Inspection Summary:
1. A copy of the NPDES permit with current facility name must be available on site and maintained within the SWPPP.
2, Analytical monitoring must be conducted semi-annually from the current three outfalls (OF-1 A, OF-1 B, and OF-2).
Sample results should be handled per the requirements in the NPDES permit and a copy maintained within the SWPPP,
3. Outfall 1A appears to be the headwaters of an unnamed tributary to Monroe Creek. These headwaters appear to
originate under the parking lot of the facility and piped by culvert to the outfall sampling location. Outfall 1 B is a side pipe
that receives road runoff through stormwater inlets and may not be representative of the upstream water quality or runoff
from the facility. Outfall 2 is representative of the facility's roof, parking lot and loading areas. Therefore, it appears that an
argument could be made for representative sampling, which could reduce the number of sampling outfalls from quantitative
analytical analysis to qualitative observations.
4. The interior and exterior of the facility is clean with very limited production area or loading/unloading areas.

RE: Watts-Spindale Stormwater Background Sampling and Representative Outfall Request
Hi, Darlene,
I'm sorry it took so long to get back to you on this one. I was tied up getting ready for the public hearing in Lumberton last night.
I read through this material, and I agree that you could consider outfall 002 as representative based on the information presented and ARO's comfort with this
call. ARO should send a letter aonrovine ROS for outfall 002 (conv us please, and I'll make sure it eoes in the n_ ermit file). It also need_ s to he dnrumented in
BIMS in Events. There are events for "Representative Outfall Status Requested;' and "Representative Outfall Status Approved," and notes/comments should
reflect which outfall represents which other ones.
I do have a few questions.
First, they mention that they only sample outfalls 001, 002, 004, and 006. They monitoring 003, 005, and 007 visually. The GP requires visual monitoring at ALL
outfalls. Also, until ROS is approved, analytical sampling should apply at all outfalls as well. How did this arrangement with "split monitoring" of the seven
outfalls come about?
Unless there are any metals or biological integrity issues with the receiving waters that are being impacted by the high metals concentrations here, I think DEQ
can proceed with ROS determination and relieve sampling at the other outfalls. However, qualitative (visual) sampling should stay in effect for ALL outfalls (not
just a subset of them). This needs to be clarified in the approval letter, unless there's a reason for omitting those observations at the other outfalls? We don't
usually do that (there is an allowance for it under NCG02, and possibly for NCG12 too, but these are exceptions to the norm).
You might include a disclaimer about DEQ's ability to revisit the ROS determination if metals concentrations cause/contribute water quality standard violations
(even if they're attributable to fencing or a roof white reflective sealant).
Let me know if you have any other questions.
Have a great weekend,
Bethany

Subject: FW: Watts-Spindale Stormwater Background Sampling and Representative Outfall Request
Bethany and Laura,
This is a very thorough and interesting report done for Watts Water Technologies. I had responded to a request to assess this site related to their Tiered
sampling and for a possible ROS for the facility late last year. I communicated with y'all about my inspection and sent them a letter saying they should
temporarily continue sampling all outfalls but on a quarterly basis. As noted at the time and in this report, they had already installed some BMPs, they were
sampling for background levels, and direct rainfall. They had also researched that the coating materials used on many flat roofs in the area has metals leaching
qualities.
They did continue their research and the outcome well documents they are heavily impacted by background inputs, including a road, railroad, and other
industrial roofs with the coating materials.
I believe that they should receive ROS for the one outfall 002, which is the only outfall that is directly related to their facility. This is my recommendation.
I'm just checking in with y'all to let me know if you have any problem with this recommendation. Do I send a letter to this effect? Do you need to send it? Can I
tell them by email that they can expect to get the ROS?
I also think they should not have to continue the Tiered sampling due to the high background levels of Cu, Zn and Pb coming to the facility. Are you ok with this?
Thanks for your input!

West Rock NCG180003 needs a name change and Laura is handing this. I've also made some recommendations on
sampling outfall changes and Bethany is thinking this through. See above.
Watts Water Technologies NCG030251 with Cu and Zn issues is another matter. (I started combining these in my head)
Will respond separately.
It looks like I have to be in the office writing NOVs today so if you need to call — please do so.

