Coverage is issued under the requirements of North Carolina General Statutes 143-215.1 and
the Memorandum of Agreement between North Carolina and the U.S. Environmental
Protection Agency.

Dear Forsyth County:
This letter is to inform you of the sale of the Microf bre Inc. facility located at 3801
K.imwell Drive, Winston-Salem, NC to HPFabrics, Inc, and to request the transfer of the permit
referenced above. The permit expiration date occurred on December 31, 2016, but a timely
renewal application was submitted on September 16, 2016.
In accordance with Forsyth County Air Quality Control Ordinance Section 3Q-0305(a)(4),
this letter also serves as an agreement between Microfibre Inc. and HPFabrics, Inc, that the transfer
of the facility will occur on April 11, 2017 at which time HPFabrics, Inc. shall be responsible for
compliance with the permit referenced above.
Attached to this letter are the following documents:

11. Part 11 Section A, 9 Microfibers has not implemented their SWPPP. Only one inspection has been
completed. There has been no monitoring, no maintenance activates has been documented,
training has not been conducted for their employees, and no documentation for implementing
of BMPs.
12. Part II Section B no Qualitative Monitoring has been conducted, Microfibers will need to conduct
its qualitative monitoring and present the results to the stormwater division after each
observation is recorded. -
The City -of Winsion-Salem Stormwater Division is requiring that Items #1-12 to be completed before
June 30, 2012. Microfiber Inc. shall keep the Stormwater Division updated an all milestones and
completed activities in its completion of all'the`required actions; this will be done on a monthly basis.

Part 4 — Required Actions to be completed by Facility
1. Replace the.,, plastic containment cap on the ammonium hydroxide transfer hose with a metal
containment cap.
2. Place the adhesive tank and 55 gallon drum of chemical solution towards the interior of the
facility. Ensure that -these products cannot be released outdoors.
3. Clean up the compactor area at Bay # 17 of accumulated adhesive material. In addition,
remove trash at Bay #19 and place materials into your compactor.
4. Install a plug in the Dyehouse dumpster since currently, a plug is absent.
5. Implement a structural BMP at the Dyehouse destitching area in order to permanently
contain all rinse water within your facility.
6. Add an extra line item to check for oil leaks around the whole facility, as per your Pollutant
Prevention and Good Housekeeping Program. Once discovered, Microfibres must clean up
residual oil leaks.
7. Increase the frequency of your outdoor clean-up schedule; ensure no free linen is left outside
and exposed to rainfall.
8. Update your site map to reflect all current outfalls, facility improvements, and industrial
activities areas. In addition, determine the drainage connectivity of Microfibres's
stormwater'conveyance system at the unloading dock area, Bays # 27-30. Evaluate and
locate SDO's that are characteristic to Microfibres's stormwater runoff.
9. Update the Stormwater Division's contact information on Microfibres's Spill Notification
List.
10. Fix leaking.gutters on the northern side of 3811 Kimwell Drive Building. The leaking
gutters are causing gulley and reel erosion near 0utfall # 1 B, which is causing off -site
sedimentation to receiving waters.
11. Make the building foundation watertight at the Compound Mixing area. Currently, clay
residue and ;hydrocarbon sheen are being released to open environment.
12. Clean up residual nylon fibers and trash at the Dyehouse dumpster area.

This is to follow up our meeting and the site visit that was conducted on July 20, 2006 at Microfibres on Kimwell
Dr in Winston Salem, NC. After having toured the exterior of the facility last Thursday I wanted to get back to you
about your request for No Exposure Certification, At this time we would not recommend this facility for No
Exposure Certification. Some of the issues noted during the inspection are:
1. Illicit discharges- The eye wash fountain and second pipe of unknown origin out of the 3760 dye house
building must be plumbed into sanitary drains. Additionally an air compressor line was leaking oil onto
the ground at the side of POTW building. (Please correct these immediately).
2. Equipment and materials storage outside the 3831 building- Disposal, cover or removal of the scrap metal
and miscellaneous machinery behind this building would be necessary.
3. Building 3811- The partially exposed drainage trough to POTW at the ammonia tank where chemicals are

stormwater exposure.
4. Open dumpsters noted around the facility should have covers or lids.
At this time the facility does not hold a stormwater permit as required by the Division of Water Quality. The
Division thanks you for taking the initiative to correct this issue. Please make application for coverage in the
next 30 days for General Permit NCG170000 for stormwater discharges associated with activities classified as
SIC 22 Textile Mill Products. The Notice of Intent and permit text can be found at the Division web site.

Alternately your facility can make the above recommended changes to qualify for a No Exposure Certification and
