Inspection Summary
On February 7 2018 Glen White of DEMLR visited the facility as part of the follow-up response after spill reporting to
NCDEQ DWR visited the facility days prior after the facility reported a hydraulic spill and fire suppression spill that reached
the stormwater outfalls The cause of the spill was related to a fire at the facility and the required fire suppression
The spill was properly documented and clean-up was initiated immediately Fire suppression does involve heavy detergents
but the detergents breakdown quickly through dilution The major harm to humans and wildlife is skin and eye irritation
rashes Fish may be harmed No fish kill was reported Fire suppression detergents are an allowable discharge

On November 10, 2015 Glen White of the Winston-Salem Regional Office of the NC Division of Energy,
Mineral, and Land Resources (DEMLR) conducted a compliance evaluation inspection at the subject facility
located at 26SS Piney Grove Church Road Jane Mabe, Purchasing and Safety Manager assisted with
inspection Her assistance and cooperation were greatly appreciated The facility was found to be in
compliance with its permit requirements An inspection report is attached for your records and the
inspection findings are summarized below
1 Permit
This facility holds General Storm water Permit No NC210073 to discharge Storm water from activities
associated with Lumber and Wood Products Except Furniture under the National Pollutant Discharge
Elimination System (NPDES) The permit became effective August 1, 2013 and expires July 31, 2018 Your
permit renewal package must be received in the Central Office in Raleigh no later than 180 days prior to the
expiration date of the permit
2 Records/Reports
Part II, Section A of the permit requires the development of a Stormwater Pollution Prevention Plan (SPPP)
The plan must include all nine (9) subsections of Part II, Section A of the permit The SPPP plan must be
updated annually and qualitative and analytical monitoring must be conducted and properly documented
semi-annually to retain compliance with the permit Training and maintenance procedures must be current
and updated annually The SPPP Plan and monitoring records, both current and for recent years, were
maintained in two 3-ring binders and found to be complete and compliant with requirements

3 Facility Site Review
This site manufactures lumber from raw timber At the time of inspection the area had received heavy rains
for several days and being a very active site, much of the traffic areas were very muddy Ms Mabe walked
the site with the inspector and outfalls were observed
4 Effluent/Receiving Waters
Effluent from this site drains to the Dan River, Class "C", "Trout waters" of the Roanoke River Basin
5 Self -Monitoring Program
Analytical Monitoring The facility has been conducting its analytical monitoring as required twice annually
and results are kept with the SPPP Results were available for review The site, in the past had an issue with
TSS and BOD and was placed in a Tier 2 Reponse That response was lifted in 2012 and normal sampling
requirements were resumed Results have been within required parameters since that time
6 Qualitative Monitoring All qualitative records do not need to be submitted to DEMLR, however, the
records must be kept on file at the facility for a period of five years This site is required to complete
qualitative monitoring twice annually Monitoring records were current and available for review at the time
of inspection

(DWQ) conducted a compliance evaluation inspection at the subject facility on 19 September 2012 The
assistance and cooperation of Ms Jane Mabe during the inspection was greatly appreciated An inspection
checklist is attached for your records and the inspection findings are summarized below
The facility is located at 2655 Piney Grove Church Rd, in Danbury, Stokes County, North Carolina
Stormwater related to industrial activity is discharged to an unnamed tributary to Bond's Branch, which is
currently classified as Class C, trout waters in the Roanoke River Basin
Documentation and Monitoring Review
The Stormwater Pollution Prevention Plan (SP3) is complete and well implemented Required
documentation was present and the plan is being reviewed and updated annually
The site has one stormwater outfall discharge Analytical and qualitative was complete and
comprehensive Records were well organized and readily accessible This facility was placed under Tier II
sampling requirements for exceeding benchmark values after the last compliance evaluation inspection As of
the last round of sampling your facility has met the three consecutive samples within benchmark range
requirement and may return to your normal sampling regime as defined in your permit

