Dear Mr. McGuire:
. Mike Thomas of the Winston-Salem Regional Office of the NC Division of Water Quality
(DWQ) conducted a compliance evaluation inspection (CEI) at the subject facility on 8
February 2011. The assistance and cooperation of Mr. Pete Hill during the inspection was
greatly appreciated. An inspection checklist is attached for your records and the inspection
findings are summarized below.
The facility is located at 1660 US HWY 311 North, in Walnut Cove, Stokes County,
North Carolina. Stormwater related to industrial activity is discharged to an unnamed tributary
to the Dan River, which is currently classified as class WS-V waters in the Roanoke River
Basin.
Documentation and Monitoring Review
The Stormwater Pollution Prevention Plan (SP3) is complete and well implemented.
Training sessions are conducted annually to keep staff informed and up to date. Records for
Qualitative Analysis were available and appeared to be complete. Routine maintenance and
daily site inspections were also recorded .and made available for inspection. Analytical
Monitoring is not required under NCG170000.
Site Review
The facility was clean and well maintained at the time of inspection. Outfalls were easily
accessible and well maintained. A layer of sediment was present in the discharge pipe at the
time of inspection. The sediment appeared to be coming from the drop inlet located near the
North Carolina Division of water Quality, Winston-Salem Regional Office

truck unloading area. This area should have the dirt and gravel removed from around the
stormwater inlet to prevent further accumulation downstream.

In accordance with your application for a discharge permit received on October 8, 2007, we are
forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state -
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
PIease take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the
Division of Land Resources, Coastal Area Management Act or any other federal or local governmental
permit that may be required.