Laura, I`ll let you handle the rescissions.
Darlene, as far as NCG180003, remember we have that intranet website with a very comprehensive record of all
renewals...it's here:
http://Portal.ncdenr.org/group/Ir/storrnwater-go-renewals
*You'll have to log in to access this page.*
In the PDF of NCG18 E-COC Records, I found a lengthy e-mail correspondence record for NCG180003, along with the COC
and cover letter — on pp. 5-10 of that PDF. Typically searching for the permit number is the best way to find stuff in
there, for future requests.
Hope that helps!

Subject: RE: Recission requests and NCG180003 - Old Rock Tenn
Hi Darlene,
I'll be back in touch about Watts NCG0302S1 separately. Here's guidance for West Rock:
I also conducted an inspection of the main Westrock facility. There is a new Env. Mgr. and he's trying to get up to
speed. Here's what I found and have questions on:
1. Their current permit appears to still be in the name of Rock Tenn In -Store Solutions, Inc. (CC)C
NCG180003). Was there a request for a name change? If not, can you guide him "on this process? Laura's
working on this. Thanks.
2. He does not find their permit on site (only the cover letter). Can you send this to him? .I sent you the one we
had in Rock Tenn's name; Laura will issue a new one in the changed name whemthat.s-processed. Thanks.
3. 1 have nothing in our paper or electronic files for a facility in this name or with this COC and kdon't know why it
would even be a Textile COC. Can you help me with this? I remember several calls from Rock Tenn about old
plant sites that they were closing and/or selling, and they hung on io borne COC's longer tliari Jtners I think. aut
maybe I have missed something here about the current activity you observed there... NCG17 is
Textiles? NCG18 is Furniture Manufacture... should they have a different permit? If so, Laura needs to know
the NCG18 COC is not the proper permit to revise/transfer here. But maybe this is the mix-u(a with Watts
facility we're also talking about? They are not manufacturing textiles. I must have gotten confused here.
4. There are currently 3 outfalls per their SWPPP (which is pretty complete). But I think only two .ire
representative of the facility. It sounds like you are waiting on guidance from me about this, L'ut there aren't
many details here._ if they have requested representative outfall status (ROS), ARO can evaluate that and decide
as appropriate. Or, if it's a case where some of the outfalls are exclusively draining areas of other sources (and
separate from the.facility's industrial drainage area), ARO can advise them not to sample those outfalls. Only
outfalls draining industrial activity or material storage outfalls are subject under the permit. If there are outfalls
that combine BOTH different road/other run-on + site activity/material drainage, it's tricky (and, unfortunately,
not so uncommon). They can either (1) sample further upstream before the combining of these different
stormwater flows so samples are more representative of the site, or (2) make a case for ROS so they don't even
need to sample these places where it's mixed with drainage from elsewhere. Does this help? The actual stream
begins under the parking lot and there are no drains to the outfall prior to the outfall OF1A. Where OF 1A and
OF 1B combine, they are sampling both but OF 1B is road runoff. OF 02 is runoff from the roof, parking area and
loading area. I will let them decide if they want to make an argument to request not sampling of OF1B.
5. The facility is very clean with very little minor production, but without more information I don't know what else
to say about the facility. Okay, sounds good. OK
6. The current contact is: Alan Hooper at 828-248-4921.
Please advise me on what to do next with this! He would like to know what he's supposed to be doing. Hopefully we've
covered all the bases — but let us know if not. —Bg Great. I think I have it and can send them a report and letter this
week.