The site was very clean and extremely well maintained The receiving waters appeared'to be in good
shape and free of indicators of pollution at the time of inspection

Stormwater inspection following; ti,imphng results conducted by ! Mabe & M A Cohan
Observations of locations 1 2, and 3 (639160 639161 &. 639162) show that a rill has
formed due to the runoff consistently going down the same track In locations 1 and 2 the
vegetation is diminished to a point that increased erosion of the `channel" is apparent
We recognise that this erosion is increasing the amount of suspended solids which also
has a direci atTect on the COD In order to correct this situation we will create a divirsion
in order to displace the rainwater Increasing the amount of vegetation in these channels
may be a necessary step

This is in response to your letter of September 1, 2008, regarding a compliance
inspection of our facility by Mr Ron Boone of your office Mr Boone identified
documentation and site inspection issues and this letter is to provide an action plan to
address these concerns
Documentation Review
A Stormwater Pollution Plan
The North Carolina Forestry Association has previously developed a
template for SP3 which is being revised to reflect the new requirements of
the Division of Water Quality The NCFA is also planning to conduct a
seminar in conjunction with the DWQ regarding the changes (expected to
occur in early November)
Our proposed action plan is to
1) Obtain the NCFA template as soon as it becomes available and
determine if it can be adapted to our situation Completion date —
November 15 (somewhat dependent upon availability of revised
NCFA information)
2) Attend the NCFA/DWQ seminar to better understand the
requirements and how best to develop and implement the SP3
Completion date --November 15
3) SP3 development and implementation Completion date —
March 31, 2009

Site Review
Your letter also listed a number of concerns trom Mr Boone s site inspection and
while the issues will be dealt with in the SP3 (and related Best Management Practices)
discussed above, this is to provide an update on actions taken to date and planned actions
in the interim period while the SP3 is in development
A Maintenance Refueling Area
1) We have cleaned the concrete pad at the refueling area and
disposed of the cleaning materials
2) We have begun cleaning the area around the hydraulic oil tank
including the removal of a small amount of soil This process is
ongoing
3) The uncovered drums containing petroleum products have been
relocated to a building where they will not be exposed to the
elements and subject to runoff
4) The empty drums have been hauled away for recycling
5) A training meeting with our truck drivers was held on the Saturday
after Mr Boone's inspection in which we discussed issues around
the refueling area We also have implemented a change in the
practice for fifth wheel lubrication which will minimize the
likelihood of contamination
6) We have identified a person to build secondary containment
barriers for the waste oil tank and the hydraulic tank located in the
refueling area The target date for completion of these barriers is
October 15
7) We have asked two local carpenters for proposals to build shelters
over the waste oil and hydraulic oil tanks to ensure that they are
not subject to the elements Target completion date for this step is
December 1

8) We have implemented a weekly inspection of the refueling area to
insure that we are maintaining this area to the required standards

The General Permit authorizes discharges of stormwater only and it specifies your obligations with respect to stormwater

the changes in the reissued permit
The more significant changes to the General Permit include the following
• Permit cover page second paragraph — The General Permit now may cover activities that DWQ determines to be similar in
either the process or in the exposed materials to the Timber Products Industry
• Part I Section A — A new provision that facilities draining to 303(d) listed waters or in watersheds with an approved TMDL
may not be eligible for continued coverage at the next renewal of the General Permit
• Part I Section B second paragraph — A new clanfication that the permit does not authorize discharges that DWQ determines
are wastewaters
• Part II Section A — The required contents of the Stormwater Pollution Prevention Plan have been expanded and clarified
• Part II Section B Table 1 —A new requirement to sample stormwater discharges twice per year for those facilities with
exposed piles of certain materials remaining on site longer than seven days
• Part II Section B Tables 2 and 3 and following — For permittees with exposed piles there are new provisions requiring the
permittee to execute Tier One and Tier Two response actions based on the first benchmark exceedence (Tier One) and the
second consecutive benchmark exceedence (Tier Two) Tier Two requires that the permittee institute monthly monitoring
instead of twice -per year monitoring until three consecutive monitoring events show no benchmark exceedences
• Part 11 Section B — For permittees with exposed piles a new provision that four exceedences of any particular benchmark
will trigger increased DWQ involvement in the permittees. stormwater management and control actions DWQ may direct
the permittee to apply for an individual permit or may direct the implementation or installation of specific stormwater control
measures
• Part II Section C — Clarification that under the qualitative monitoring provisions of the permit the permittee is obligated to
respond to repeated observations of stormwater pollution DWQ may impose additional stormwater management
requirements if the permittee is non -responsive or if the responses are ineffective

