shall not be less than two feet at any time.
13. Any monitoring deemed necessary by the Division of Environmental Management to insure
surface and ground water protection will be established and an acceptable sampling
reporting schedule shall be followed.
14. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to
insure proper operation of the subject facilities.
15. The Permittee or his designee shall inspect the wastewater recycle facilities to prevent
malfunctions and deterioration, operator errors and discharges which may cause or lead to
the release of wastes to the environment, a threat to human health, or a nuisance. The
Permittee shall keep an inspection log or summary including at least the date and time of
inspection, observations made, and any maintenance, repairs, or corrective actions taken
by the Permittee. This log of inspections shall be maintained by the Permittee for a period
of three years from the date of the inspection and shall be made available upon request to
the Division of Environmental Management or other permitting authority.
16. Any duly authorized officer, employee, or representative of the Division of Environmental
Management may, upon presentation of credentials, enter and inspect any property,
premises or place on or related to the recycle system at any reasonable time for the purpose
of determining compliance with this permit; may inspect or copy any records that must be
kept under the terms and conditions of this permit; or may obtain samples of groundwater,
surface water, or leachate.
17. The annual administering and compliance fee must be paid by the Permittee within thirty
(30) days after being billed by the Division. Failure to pay the fee accordingly may cause
the Division to initiate action to revoke this permit as specified by 15 NCAC 2H .0205
(c)(4).
18. Failure to abide by the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division of Environmental Management in
accordance with North Carolina General Statute 143-215.6A to 143-215.6C.

19. The issuance of this permit does not preclude the Permittee from complying with any and
all statutes, rules, regulations, or ordinances which may be imposed by other government
agencies (local, state, and federal) which have jurisdiction.
20. A set of approved plans and specifications for the subject project must be retained by the
Permittee for the life of this project.
21. Noncompliance Notification:
The Permittee shall report by telephone to the Fayetteville Regional Office, telephone
number (919) 487-1541, as soon as possible, but in no case more than 24 hours or on the
next working day following the occurrence or first knowledge of the occurrence of any of
the following:
a. Any occurrence at the wastewater treatment facility which results in the treatment of
significant amounts of wastes which are abnormal in quantity or characteristic, such
as the dumping of the contents of a basin or tank; the known passage of a slug of
hazardous substance through the facility; or any other unusual circumstances.
b. Any process unit failure, due to known or unknown reasons, that render the facility
incapable of adequate wastewater treatment such as mechanical or electrical failures
of pumps, aerators, compressors, etc.
c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-
pass directly to receiving waters without treatment of all or any portion of the
influent to such station or facility.
d. Any time that self -monitoring information indicates that the facility is not in
compliance with its permit limitations.
Persons reporting such occurrences by telephone shall also file a written report in letter
form within 15 days following first knowledge of the occurrence. This report must outline
the actions taken or proposed to be taken to ensure that the problem does not recur.
22. Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of
the wastewater treatment facilities. The operator must hold a certificate of the type and
grade at least equivalent to or greater than the classification assigned to the wastewater
treatment facilities by the Certification Commission. The Perrnittee must also employ a
certified back-up operator of the appropriate type and grade to comply with the conditions
of Title 15A, Chapter 8A, .0202. The ORC of the facility must visit each Class I facility at
least weekly and each Class 11, III, and IV facility at least daily, excluding weekends and
holidays, and must properly manage and document daily operation and maintenance of the
facility and must comply with all other conditions of Title 15A, Chapter 8A, .0202. Once
the facility is classified, the Permittee must submit a letter to the Certification Commission
which designates the operator in responsible charge within thirty days after the wastewater
treatment facilities are 50% complete.

23. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its
extension. Upon receipt of the request, the Commission will review the adequacy of the
facilities described therein, and if warranted, will extend the permit for such period of time
and under such conditions and hrrutations as it may deem appropriate.

are exposed to rain. It is judged to be impractical to cover these stockpiles.
Since the aggregates are all processed with water and required to contain less
than 2% by weight passing the 9200 sieve, there is minimal contribution of
sediments from this source.
B. Best Management Practices (BMP's)-
Best management practices are physical, structural or management practices
that prevent or reduce pollution of waters of the state. The following BMP's
are in effect at this plant:
1. Process water is channeled from the plant in to previously mine area to
allow sediment to settle out.
2. Drivers and operators are assigned a specific parking location. They
must report any fuel, hydraulic, or other fluid leaks to the plant
manager.
3. Water from the aggregate stockpiles drains into previously mined area.

