On behalf of Blue Ridge Fiberboard, Inc. (BRF), this correspondence provides the written report regarding an
accidental discharge to state waters from the BRF facility, located at 250 Celotex Drive in Danville, Virginia
(hereinafter "Facility" or "Site"). The Facility currently operates under North Carolina Pollutant Discharge
Elimination System Permit No. NCG210000.
1. A description of the nature and location of discharge:
Fire -fighting water containing ash/soot was discharged to outfall number 2, as well as to the
Andrews Branch and Hogans Creek. Both locations discharge into the Dan River.
2. The cause of discharge:
A product fire occurred outdoors that required the use of approximately 175,000 gallons of
Fire -fighting water. The product that caught fire was a pine -based wooded board.
3. The date on which the discharge occurred:
Friday, May 30, 2014 in conjunction with fire -fighting activities.
4. The length of time that the discharge continued:
Approximately four to six hours.
5. The volume of the discharge:
Approximately 175,000 gallons.
6. If the discharge is continuing, for how long is it expected to continue:
The discharge has stopped.
7. if the discharge is continuing, what will be the expected total volume of the discharge:
See item #6 above.
8. Any steps planned or taken to reduce, eliminate and prevent recurrence of the present discharge or any
future discharges not authorized by this permit:
Additional quality control measures will be implemented to prevent future product fires.

On March 9, 2010, Jenny Graznak of this office performed a Stormwater Permit
Inspection at Blue Ridge Fiberboard located at 250 Celotex Drive in Danville, Virginia. Blue
Ridge Fiberboard purchased this property formerly owned by Knight-Celotex in November 2009.
Knight-Celotex filed for Chapter 7 bankruptcy in June 2009. Knight-Celotex held General
Stormwater Permit No. NCG21011 under the National Pollutant Discharge Elimination System
(NPDES) to discharge stormwater associated with Timber Products. As Plant manager, you
were present for the inspection. This type of inspection consists of two basic parts: a review of
facility files and self -monitoring data and an inspection of the facility's storage areas and
discharge points. This particular inspection evaluated five (5) areas, and observations from
each area are addressed below:
1. Permit
The permit became effective August 1, 2008 and expires July 31, 2013. A copy of the permit
was available during the inspection. As discussed during the inspection, you have completed
our Division's Name/Ownership Change Form and are in the process of switching permit
ownership from Knight-Celotex to Blue Ridge Fiberboard.
2. Records/Reports
Since Knight-Celotex did not leave any records from the storm water permit, you do not have a
Stormwater Pollution Prevention Plan (SPPP) and will be required to create one as required by
the permit. Please refer to permit text Part II Section A for a list of the requirements of the
SPPP. As a new permitee, you will have until the end of the 2010 calendar year
(December 31, 2010) to create and implement the SPPP.
3. Facility, -Site Review
Blue Ridge Fiberboard manufactures the Celotex brand of fiberboard products for the
construction industry. The on -site wastewater plant has been abandoned since the
manufacturing process is less water -intensive now and the need for pretreatment of wastewater
no longer exists. Due to the existence of the wastewater plant, many of the storm water inlets
on site are tied in into the wastewater flow discharging to City of Danville. Therefore, very little
storm water associated with the manufacturing process discharges to waters of the State. Oils

and greases are maintained in small barrels with secondary containment. The storm drain from
this area is plumbed into wastewater. The site also has bulk asphalt tank and bulk storage of
chemicals such as clay and cornstarch for the manufacturing process. Finished product,
wrapped in plastic, is stored outside. There are also pallets and solid waste dumpsters on site.
4. Effluent/Receiving Waters
The site has two outfalls that discharge to North Carolina surface waters. Both have dams with
shut off valves in case of spill emergency. Outfall 001 is laden with cattails and requires some
light housekeeping. Outfall 002 is a muddy ditch at the rear of the property. Stormwater from
the property discharges to Dan River (Class C) in the Roanoke River Basin.
5. Self -Monitoring Program
Please note that you must immediately begin your storm water monitoring requirements as
stipulated by the permit. The stormwater permit requires semi-annual qualitative and quantitative
monitoring of stormwater discharge outfalls and effluent. Please refer to permit text Part it Section
B for the monitoring requirements. You stated during the inspection that James R. Reed &
Associates of Newport News, VA handles all laboratory work for the facility and will be handling the
storm water monitoring for the permit. You must perform your first monitoring event prior to
June 30, 2010.
Please be reminded that the failure to comply with the above requirements is a violation
of the General Permit. Under North Carolina state law, a daily civil penalty of not more than
twenty-five thousand dollars ($25,000) per violation may be assessed against any person who
violates or fails to act in accordance with the terms, conditions, or requirements of a permit.

In response to your renewal application for continued coverage under stormwater General Permit NCG210000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit has been reissued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007.
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Stormwater Permit NCG210000
• A copy of the Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes to the General Permit include the following:
• Permit cover page, second paragraph — The General Permit now may cover activities that DWQ determines to be similar in
either the process, or in the exposed materials, to the Timber Products Industry.
• Part I Section A — A new provision that facilities draining to 303(d) listed waters, or in watersheds with an approved TMDL,
may not be eligible for continued coverage at the next renewal of the General Permit.
• Part I Section B second paragraph — A new clarification that the permit does not authorize discharges that DWQ determines
are wastewaters.
• Part 11 Section A — The required contents of the Stormwater Pollution Prevention Plan have been expanded and clarified.
• Part II Section B Table 1 —A new requirement to sample stormwater discharges twice per year for those facilities with
exposed piles of certain materials remaining on site longer than seven days.
• Part II Section B Tables 2 and 3 and following — For permittees with exposed piles, there are new provisions requiring the
permittee to execute Tier One and Tier Two response actions, based on the first benchmark exceedence (Tier One) and the
second consecutive benchmark exceedence (Tier Two). Tier Two requires that the permittee institute monthly monitoring
instead of twice -per -year monitoring, until three consecutive monitoring events show no benchmark exceedences.
• Part 11 Section B — For permittees with exposed piles, a new provision that four exceedences of any particular benchmark
will trigger increased DWQ involvement in the permittee's stormwater management and control actions. DWQ may direct
the permittee to apply for an individual permit, or may direct the implementation or installation of specific stormwater control
measures.
• Part 11 Section C — Clarification that under the qualitative monitoring provisions of the permit, the permittee is obligated to
respond to repeated observations of stormwater pollution. DWQ may impose additional stormwater management
requirements if the permittee is non -responsive, or if the responses are ineffective.
Your coverage under the General Permit is transferable only through the specific action of DWQ.
