Saturday that investiga-
tors are still unclear what
caused the fire at the scrap
yard but hope to know this
week.
Also, officials on Sat-
urday reported no public
health effects from the run-
off that came from the up
to 1.4 million gallons of wa-
ter poured on the fire.
Hunter said a D.H. Grif-
fin representative couldn't
tell on Friday how much
damage was caused by the
fire, which took firefight-
ers more than 12 hours to
bring under control.
The fire at 4716 Hilltop
Road was reported at 8:06
a.m., and Hunter said fire-
fighters were able to get
it under control by about
8:30 p.m. One firefighter
sprained an ankle battling
the fire. No other injuries
were reported.
Fire officials said work-
ers were moving light met-
als around on the pile when
the fire erupted.
- Firefighters poured 2,000
gallons of water a minute
on the fire, tried to smother
it with foam and pulled the
scrap pile apart to get to
the fire that appeared to be
deep inside the pile.
That water flowed to-
ward a tributary of South
Buffalo Creek, a situation
that Greensboro's water
resources department is
monitoring for acidity and
levels of dissolved metals
and -dissolved oxygen.'
Fire Battalion Chief Jim
Robinson said Saturday
that water department staff
members visited the area
earlier in the day to check
the streams around the fire
site and reported to him
that they found nothing out
of the ordinary.
"They're still following
up on any runoff issues,
Robinson said.

On September 17, 2013 the City of Greensboro Stormwater Management Division completed an
industrial compliance inspection of D.H. Griffin Wrecking Co., Inc.. Local regulations permit an
inspection of industrial facilities under authority granted by City of Greensboro Code of .
Ordinance Chapter 30, Article 12, Section 7.1 (C-S). This inspection consisted of two parts: a
review of the facility files/records and an on -site facility inspection.
The inspection was performed by Rebecca Wells, and the following deficiencies/observations
were noted during the inspection.

On May 19, 2011 an Inspection was performed by George Smith, of this office. Mr. Kent Baltzer, Scrap Yard
manager and Ms. Evelyn Walker, Safety & Environmental Engineer was present for the inspection. This facility
holds a NPDES General Permit NCG200000, which covers the discharge of stormwater for establishments
primarily engaged in Scrap Metal (SIC 5093).
The following Sections of the permit were inspected:
Part I1, Section A - Stormwater Pollution Prevention Plan
A 3-ring binder contains all nine (9) elements of the plan. The plan is being implemented in a very good
manner.
Part II, Section B - Analytical Monitoring
The analytical monitoring is performed semi-annually as described in Table 4. The 2009 and 2010 data was not
.organized in a good manner. None of the samples exceeded the benchmark value for two consecutive samples.
Please insure that the rainfall data is kept with the respective sample events.
Part II, Section C - Qualitative Monitoring
The qualitative monitoring is performed once a month, which exceeds the semi-annual monitoring requirement
as required in Table 4.

Part II, Sections D — On -Site Vehicle Maintenance Monitoring
The facility performs sampling as required per Table S. The oil & grease samples were collected in a plastic
container. These were not accepted by the lab, and will be properly sampled in glass containers.
Please insure that the annual motor oil usage is included in the records.
Facility Site Review
There are two Outfalls that discharge stonnwater from the facility.
Monthly environmental audits are performed and documented on a form, which would potentially identify any
water quality threat hazard.
The facility has at least eight (8) concrete basins that act as oil/water separators. These are maintained by
drawing any oil off the surface on a regular basis and dipping the basin periodically so that solids are kept to a
minimum.
The yard is well organized. There are areas that are designated for specific scrap metals (ferrous, nonferrous,
aluminum, copper, etc.).
There are specific holding tanks that contain fluids, such as oil and antifreeze, which is taken from used vehicles
and the oil/water separators. The tanks have secondary containment.
The inspection is satisfactory. Mr. Baltzer and Ms. Walker do a very good job implementing the stormwater
permit. The facility is very environmentally conscious. If you have any questions, or need assistance please

In response to your renewal application for continued coverage under general permit NCG200000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. Due to resource constraints,
the Division has been unable to adequately consider appropriate modifications to the permit. Therefore, the permit
has been reissued without changes from the previous permit, so all conditions remain the same. The permit term for
the reissued permit is two years, expiring on September 30, 2008. The general permit may be modified and reissued
prior to its expiration. The permit is reissued pursuant to the requirements of North Carolina General Statute 143-
215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental
Protection Agency, dated December 6, 1981

Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require

Your facility is currently covered for stormwater discharge under General Permit NCG200000. This permit expires on
October 31, 2005. The Division staff is currently in the process of renewing this permit. When a draft version is
available for notice and public comment, it will be posted on our website at http://h2o.enr,state.nc.us/su/. Once
the permit is reissued, your facility would be eligible for continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water .
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, enclosed you will find a
Permit Coverage Renewal Application Form. The application must be completed and returned by August 19, 2005 in
order to assure continued coverage under the general permit. Letters confirming our receipt of the completed
application will not be sent.

Failure to request renewal within the time period specified may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10,000 per day.
Please note that "no exposure exclusion" is now available to all operators of industrial facilities in any of the
categories of "storm water discharges associated with industrial activity," (except construction activities). If you feel
your facility can certify a condition of "no exposure", i.e. the facility industrial materials and operations are not
exposed to stormwater, you can apply for the no exposure exclusion. For additional information contact the Central
Office Stormwater Staff member listed below or check the Stormwater Permitting Unit Web Site at
http://h2o.enr.state.nc.us/su/.
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions can be found on the bottom of the form. You will be notified when
the rescission process has been completed.

The general stormwater permit for scrap metal recycling industries, NCG200000, was issued on
November 1, 2000. Since there has been a considerable time lag since your permit application was received on
April 21, 1997 this letter is being sent to ensure that the contact information that we have is correct.
Enclosed with this letter you should find a contact information update form with the information that the
Division has on your facility. Please check to see whether or not this information is correct. If any of the
information has changed from what our records show, please make the appropriate corrections in the empty
blanks to the right of the information and mail the corrected form to us by December 8, 2000. Sign and mail the
information form regardless of whether or not any changes need to be made.
Upon receipt of your information form you will be mailed a Certificate of Coverage with a new number of the
format NCG200***. If you were one of the few facilities that received coverage under the NCG030000 general
permit prior to the issuance of this permit your existing NCG030000 permit coverage will be rescinded
concurrent with the issuance of your new Certificate of Coverage.

subject certificate of coverage to discharge under the subject state - NPDES general permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection agency dated December 6, 1983.
Upon issuance of this new Certificate of Coverage your existing Certificate of Coverage, NCG030420, is rescinded
simultaneously.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have
the right to request an individual permit by submitting an individual permit application. Unless such demand is made, this
certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of Water Quality.
The Division of Water Quality may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division of Water
Quality or permits required by the Division of Land Resources, Coastal Area Management Act or any other Federal or
Local governmental permit that may be required.