Su ject: Watts ."rater Technolog;es - ^JCG 030251
I received a request to conduct an inspection at the Watts Water Technologies facility in Spindale, Rutherford County. The facility is currently under Tier III
sampling for Cu and Zn at OF 1,2 and 4 and Cu at OF 6. Given their high sample results, they brought in Hart Hickman environmental solutions firm out of
Charlotte to help them figure out how to reduce these elevated levels. They provided me with a thorough report of their analysis. Here's the summary:
1. The facility has 3 operation sections: warehouse, recovery (recycling region) and a very small and very light assembly (basically they put two parts
together with nuts and bolts).
2. They have implemented 3 key items: stormwater filter socks, adding rip rap in all eroding areas, improving grass coverage density for soil stabilization,
and internal policies of sweeping daily in the small area of assembly.
3. Metal concentrations decreased as a result of these efforts, but still remain above the benchmark levels. They have conducted two samplings since the
implementation of these items but results are only available for one of these events. The other one will be available soon.
4. They added sampling of road runoff ( Cu=.438 mg/L and Zn=.178 mg/L) and directly from the roof drains (Cu=.205 mg/L and Zn=1.89 mg/L and .338 mg/L
at another drain).
5. Outfalls in this same sampling showed Cu= .072, .005,.055, .037 and Zn=.073, .028, .196 and .117
6. Complicating factors:
a. OF002 is really the only outfall that is only related to the facility. Test results from this outfall were Cu=.0059 and Zn=.0287 (well below
benchmarks).
b. The other outfalls are heavily impacted by the railroad tracks and Watts Rd runoff. I don't think that OFs 1, 4, or 6 are representative of the
facility at all. OF 4 is runoff from Watts Rd and OFs 1 and 6 are runoff from the railroad tracks and Callahan Koon Rd.
c. This facility was a textile production facility for a long time, but this production ended about 18 years ago. There could be residual effects?
d. The whitening material used on the roof (sort of a paint) may be a source. This covering was recently put on the roof. With research efforts,
they found a study showing this material may have a Zn component. Many buildings in this area have used this product on their roofs, so there
could be a background influence. You can see this in the test results noted above.
I can provide you with this report and the results. But after inspecting the interior and exterior conditions of this facility as it's currently operating, I think they
have done what they can do to address the metals levels ireportedly they spent about $60,000 on items in #2) and that only OF 2 represents runoff from any
processing areas. I believe their sample results are heavily influenced by background levels and inputs.
I told them to keep doing what they're doing until they hear back from me. So I will wait to hear from you as to what else you need and how to proceed.
Thanks for your guidance.

Hi Tom,
Thanks for your voicemail and email. We have the company change in-house. Laura Alexander is processing the most
recent paper work (transfer to Rock Tenn), and she will let me know when that is completed (right now, our permitting
database has the owner and facility as A.G. Industries). When we finalize it, we will mail out a paper package with the
new Certificate of Coverage with Rock Tenn as the current owner.
Our on-line renewal database contains information about this permit when it was still owned by American
Greetings. So, if you stepped through the instructions below, the COC (with an effective date of September 1, 2014)
wouid show the owner prior to the August 21" name change. That's because we loaded the information to the website
earlier in the summer, before the name change request came in.
Here's what I would recommend:
(1) Don't access the COC on-line, but instead wait for the newest COC reflecting the transfer;
(2) 1 will email you,a copy of that new COC when it's been signed (in addition, a paper copy will go to the applicant
listed);
(3) DO go to the website in the message below under step 6. (http://portal.ncd_enr.org/web/Ir/storrrwater/2014-
gpss) and download the new permit package for NCG180000. That has all the new permit documents (including
monitoring forms and the General Permit in its entirety) included. That full package won't be included in the
mail out for the name change, so the facility and employees responsible for implementing the permit need that
information.
Thank you,

done with this renewal.
On August 21 2014. n name rhangrs was i5criarf fnr the rmmnany taking it to AG Inrlii<triac Inr, Than nn
August 29 the company was sold from American Greetings to Rock Tenn.
would think that Rock Tenn has either done a transfer of the Storm water permit into their name or applied
for a new Certificate of Coverage, which would have occurred on Sept 1 or soon after. If they have not, or if I
need to complete this to get the coverage for the facility, which will then be transferred to Rock Tenn, please
let me know.
I just need to make sure the permit transfer / update is handled correctly, given we have both the new permit
and the new owners for this facility.

