CMC Recycling (CMC) reports that it did not collect the required monthly Tier 2
analytical stormwater samples for the monitoring period of January 2014 - June 2014.
Stormwater discharges did not occur within the sampling period while the facility was
staffed and operated with personnel trained to collect stormwater samples. CMC has
reviewed its stormwater sampling requirements with facility personnel and plans to
train additional persons to collect the required samples.

Attached is the DMR report for storm water discharge samples collected
during qualifying storm events during calendar year 2011. CMC Burlington
submitted the semi-annual DMR reports to the NC Division of Water
Quality office in Raleigh, NC on January 121h and November I I" of 2011.
In addition, CMC Burlington is notifying your Department, per the Tier III
notification requirements, that during CY 2011, CMC instituted Tier lI
monthly sampling from our three outfalls. Because of the monthly sample
results from each of the three outfalls, from October 2011 to present, CMC
will continue sampling monthly, and is evaluating effective Best
Management Practice solutions for the purpose of reducing pollutant loads
from our outfalls. No rain event, which resulted in a discharge, occurred
during January 2012.

For calendar year 2010, CMC Burlington submitted the semi-annual DMR
reports to the NC Division of Water Quality office in Raleigh, NC on August
12'h, 2010 and January 12, 2011 respectively. Please accept the attached
report as the CY 2010 annual DMR report required to be submitted to the
Regional DWQ office. CMC Burlington has a new reporting tracking
system in place that will ensure this report for CY 2011 is submitted to the
Regional office by March 1, 2012.