appreciated An inspection checklist is attached for your records and the inspection findings are summarized below
2 The facility is located at 2655 Piney Grove Church Road in Danbury, Stokes County North Carolina According to
our database records the facility currently discharges stormwater to Bond s Branch which is currently classified as
Class C trout waters in the Roanoke River Basin Our records reflect only one stormwater discharge outfall (SDO)
however there is no site plan and you did not know the number or location of the SDO(s) at the time of the inspection
Documentation Review
3 The facility does not currently have a stormwater pollution prevention plan (SP3) and neither qualitative nor analytical
monitoring have been conducted as required
Site Review
4 The site was generally well kept except for the vehicle maintenance refueling area The waste oil tank and another
aboveground tank, which is located near the underground diesel storage tank both require secondary containment
There was also a considerable amount of oil and other petroleum products on the ground in the vicinity of both tanks
Additionally several drums were uncovered, exposed to the elements and contained some type of petroleum product
Please note that the discharge of such substances onto the ground or into surface waters is unlawful and all spills of
this nature should always be cleaned up immediately and the contaminated materials disposed of properly and in a
timely fashion Please also note that such spills must be reported to the North Carolina Department of Environment
and Natural Resources (DENR) Development and implementation of the SP3 if done according to the permit, should
facilitate the development Best Management Practices (BMP) to address spills
5 You may wish to commission the services of a professional environmental engineering or consulting firm to help in the
development and implementation of your SP3 as required by the permit Please use your local phone book and/or the
Internet to locate a firm that can assist you It is best to ensure such a firm has experience in stormwater
management and development of SP3s Also upon your request and once the SDOs at your facility have been

NCG040000, the Division of Water Quality (DWQ) is forwarding herewith the General Permit
NCG210000 which is anew General Permit to cover most of the Timber Products Industry You
have now been given coverage under NCG210000 and your coverage under NCGO40000 is hereby
terminated This permit is issued pursuant to the requirements of North Carolina General Statute
143-215 1 and the Memorandum of Agreement between North Carolina and the US Environmental
Protection Agency dated December 6, 1983
The following information is included with your permit package
■ A copy of the stormwater general pernut NCG210000
■ A new Certificate of Coverage under general permit NCG210000
w A Stormwater Pollution Prevention Plan Certification Form This form certifies that you have
developed and implemented the Stormwater Pollution Prevention Plan (SPPP) required in your
pernut (both NCG040000 and NCG210000) This form must be completed and returned to the
Division within 30 days of receipt of this letter DO NOT send the SPPP with the signed form
■ Five copies of Analytical Morutoring forms
m Five copies of Qualitative Monitoring forms
■ A copy of a Technical Bulletin on the stormwater program which outlines program components
and addresses frequently asked questions
Your certificate of coverage is not transferable except after notice to DWQ The Division of Water
Quality may require modification or revocation and reissuance of the certificate of coverage
This permit does not affect the legal requirements to obtain other permits which may be required by
DWQ or permits required by the Division of Land Resources, Division of Air Quality, Coastal Area
Management Act or any other Federal or Local governmental permits that may be required