Re: Notification of Rock Tenn- Mead Westvaco Business Combination
Permittee: ROCK TENN IN-STORE SOLUTIONS, LLC ("Permittee")
Facility Address: 376 Pine Street, Forest City, NC
Permit Number: NCG 180003
Dear Sir/Madam:
Permittee currently operates the above -referenced facility pursuant to an environmental permit
issued by your agency. Permittee is a wholly owned subsidiary of Rock -Tenn Company. The
purpose of this letter is to notify you that on or about June 1, 2015, Rock -Tenn Company will
enter into a business contibination with i'vieau`w'estvaco, a global packaging company
headquartered in Richmond, Virginia. The transaction will occur in multiple steps that will
result in Rock -Tenn Company becoming a wholly owned subsidiary of a new corporation,
Rome -Milan Holdings, Inc. Rome -Milan Holdings, Inc. will be renamed as of the date of the
transaction closing; however, the new company name has not yet been identi tied.
The transaction will not directly change the legal entity that holds the above -referenced permit,
nor will it alter the day-to-day operation of the facility. Further, we do not anticipate any
changes to our local management team or our permitted environmental emissions or discharges
as a result of the transaction. Because the legal name of Permittee will remain unchanged, we do
not believe that the transaction triggers the need for any permit transfer or name change process
at this time. If Permittee changes its legal name or adopts a trade name in the future, we will
take all steps required to ensure the new name is properly reflected on our permit.
If you have any questions about the impending transaction as it relates to the environmental
permit issued by your agency, please contact RockTenn's corporate Environmental Department
at 770-326-8139 or amulenga@rocktenn.com.
Sincerely,

We are requesting representative outfall status for outfall 2. The reason for this
request, besides similar historic test data from outfalls 1 and 2, is because any
samples collected at outfall 1 will contain water which entered our property from the
upstream creek drainage basin. This basin includes many houses, a small business,
and now a new road which crosses the creek approximately one half mile upstream
from our property line. We have no control over any pollutants which may enter this
upstream water.
As you can see from the map, the creek enters a five foot diameter pipe at headwall
1, and flows through this underground pipe to headwall 2 where it exits approximately
fifty feet from our property line. Between headwall 1 and 2, fivp underground pines
deliver out storm water to'be mixed with the creek water before it leaves the pipe.
Outfall 1 is located at headwall 2.
If you have any questions, please call me.
Sincerely,
Fred Burgin, Jr.
EHS Manager

Your letter requesting representative outfall status was received by this office on December
14, 1994. In the letter you request to perform analytical monitoring on only outfall #2 because you
feel that the sampling data from your application shows that outfalls #1 and #2 are sufficiently
similar to call outfall #2 a representative outfall.
After reviewing the sampling results the data appeared quite similar, however it is difficult
to make the determination that outfall #2 is truly representative of the entire site if there is no site
map showing drainage naths and possible pollutant exposure points. Therefore, before a
determination on representative outfall status can be made, a site map that delineates drainage areas,
exposure points, and all outfalls must be provided.
If you have any questions regarding this matter please call me at (919) 733-5083.
Sincerely,
Aisha H. Lau

AN AMERICAN GREETINGS COMPANY
Dear Mr. Edwards:
As you know, our new General Permit COC NCG 180003 was issued on
October 14, 1994; however, ;r was not receiiA at '-• ' T_- - L _ -- "
, W uL our p,a►,L untr uvGlliuer a. it was
originally mailed to the attention of our former President and was forwarded to our
corporate headquarters in Ohio where no one understood what it was. This .was not the
fault of the NCDEHNR, but simply an unfortunate occurrence. Because of this, we were
unable to secure the necessary sampling equipment to complete the required testing "during
the April -November window as specified by the permit.
I would like to do the sampling as soon as I am able to secure sampling equipment-
- probably in late December or early January. Also, I would like permission to sample
only one outfall rather than the two we sampled in our individual permit application.
Outfall # 2 is preferred because it is representative of our stormwater effluent and because
it does not have the potential for run on pollutants from upstream properties found at
outfall # 1. Attached is a copy of test results taken at both outfall # 1 and outfa11 #2 for our
original individual permit application. As you can see, there is very little difference
between the results from the two outfalls.
I will continue my efforts to locate and secure the sampling equipment, but will do
no sampling until _1 hear from vn, if ,.._,; h..o 4 --- --- "
