This permit is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.) The following information is included with your permit package: • A new Certificate of Coverage • A copy of the stormwater General Permit • A copy of a Technical Bulletin for the General Permit • Copies of the Discharge Monitoring Report (DMR) Form • Copies of the Annual Discharge Monitoring Report Form (if applicable) • Copies of the Qualitative Monitoring Report Form The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater discharge controls, management, monitoring and record keeping. Please review the new permit to familiarize yourself with all changes in the reissued permit. Significant changes to the General Permit are outlined in the attached Technical Bulletin. Your coverage under the General Permit is transferable only through the specific action of DEMLR. Mr. Dwight Church was present for the inspection. This facility is required to apply for a NPDES Permit NCG210000, which covers the discharge of stormwater for establishments primarily engaged in Timber Products (SIC 24). At the time of the inspection an application for the stormwater permit was left on your desk at the office, along with a copy of the permit and a technical bulletin. Please complete the application and apply for the permit within ten days of receiving this letter. The following Sections of the permit arc required to be developed and implemented: Part 11, Section A - Stormwater Pollution Prevention Plan Please make a 3-ring binder or a file to contain all nine (9) elements of the plan. These will be reviewed during the inspection. Part 11, Sections B - Analytical Monitoring The monitoring will need to be performed at each location where stormwater discharged from your property. Please make a site map and locate these outfalls, otherwise known as (SDO). Samples will need to be taken, as shown in Table 1, from each outfall and tested by a certified lab. This appears to be a separate discharge There is an Outfall that receives stormwater from the sawdust operation building and a sawdust pile, which is located in a pit. This probably should have some stone before the outfall pipe. It would be helpful to keep this area clean as possible, so that the samples will meet the benchmark values.
