Department of Minerals and Land Resources met with you, Larry Ryan and JC Coggins at the
facility located at 7720 A Hwy 158 in Stokesdale, to conduct a compliance inspection per the
Industrial Stormwater Permit requirements. Inspection consist of review of the Stormwater
Pollution Prevention Plan, training and monitoring records and observation of the general
condition of the site and outfalls. Your assistance with the inspection was greatly appreciated.
Permit:
This facility holds General Stormwater Permit NCG210425 to discharge stormwater from
industrial activity associated with Lumber and Wood Products, Except Furniture, [SIC 241
under the National Pollutant Discharge Elimination System (NPDES). The permit became
effective August 01, 2013 and expires on July 31, 2018. A copy of the current permit is required
to be maintained with the Stormwater Pollution Prevention Plan (SPPP) documents. The current
permit was available for review and was included in the SPPP.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part 11, Section A of the permit. The SPPP along with monitoring
and training records were complete and have been recently updated. No deficiencies noted.
Facility Site Review:
Endura manufactures door sill and frame components and accessories that improve durability
and longevity of the sill and frame. Endura shares the leased building with two (2) other tenants.
The facility appeared clean and well maintained. No deficiencies noted.

Effluent Receiving Waters:
Effluent from the facility drains to an unnamed tributary of Troublesome Creek via the outfall
located along the rear of the property and via the ditch located along Hwy 15 9.
Self -Monitoring Program:
This facility is only required to conduct qualitative monitoring under current operating
procedures. The building has three (3) outfalls, two (2) are accessible to Endura. Both are
monitored twice annually and were observed during inspection. One is located at front parking
lot catch basin that drains to Hwy 158. The other is along the rear property line where a ditch
and pipe empty into an unnamed channel. Monitoring is performed as required.
Endura Products was found to be compliant with the requirements of its industrial stormwater

Yeh, it seems that lots of permits issued near or when the permit is being renewed fall thru the cracks. I'll check this one
and correct.
About twenty permits that I had issued got re -sent to me as in review. I thought I had corrected all of them in BIMS
listed as assigned to me,
I do always have a list of stuff in review. This staff here is half the size it used to be. We's working on 'deco permits.
Bradley has worked with the BIMS people to try and figure out the scenarios under which changes are incorrectly
made.
Let me know if you spot anything else.

On May 15 2013, the above Certificate of Coverage (COC) was issued to Endura Products for the
discharge of stormwater from the Stokesdale facility. Below is a partial summary of NPDES General
Permit requirements. Please review the entire permit for other items that are applicable to the Endura
Products facility. As noted in the COC letter, the permit expires July 31, 2013. The next permit may
contain changes to the requirements below.
Stormwater Pollution Prevention Plan: Part II, Section A of the permit requires the development of a
Stormwater Pollution Prevention Plan (SPPP). For new facilities applying for permit coverage for the first
time, all conditions become effective immediately upon issuance of the COC. For existing facilities
already operating, the SPPP should be developed and implemented within 12 months of the effective date
of the COC. The SPPP should follow the outline specified in Section A. The SPPP may be prepared in-
house or you may retain the services of a consultant to assist you.
Analytical Monitoring Requirements: Part 13, Section B of the permit requires the semi-anxival
collection of analytical samples (See Table 1) from each outfall during a representative storm event. This
monitoring is waived if exposed piles of sawdust, wood chips, bark, mulch, etc., are stored on site for less
than seven days before removing. Sampling must occur once during the period January — June and once
during July -w December of each year (See Table 2). In addition, benchmark values are established in the
permit (See Table 3). Exceedances of benchmark values require the permittee to take certain actions per
the Tier 1, Tier 2 and Tier 3 response actions. Samples analyzed per the permit must be reported to the
Division on the proper form no later than 30 days from the date the facility receives the sampling results
from the laboratory.

Bualitative Monitoring Rcquire.ments: Part II, Section C of the permit requires that semi-annual visual
inspections of each stormwater outfall be performed during a representative storm event. These
qualitative records do not need to be submitted to the Division, however, the records must be kept on file
at the facility for a period of five years.
On -Site Vehicle and Equipment Maintenance Monitoring: The analytical monitoring specified in Part
II, Section D of the permit is only required if Endura Products uses more than 55 gallons of new motor oil
when averaged over the calendar year.
The analytical sampling and qualitative monitoring must begin immediately upon permit issuance.

As for a storm water permit, I believe we are covered by the initial permit that I have in place with Guilford county. As
part of the permitting process I had to have the dust system plans evaluated by the environmental part of the permitting
group and they signed off on it.
