On August 31, 2016 Glen White ofthe North Carolina Department of Environmental Quality;
Department of Energy, Minerals, and Land Resources visited the facility located at 260
Huckleberry Ridge Rd in Crumpler, to conduct a compliance inspection. This inspection
consists of review of the Stormwater Pollution Prevention Plan, training, monitoring and spills
records as well as observation of the general condition of the site.
Permit:
This facility holds General Stormwater Permit NCG210273 to discharge stormwater from
industrial activity associated with Lumber and Wood Products, Except Furniture [SIC 241
under the National Pollutant Discharge Elimination System (NPDES). The permit became
effective August 01, 2013 and expires on July 31, 2018. The current permit was available for
review and was included in the SPPP.
Records/Reports:
This facility was required to develop and maintain a Stormwater Pollution Prevention Plan
(SPPP) in accordance with Part II, Section A of the permit. All qualitative and analytical
monitoring records are required to be maintained with the SPPP for a minimum of five (S) years.
Please update. the SPPP annually and keep all records in the SPPP to be maintained onsite.
Future inspections that verify missing records may result in a notice of deficiency.
Facility Site Review:
This facility is a sawmill. Hardwood and pine are processed here. Most of the product is used in
furniture manufacturing. Sawdust is used to fuel the boilers. The site appears to be relatively
clean and well maintained.

Effluent Receiving Waters:
Effluent from this facility drains to Nathans Creek, Class "B", Trout waters of the New River.
Self -Monitoring Program:
This facility is required to provide Qualitative Monitoring twice annually. Analytical
Monitoring is being conducted as required. The facility was found to be compliant with its

On March 11, 2010 a Compliance Evaltation Inspection was performed by George Smith, of this office. Mr. James
Lemly was present for the inspection.
Permit
The following Sections of the permit are required to be implemented by L & E Lumber Co Incorporated:
Part 1I, Section A - Stormwater Pollution Prevention Plan.
Part 11, Section B - Analytical Monitoring,
Part 11, Section C - QualitatiNre Monitoring.
Records & Reports
The Storrmvater Pollution Prevention Plan was not developed. Also, there were no monitoring reports.
There were no records for how long the mulch and sawdust were stored on site.
Facility Site Rer'iew
The facility stores mulch and sawdust for more than seven (7) days. During the inspection George Smith assigned
numbers to the Outfalls for easy identification.

Representative sampling from the following locations is required:
Outfall #001 — The stone lined ditch adjacent to Huckleberry Ridge Road.
Outfall #002 — The stormwater grate next to the building, which receives stormwater from the mulch pile.
Outfal1 #003 — The drainage ditch that receives stormwater from the sawdust pile and operations building.
Outfall #001 receives stormwater from a portion of the timber (logs) storage yard located along Huckleberry Ridge
Road.
Outfall #002 receives stormwater from storage of a mulch pile.
Outfall #003 receives stormwater from the sawdust operation building and a sawdust pile.
Self -Monitoring Program
The facility failed to develop a Stormwater Pollution Prevention Plan. This is a permit violation.
The facility failed to perform the analytical & qualitative monitoring, semi-annually, as required by the permit.
The analytical and qualitative semi-annual monitoring is required for all Three (3) outfalls. This is a permit violation
at all three (3) outfalls, and for each parameter not monitored at each outfall.
Please, be advised, these violations and any future permit violations are subject to civil penalty assessment of
up to S25,000.00 per day, per Violation, as well as other actions by the Division of Water Quality.
The analytical & qualitative sampling must be performed durinp, a representative storm event, _once for the l' six
_months of 2010, and once for the last six months of 2010. Please perform the monitoring for all three Outfalls_and
send the monitorin , results to this office. Also the Stormwater Pollution Prevention Plan must be developed, and
can be placed in a three ring binder. Please respond to this letter within 1.5 days of receipt of this notice.