C. Spill Prevention and Countermeasure Plan
The best defense for spill prevention is training and awareness by all plant
personnel. This training should include the importance of operating procedures to
minimize potential spills and emergency responses to take in the event of a spill.
Diesel and hydraulic oil spills are also a concern with the high volume of activity we
have with our off -road trucks. Large spills will be contained with sand and properly
disposed of The state will be notified of any fuel/oil in excess of 25 gallons.
Spills of other liquids are most likely to occur as a result of handling liquids in 5 gallon
buckets or transferring liquids from 55 gallon drums. These spills should be small enough
to be easily manageable.
5. Preventive Maintenance & Good Housekeeping -
A. Preventive Maintenance & Monthly Visual Inspection Log
Preventive maintenance (PM) includes monthly inspections and random
visual inspections of the maintenance area, plant facilities and equipment. The
purpose of PM is to locate and correct equipment problems before they occur or
cause major problems. PM will reduce truck breakdowns and plant shutdowns in
addition to reducing the risk of polluting storm water. The following lists items to
be checked and frequency:
1. Trucks and equipment will be inspected daily for fuel, oil, and
hydraulic leaks and recorded on the site inspection report.
2. Settling ponds and detention one_ d/pit will be maintained in good
operating condition.
3. Secondary containment structures will be inspected as part of the
Monthly Visual Inspection Report. Reports will be maintained in
S WPPP file. Any needed repairs will be made immediately.
4. Storm Drain Outfalls will be inspected monthly as part of the Good
Housekeeping Checklist. (There are no SDO at this site)
B. Good Housekeeping ,
Good housekeeping means taking the time and effort to maintain a clean, orderly,
and safe environment in the work place. These steps will help to minimize any potential
for storm water pollution in addition to promoting safety and a sense of well being at
work:
1. Trash and other waste will be picked up regularly. A covered trash receptacle
will be placed outside and emptied on a regular basis.
2. Interior floors will be kept clean and dry. Brooms, mops, and dustpans will be
stored for convenient use.
3. Empty drums and unused pallets will be disposed of.
4. The Monthly AMC Housekeeping Inspection will be performed by the plant
manager and kept on file in the SWPPP.
6. Training
Training of employees is essential to the success of the program. The following
training will be conducted:
A. Annual training for all employees will include:
1. An introduction to the SWPPP, the Storm Water Discharge Permit and
what their responsibilities are.
2. Spill response and cleanup procedures.
3. Preventive maintenance requirements.
4. The role that good housekeeping plays in the plan.
B. Follow-up training at the plant will deal with specific problems and problem
areas to include the implementation of any new BMP's.
C. Training will be continued monthly during the safety meetings.
7. Responsible Party
The Plant Manager shall be responsible for the overall coordination, development,
implementation and revision of this plan. Other responsible parties and their
responsibilities are listed in the Action List at the end of this plan.
8. Plan Amendments
This plan will be amended whenever there is a change in design, construction,
operation, or maintenance, which has a significant effect on the potential for the
discharge of pollutants to surface waters. At a minimum the plan will be reviewed and
updated on an annual basis.
9. Facility Inspections and Monitoring Inspections and monitoring will be conducted as
follows:
1. Each outfall is identified on the site plan.
2. Each outfall must be observed twice a year, once in the spring (April through
July) and once in the fall (September through November).

7. Responsible Party
The Plant Manager shall be responsible for the overall coordination, development,
implementation and revision of this plan. Other responsible parties and their
responsibilities are listed in the Action List at the end of this plan.
8. Plan Amendments
This plan will be amended whenever there is a change in design, construction,
operation, or maintenance, which has a significant effect on the potential for the
discharge of pollutants to surface waters. At a minimum the plan will be reviewed and
updated on an annual basis.
9. Facility Inspections and Monitoring Inspections and monitoring will be conducted as
follows:
1. Each outfall is identified on the site plan.
2. Each outfall must be observed twice a year, once in the spring (April through
July) and once in the fall (September through November).

10. Implementation — Implementation of this plan will include the required
documentation of all monitoring, measurements, inspections, maintenance activities, and
training provided to employees.
